Liver transplantation for patients with alcoholism and end-stage liver disease.

Liver transplantation for alcoholic cirrhosis remains controversial. In particular, criteria for the selection of patients who will remain recovered from alcoholism post-transplant require better definition. We analyzed the long-term predictive value of categorizing transplant referral patients with alcoholism and end-stage liver disease into risk groups for recidivism and noncompliance. Forty-seven patients with the diagnosis of alcoholism and advanced liver disease were evaluated and placed into predefined risk groups (low-, moderate-, and high-risk) for recidivism and noncompliance. No abso…

Is alcoholism more often the cause or the result of depression?

The high rate of comorbidity between major depressive disorder and alcohol abuse is noted and the possible causes of the association are examined. Three reasons for the high degree of comorbidity are discarded--underlying dependent personality disorder and/or childhood deprivation, a common hereditary defect, and self-medication with alcohol for primary depression. Alternative explanations are favored, namely, the chance simultaneous occurrence and subsequent oversampling of two common disorders, and the fact that alcohol abuse is often the "horse" to the "cart" of major depressive disorder. Of study subjects who completed the SMAST, 49% attained scores suggesting potential or probable alcoholism, and 20% had already sought some type of treatment, despite their young age. Parental alcoholism: an examination of male and female alcoholics in treatment.

This study investigated the implications of parental alcoholism in 639 male and female alcoholics who were in treatment. In comparison with persons who did not report a parental history of alcoholism (PA-), the 264 offspring of alcoholics (PA+) were characterized by higher levels of alcohol dependency and alcohol-related consequences, increased reports of use of other drugs and higher levels of MMPI-measured psychopathology. Comparisons of PA+ patients with PA- patients with the same age of onset of problematic alcohol use suggested that PA+ persons are characterized more by an early onset of …

[Clinical aspects, epidemiologic progression of fetal alcoholism: a current daily problem].

Foetal and neonatal consequences of exogenous intoxications are nowadays better apprehended especially regarding alcohol. Alcoholism during pregnancy is the cause of the syndrome of foetal alcoholism described in France by Lemoine in 1968 and then by Jones and Smith in 1973. The prevalence of the table is 1 out of 700 to 800 deliveries for the severe forms and concerns especially by not exclusively the most underprivileged sections of the population and shows in: -- a very specific dysmorphy of skull and face and other abnormalities which give to the face a singular and persistent aspect; -- v…

Alcoholism among homeless adults in the inner city of Los Angeles.

The prevalence, course, and severity of alcoholism were examined in a probability sample of homeless adults in Los Angeles' inner city and in a household sample (drawn from two communities in Los Angeles), matched to the demographic characteristics of the homeless sample. Both lifetime and current prevalence of alcoholism (as measured by the National Institute of Mental Health's Diagnostic Interview Schedule) was substantially higher among homeless individuals. Moreover, compared with household alcoholics, homeless alcoholics were characterized by (1) a substantially higher prevalence of other…

Efficacy of an alcoholism self-report questionnaire in a residency clinic.

The efficacy of the CAGE alcoholism screening test, when used as a part of a much larger health and social history questionnaire, was assessed in a family medicine residency clinic by retrospectively studying 155 charts of randomly selected adult patients. The rate of problem drinking, as determined by the CAGE, was then compared with the rate found in other studies using the CAGE questionnaire. The CAGE responses were also compared with other information in the patients' charts to assess how frequently this screening test might support a diagnosis of problem drinking. The incidence of problem…

Family history of alcoholism does not predict neuropsychological performance in alcoholics.

We examined the relationship of history of alcoholism in first-degree relatives to neuropsychological performance of alcoholics abstinent from several weeks to several years. Eighty-four men were assigned to four groups based on "strength" of family history of alcoholism. The groups were: (1) "strong history," a parent plus another first-degree relative positive; (2) "moderate," parent only positive; (3) "weak," nonparent first-degree relative only positive; and (4) "negative," no first-degree relative positive. There were no significant between-group differences in NP performance. In other an…

[Importance of alcoholism markers in the diagnosis of alcoholic heart lesions].

A study was made of opportunities for the use of some signs of alcoholic intoxication in the verification of alcoholic etiology of heart lesion. Clinical signs like hyperemia of the face with telangiectasia, venous plethora of the eyeballs, tremor of the lips, tongue, limbs, Dupuytren's contracture, enlarged liver size combined with a positive macrocytosis test and, to a lesser degree, with a higher activity of gamma-glutamyl transpeptidase, and the detection of fatty hepatosis in liver puncture biopsy were shown to suggest alcoholic intoxication, and excluding other cases of heart lesion they

Disorders of divalent ions and vitamin D metabolism in chronic alcoholism.

This chapter reviews the pathogenesis of disordered divalent mineral and bone metabolism in alcoholism, emphasizing the role of impaired vitamin D physiology. Normally, vitamin D metabolites are derived principally from cholecalciferol, which is synthesized in the skin via the energy of sunlight. Most alcoholics have subnormal levels of 25-hydroxyvitamin D [25(OH)D]. Those with Laennec's cirrhosis also have low levels of vitamin D binding protein due to impaired hepatic protein synthesis and as a result, have low serum concentrations of total, but not free, 1,25-dihydroxyvitamin D. The causes …

Alcoholism in men patients subtyped by family history and antisocial personality.

Alcoholism is known to be a familial disorder and the presence of a family history of alcoholism is recognized as an important variable in alcoholism research. The course and consequences of alcoholism in a group of men alcoholics are examined in relation to family pedigree for alcoholism. Consideration was also given to antisocial personality, a variable that may underlie and contribute to a positive family history of alcoholism. The presence of antisocial personality affected the course of alcoholism to a greater extent than having a positive pedigree for alcoholism. On the other hand, havin…

Experimental studies on the influence of male alcoholism on fetal development.

To study the effect of paternal chronic ethanol consumption on fetal development, an experimental rat model was established and compared to … velopment was shown clearly and was not related to paternal ethanol. The adverse effect of paternal alcoholism on the fetus was shown in decreased litter size, or decreased body weight, cerebral weight and cerebral DNA, RNA and leucine incorporation into protein without a decrease in the litter size. Alcoholism, nonprescription drug and hepatotoxicity. The risk from unknown acetaminophen ingestion.

An alcoholic patient developed severe hepatitis and renal failure after ingesting the contents of a 300 ml. bottle of Nyquil for its ethanol content. This illness was presumably related to the 6 gm. of acetaminophen contained in this nonprescription preparation. Although hepatic damage with or without renal failure is usually described after a massive overdose of acetaminophen, the changes in drug metabolism induced by chronic ethanol consumption may increase the risk for acetaminophen toxicity. The large number of nonprescription acetaminophen-ethanol preparations available should be consider

[Epidemiological aspects of the foetal alcoholism syndrome. 45 cases (author's transl)].

During a prospective study conducted in Roubaix, 45 neonates presenting with the craniofacial malformation characteristic of the foetal alcohol syndrome were identified over a 3-year period. Twelve had signs of severe alcoholic embryo-foetopathy. This condition, which affects one in 700 neonates, occurs in multiparous women (mean age, 33 years) of low socio-economic status suffering from long standing, complicated alcoholism. Although the mean ethanol blood level may be as high as 1.82 g/l, this substance does not seem to be the only teratogenic agent.
