Current role of cryoamputation.

Cryoamputation or physiologic amputation has been used at our institution for more than 30 years. From 1971 through 1989, 891 major lower extremity amputations were performed in 750 patients. With the use of dry ice or mechanical refrigeration, 320 (36%) physiologic amputations were performed in 292 patients. After physiologic amputation, the initially elevated white blood cell count and temperature decreased. Complications of physiologic amputation were unusual; 3% of patients developed minor freezing above the tourniquet, which did not alter the amputation level, while 1% had purulence at th

Deer antler renewal gives insights into mammalian epimorphic regeneration.

Deer antlers are the only known mammalian organ that, once lost, can fully grow back naturally. Hence, the antler offers a unique opportunity to learn how nature has solved the problem of mammalian epimorphic regeneration (EpR). Comprehensive comparisons amongst different types of EpR reveal that antler renewal is fundamentally different from that in lower vertebrates such as regeneration of the newt limb. Surprisingly, antler renewal is comparable to wound healing over a stump of regeneration-incompetent digit/limb, bone fracture repair, and to a lesser extent to digit tip regeneration in mam

Efficacy and Safety of Negative Pressure Wound Therapy in Managing Lower Limb Amputation; An Updated Systematic Review and Meta-Analysis With Individual Patients Data Meta-Analysis and GRADE Assessment.

Major lower-limb amputation often leads to wound complications that delay recovery and prosthetic use. Negative Pressure Wound Therapy (NPWT) has been suggested to improve outcomes, but evidence is inconsistent. The objective of this study is to compare NPWT with standard wound therapy in fresh amputation stumps. A systematic review and meta-analysis following PRISMA and Cochrane guidelines included randomised and controlled observational trials. Primary outcomes were wound complications, including surgical-site infections (SSI) and wound dehiscence. Secondary outcomes included re-amputation, …

Pedicled Abdominal Flaps for Enhanced Digital Salvage After Severe Frostbite Injury.

Extremities are the area of the body most commonly affected by frostbite, which can also affect the face, ears, perineum, or genitals. Optimum management has moved away from early amputation and debridement toward maximizing tissue preservation and delaying surgical intervention. Increasing length of digit amputation increases morbidity, in terms of loss of hand function, experienced by patients. Reconstruction of affected digits is limited by bone necrosis, which often leads to shortened residual stumps and limited functional outcomes. This case describes the management of a severe frostbite

A randomised double blind trial of the effect of pre-emptive epidural ketamine on persistent pain after lower limb amputation.

Persistent pain has been reported in up to 80% of patients after limb amputation. The mechanisms are not fully understood, but nerve injury during amputation is important, with evidence for the crucial involvement of the spinal N-methyl d-aspartate (NMDA) receptor in central changes. The study objective was to assess the effect of pre-emptively modulating sensory input with epidural ketamine (an NMDA antagonist) on post-amputation pain and sensory processing. The study recruited 53 patients undergoing lower limb amputation who received a combined intrathecal/epidural anaesthetic for surgery fo

Surgical results of leg amputation according to Ghormley's technique in the treatment of chronic lower limb ischaemia.

The authors present their experience in the treatment of chronic lower limb ischaemia resulting from atherosclerosis by below knee amputation according to Ghormley's technique, with the immediate application of a semi-rigid plaster cast and early rehabilitation. From a group of 664 patients requiring major amputations, 61 patients (52 men) aged 64 +/- 11 fulfilled the criteria for unilateral below-knee amputation for critical limb ischaemia for atherosclerosis consequences. Peri-operative (30-days) mortality was of 3.28%. Stump healing was very good or good (no need for further surgery) in ove

Reconstruction of the hip stump.

The formation of a functional limb stump after combat injuries with extensive soft tissue damage is challenging. After amputations or revision surgeries, long femoral stumps may remain. Although functionally inferior to tibial stumps, they can still provide satisfactory movement and performance. However, prolonged prosthesis use or poor-quality muscle grafting can lead to complications, such as skin breakdown, trophic issues, and inability to continue prosthetic use. These problems are partly due to the mismatch between the area of the bone stump end and the applied body weight. Ilizarov compr

Management of Major Traumatic Upper Extremity Amputations.

Traumatic upper extremity amputation is a life-altering event, and recovery of function depends on proper surgical management and postoperative rehabilitation. Many injuries require revision amputation and postoperative prosthesis fitting. Care should be taken to preserve maximal length of the limb and motion of the remaining joints. Skin grafting or free tissue transfer may be necessary for coverage to allow preservation of length. Early prosthetic fitting within 30 days of surgery should be performed so the amputee can start rehabilitation while the wound is healing and the stump is maturing

Afferent sensory feedback for lower extremity prosthesis.

Electrical stimulation has been applied to sciatic nerves of patients to achieve sensory feedback after lower limb amputation for periods of up to six years. Patients used the sensory feedback device daily. Pain, infection and electrode displacement have not been problems. The immediate postoperative benefits are that pain is minimized after amputation and stump healing is improved. Furthermore, the stimulus affords the patient increased confidence when walking due to renewed awareness of the center of gravity. Improved ability to function in the dark and when walking up and down stairways mak

Amputation for peripheral vascular disease: experience of a district general hospital.

Two hundred and ninety major lower-limb amputations were performed on 286 patients during the period 1969-79. The early mortality rate was 6.9%. The mean age of the amputees was 70.2 (range 35-91) years. The commonest site for amputation was above the knee (77.2%). Failure to obtain immediate satisfactory healing of the stump occurred in 18.4% and reamputation was required in 5.6%. The hospital mortality rate was 25.5%. Routine prophylactic penicillin was not given. One patient developed gas gangrene.

Leg length preservation with pedicled fillet of foot flaps after traumatic amputations.

Six patients with insufficient soft-tissue coverage after lower limb trauma were treated with pedicled fillet of foot flaps to achieve primary stump closure and to preserve leg length. The flaps used were all based on either the posterior tibial neurovascular pedicle, the anterior tibial neurovascular pedicle, or both. Five flaps survived; one patient required conversion of a through-knee to an above-knee amputation and debridement of the flap because of venous thrombosis of the pedicle. In three of the cases, a functional knee joint was preserved. The patients ranged in age from 21 to 54 year

The Use of the Pedicled Nonislanded Foot Fillet Flap to Avoid an Above-the-Knee Amputation after Trauma.

BACKGROUND: Lower extremity trauma can have a devastating effect on the quality of the life of patients. To avoid limb amputation and its associated social stigma, many patients spend years undergoing multiple costly procedures, with prolonged hospital stays, all the while hoping for an elusive limb salvage. People with lower limb amputation experience higher energy requirements for walking, reduced aerobic capacity, and slower walking speeds, when compared with people with normal limbs. A below-knee amputation (BKA) is functionally superior to an above-knee amputation. METHODS: Between 2012

Measuring discomfort-An objective method for quantifying peak pressure discomfort and improved fit in adults with transtibial amputation.

BACKGROUND: Prosthetic socket coupling with the residual limb should be comfortable without causing skin breakdown or excessive pressure. However, users report socket discomfort, and there is a scarcity of objective measurements available to assess this feeling of discomfort. Quantifying the specific pressure may enable clinicians and users to determine and improve comfort levels objectively. OBJECTIVES: (1) To evaluate if a peak pressure reduction assists in resolving wounds, redness, and pain inside prosthetic sockets in people with transtibial amputation. (2) To determine if peak pressures

Estimating the Impact of Postamputation Pain.

BACKGROUND: Neuromas, neuralgia, and phantom limb pain commonly occur after lower-extremity amputations; however, incidence of these issues is poorly reported and understood. Present literature is limited to small cohort studies of amputees, and the reported incidence of chronic pain after amputation ranges as widely as 0% to 80%. We sought to objectively investigate the incidence of postamputation pain and nerve-related complications after lower-extremity amputation. METHODS: Patients who underwent lower-extremity amputation between 2007 and 2017 were identified using a national insurance-ba

Delayed Onset of Osteomyelitis in Fetal Limb Amputation Secondary to Amniotic Band Syndrome: A Case Report.

CASE: We present the case of an infant with an amputation from amniotic band syndrome and no other complications. During the seventh week of life, the infant developed severe pain in the affected limb that was ultimately found to be indicative of osteomyelitis. CONCLUSION: The infant displayed minimal systemic response, and cultures that grew methicillin-sensitive Staphylococcus aureus were needed to confirm the diagnosis. Surgical debridement, stump revision, and oral antibiotic therapy provided definitive treatment for our patient. The reader is encouraged to consider the possibility of ost

Peripheral Neuromodulation to Treat Postamputation Pain.

Some of the more common peripherally mediated pain disorders are postamputation stump pain and phantom pain. These disabling conditions have proven difficult to treat. Here we aim to illustrate an option to treat postamputation pain using peripheral neurostimulation techniques. Traditional peripheral neuromodulation techniques use standard stimulation parameters and work by stimulation of nerve tissues which are then felt by the patient as a tingling sensation or paresthesia. Recently introduced high-frequency (10 kHz) electrical nerve block [HFAC (high-frequency alternating current) block] vi
