OBJECTIVE: The goal of this study is to discuss our initial experience with a multimodal opioid-sparing cocktail containing ropivacaine, epinephrine, clonidine, and ketorolac (RECK) in the postoperative management of lumbar decompression surgeries.
METHODS: Patients were either administered no local anesthetic at the incision site or were administered a weight-based amount of RECK into the paraspinal musculature and subdermal space surrounding the operative site once the fascia was closed. We performed a retrospective chart review of all patients 18 years of age or older undergoing lumbar lam

OBJECTIVES: It is our experience that a small portion of patients with neuropathic abdominal wall pain syndromes such as the anterior cutaneous nerve entrapment syndrome (ACNES) have a long term beneficial response following just one single tender point injection (TPI) with a local anesthetic agent. This report focuses on the phenomenon of ongoing pain relief following a single local anesthetic injection in neuropathic abdominal wall and groin pain syndromes.
METHODS: This report is an overview based on earlier studies from a center of expertise for neuropathic abdominal wall and groin pain s

Novel anaesthetic formulation LHT-15-32 was studied. The experimental study involved 66 white mice, 15 Rana radibunda frogs and 50 male Sprague Dawley rats. Its acute intravenous and subcutaneous toxicity was determined in mice. Pain sensitivity threshold of upper second molar was determined in rats with experimental periodontitis. Oxidative stress activity and total antioxidant capacity were determined in rats' gingival mucosa by induced chemiluminescence. Tissue IL-1β, IL-10 and TNF-a concentration was quantitatively assessed by ELISA. LHT-15-32 Na-blocking activity was studied on isolated n

BACKGROUND: The injection of local anesthetic into the skin is often the only memorable event described by the patient after dermatologic procedures.
OBJECTIVE: The authors compared the pain felt during injection of local anesthetic using a minimal needle insertion technique with a 30- or 33-gauge needle.
MATERIALS AND METHODS: Three hundred eighteen patients with tumors on the head and neck were injected with lidocaine using a previously described technique with either a 30- or 33-gauge needle. After injection, patients were surveyed using the visual assessment scale for pain.
RESULTS: Sev

Patients with isolated peripheral branch neuralgia of trigeminal nerve usually receive traditional treatment such as medical therapy and interventional procedures targeting the entire trigeminal nerve or related ganglions. However, if the intractable pain is limited to a certain branch, the patient may also benefit from a peripheral and nerve-targeted interventional approach. Here, we report the management of a patient with isolated infraorbital neuralgia by ultrasound-guided infraorbital nerve block with steroid and local anesthetic combination. 48years-old male patient diagnosed with trigemi

Cavitation plays a substantial role in the clinical effects of extracorporeal shock wave therapy (ESWT). It is also generally accepted as a major mechanism in sonophoresis. To identify the enhancing effect of extracorporeal shock wave-mediated transdermal drug delivery, 24 Wistar rats were randomly assigned to four groups: (i) topical application of a eutectic mixture of local anesthetics (EMLA); (ii) 1-MHz ultrasound; (iii) ESWT pre-treatment combined with EMLA application; (iv) ESWT concurrent with EMLA application on rat tails. The degree of anesthesia was assessed using the amplitude and l

OBJECTIVE: The objective of this study was to compare the effect of long acting anesthetics on postoperative pain in teeth with irreversible pulpitis.
METHODOLOGY: Forty patients were randomly assigned into two groups of twenty patients each. Each patient who fit the inclusion criteria was administered local anesthesia before undergoing root canal treatment. The anesthetic solution was either 2% lidocaine with 1:80,000 epinephrine or 0.5% bupivacaine with 1:200,000 epinephrine. Patients were instructed to complete a VAS pain score at 6, 12, 24 h after single visit root canal treatment. Data w

BACKGROUND: Glenohumeral chondrolysis is the irreversible destruction of previously normal articular cartilage, occurring most commonly after shoulder surgery in young individuals. The reported incidence of this complication has risen rapidly since the early 2000s. As chondrolysis cannot be reversed, its occurrence can only be prevented by establishing and avoiding its causes.
METHODS: We analyzed all published cases of glenohumeral chondrolysis, including the relevant published laboratory data, to consolidate the available evidence on the causation of this complication by the postoperative i

Traditionally, stellate ganglion blockade has been used for the diagnosis and treatment of upper extremity sympathetic pain. However, this treatment has not been shown to provide adequate sympathetic blockade of the upper extremities. This study demonstrates that a carefully performed upper thoracic sympathetic block with imaging guidance can result in a successful sympathetic blockade of the upper extremities. This study furthermore demonstrates that the occurrence of a Horners syndrome is not a testimony to a successful sympathetic block of the upper extremities.

BACKGROUND AND OBJECTIVES: Cauda equina syndrome has been reported recently in patients receiving continuous spinal anesthesia using newly developed microcatheters (28 gauge). Failure of microcatheters to allow adequate mixing of local anesthetic was studied as a possible mechanism of the neurologic injury reported with these catheters.
METHODS: A spinal canal model was developed and the distribution of hyperbaric lidocaine was measured after injection through catheters typically used for continuous spinal anesthesia (i.e., 20, 28, and 32 gauge).
RESULTS: Lidocaine distribution was less unif

A number of compounds showing general anesthetic action in the rotifer Brachionus calyciflorus were investigated in the presence of acetylcholine. Non-ionizing anesthetics, including tricaine, showed no interaction with acetylcholine. However, highly ionized compounds like the local anesthetics procaine and lidocaine, the muscarinic blocker and local anesthetic atropine, and the beta-adrenergic blocker propranolol showed a synergistic effect with acetylcholine. ACh increased the general anesthetic effect of these compounds in a statistically highly significant dose-dependent fashion. To accoun

Local anesthetic systemic toxicity (LAST) is a rare but potentially life-threatening complication of regional anesthesia. The increasing popularity of regional anesthesia of all kinds means that the risk of encountering LAST in clinical practice is nevertheless real.Part 1 of this CME-article focuses on the recognition and understanding of LAST, providing an up-to-date overview of its pathophysiology, risk factors, and clinical presentation. The pathophysiological processes underlying local anesthetic systemic toxicity (LAST) stem from the interplay of various complex molecular mechanisms invol

Background and Clinical Significance: Needle fracture during inferior alveolar nerve block is a rare complication, but it can nevertheless result in serious complications, especially when the fragment migrates into deep anatomical spaces like the pterygomandibular region. Accurate localization and safe retrieval are vital in preventing infection, chronic pain, neurovascular injury, and long-term functional impairment. Case Presentation: We present a case of a 27-year-old patient who had a fractured needle fragment from a local anesthetic procedure retained in the left pterygomandibular space.

BACKGROUND: Vocal fold injection (VFI) with calcium hydroxylapatite (CaHa) is a well-established surgical technique to improve speech and swallow outcomes in unilateral vocal fold palsy (UVFP). While often performed under local anesthetic (LA), there is limited data on its safety profile and the frequency of adverse events.
AIMS: To systematically evaluate the safety profile of CaHa VFI under LA for UVFP in adults.
METHODOLOGY: A systematic literature search was conducted of EMBASE and Ovid MEDLINE databases in August 2024. Results were screened, and inclusion criteria were applied. Studies

BACKGROUND: Complications from in-office inferior turbinate (IT) and septal swell body coblation typically include pain, congestion, and bleeding. However, rare cases of cranial nerve palsies from local anesthetic infiltration can occur. We present a rare case of an oculomotor nerve palsy and palsies of the ophthalmic (V1) and maxillary (V2) branches of the trigeminal nerve following intranasal local anesthetic infiltration prior to coblation, a complication not previously documented in the literature. Awareness of such rare and transient complications can help surgeons and clinicians avoid un
