High resolution Tc-99m HMPAO SPECT in a patient with transient global amnesia.

This is a report of a patient who underwent high-resolution brain perfusion SPECT studies during and after an episode of transient global amnesia. During the attack, SPECT imaging showed increased perfusion in the left medial temporal region involving the amygdala and hippocampus. After recovery from amnesia, a follow-up SPECT examination showed slightly decreased perfusion in this region. These findings support the hypothesis that transient global amnesia is associated with transient hyperperfusion in the medial temporal brain structures, and confirm the utility of high-resolution SPECT imagi

A model-based study of learning and memory following transient global amnesia attacks.

Verbal learning and forgetting were studied in patients one month after an episode of Transient Global Amnesia and in normal control subjects by means of a two-stage stochastic model, which allows independent measurements of encoding, storage and forgetting. We preliminarily ascertained both the necessity and the sufficiency of the model to account for several performance scores of the two experimental groups, and then compared the learning and forgetting functions between groups. In spite of the analytical power of the statistical method adopted. Transient Global Amnesia was not found to enta

Transient global amnesia: neuropsychological dysfunction during attack and recovery in two "pure" cases.

Two patients with transient global amnesia are reported. Comprehensive neuropsychological evaluation, during the amnesic episode, as well as follow-up examinations on memory were performed. The course of the amnesia was exemplified by two comparable memory tests in different modalities. Partial retrograde amnesia and complete anterograde amnesia were demonstrated during the transient episode. Objective recovery was found to be slower than subjectively experienced, but test performance was completely normal one month after onset.

Transient Global Amnesia (TGA): Is It Really Benign? A Pilot Study on Blood Biomarkers.

We aimed to determine whether transient global amnesia (TGA) is associated with alterations in central nervous system (CNS) injury biomarkers-serum neurofilament light chain (sNfL) and serum glial fibrillary acidic protein (sGFAP). In a prospective cohort of TGA patients, blood samples were obtained within 24-48 h of TGA onset (t0) and 6 weeks thereafter (t1). We assessed sNfL and sGFAP levels using the highly sensitive single-molecule array assay and calculated Z-scores adjusted for age, gender, and body mass index (BMI). Demographics, electroencephalography (EEG), and cerebral magnetic reson

MRI in the Diagnosis of Transient Global Amnesia: A Case Series and Review of Current Evidence.

Transient global amnesia (TGA) is a common acute amnestic syndrome characterised by sudden-onset predominant anterograde amnesia lasting up to 24 hours. Patients with TGA frequently ask repetitive questions reflecting disorientation and may have some degree of inability to recall general or personal information (retrograde amnesia) while the episode lasts. During the episode of TGA, other cognitive functions are normal. Episodes are self-limited and, by definition, resolve within 24 hours, with recovery of memory function symptoms, except for what happened during the episode. Although diagnosi

Dizziness and spatial disorientation in patients with transient global amnesia: clinical and vestibular correlates.

OBJECTIVE: To determine the prevalence and clinical significance of dizziness and spatial disorientation in patients with transient global amnesia (TGA), and to examine their relationships with vestibular function and imaging findings. METHODS: We retrospective analyzed 64 TGA episodes from 63 patients [46 (71.9%) women, mean age ± SD = 58.6 ± 9.4]. Vestibular and ocular motor function test [including vestibular-evoked myogenic potentials (VEMPs)], neuropsychological assessments, and brain imaging were evaluated. Logistic regression was used to identify factors associated with dizziness and s

Transient Global Amnesia as the Sole Presentation of an Acute Stroke in the Left Cingulate Gyrus.

We discuss a patient who presented to our hospital with signs, symptoms, and clinical course consistent with transient global amnesia (TGA). Her hospital work-up was overall unremarkable except for the presence of a diffusion weighted imaging (DWI) abnormality on MRI suggestive of acute infarction in the left cingulate gyrus. Symptoms quickly resolved, and she was discharged home in stable condition. While the etiology of TGA remains controversial, in discussing this case and the current literature we hope to provide further data supporting the possibility of an underlying ischemic process, as

Emergency department visits for transient global amnesia alone or in association with substance abuse.

OBJECTIVES: To study the epidemiology of emergency department visits for transient global amnesia (TGA) by itself or associated with substance abuse or sexual assault. MATERIAL AND METHODS: Retrospective study of cases treated from January to December 2018. Data for all patients with TGA were extracted, and cases were classified as associated with substance abuse (TGASUB), sexual assault (TGASEX), or neither (TGAONLY). RESULTS: A total of 287 TGA cases were found: 169 (58.9%) were TGASEX, 62 (21.6%) TGAONLY, and 56 (19.5%) TGASUB. Two hundred eighteen (76%) were female and 69 (24%) were male

Isolated punctuate hippocampal infarction and transient global amnesia are indistinguishable by means of MRI.

Background Small punctuate lesions in the hippocampus on diffusion-weighted images are a typical finding in transient global amnesia. Consequently, it has been suggested that diffusion-weighted images findings might corroborate the diagnosis of transient global amnesia. However, isolated punctuate hippocampal infarction might be a differential diagnosis of transient global amnesia. Aim Evaluation of isolated punctuate hippocampal infarction frequency and comparison of its clinical presentation and MRI findings to transient global amnesia. Methods From an MRI database, we identified 10 patients

Migraine is associated with a higher risk of transient global amnesia: a nationwide cohort study.

BACKGROUND AND PURPOSE: The association between migraine and transient global amnesia (TGA) is not determined. Only two clinic-based studies showed that TGA patients had a higher frequency of migraine history. Our population-based study aimed to investigate whether migraine patients were associated with a higher risk of developing TGA. METHODS: Patients with migraine aged ≥18 years were identified from the Taiwan National Health Insurance Research Database between 2005 and 2009. Each migraine patient was randomly matched to one subject without migraine or other headache disorders based on age

Transient global amnesia followed by vertebrobasilar ischemia in a case of dolichoectatic basilar artery.

A 59-year-old hypertensive patient presented with transient global amnesia without neurological signs or symptoms. During the following 3 years, he experienced several attacks of vertebrobasilar insufficiency and a cerebellar infarction. CT scan and MRI found a partially thrombosed dolichoectatic basilar artery but no significant lesion within the limbic system. These data suggest transient global amnesia was due to a transient ischemia in the basilar artery distribution.

Prolonged Transient Global Amnesia: Part of the Clinical Spectrum or a Separate Disease Entity?

BACKGROUND: Transient global amnesia (TGA) is the prototypical neurologic disease for acute-onset reversible amnesia. It is currently defined by resolution of symptoms within 24-hours. In this case report we describe an atypical case of prolonged TGA, emphasizing our current lack of knowledge surrounding this disease entity and its pathophysiology. RESULTS: A 66-year old female presented acutely with profound anterograde amnesia and variable retrograde amnesia with no inciting event. A thorough workup to exclude alternative causes of amnesia (including computed tomography angiogram and electr

Involvement of the default mode network in patients with transient global amnesia: multilayer network.

INTRODUCTION: We aimed to investigate the alterations in the multilayer network in patients with transient global amnesia (TGA). METHODS: We enrolled 124 patients with TGA and 80 healthy controls. Both patients with TGA and healthy controls underwent a three-teslar brain magnetic resonance imaging (MRI). A gray matter layer matrix was created using a morphometric similarity network derived from the T1-weighted imaging, and a white matter layer matrix was constructed using structural connectivity based on the diffusion tensor imaging. A multilayer network analysis was performed by applying gra

Prevalence of Mimics and Severe Comorbidity in Patients with Clinically Suspected Transient Global Amnesia.

BACKGROUND: Transient global amnesia (TGA) is a syndrome featuring acute anterograde amnesia as the most striking clinical symptom. Its etiology is still a matter of debate. Most neurological guidelines allow the diagnosis on the basis of clinical criteria only; a more extensive evaluation is recommended only for patients with "red flags" like severe headache, nausea or vomiting, or metabolic abnormalities. The aim of our study was to assess the frequency of a severe underlying disease or alternative diagnoses (mimics) in patients fulfilling the clinical criteria. METHODS: We evaluated the me
