OBJECTIVES: Agonistic AT1 receptor autoantibodies have been described in patients with hypertension and preeclampsia. These autoantibodies could stimulate proliferation of vascular smooth muscle cells (VSMCs), which are involved in angiotensin II-induced vascular injury in cardiovascular disease. Hence, in this study, we explored the existence of agonistic AT1 receptor autoantibodies in unstable angina (UA) patients and the possible effects of them on the in-stent restenosis of these patients.
METHODS: A total of 95 UA patients and 98 healthy volunteers were enrolled. The serum of each patien

BACKGROUND: Melatonin, a lypophylic antioxidant, was reported to be low in patients suffering from myocardial infarction, a disease due to coronary atherosclerosis associated with increased reactive oxygen species. Studies carried out till now have shown association of either melatonin or another antioxidant Cu/Zn SOD in patients suffering from coronary artery disease but not both together. Concurrent measurement of these two parameters will reveal the relationship if any in the probable causation of the disease.
METHODS: We estimated the serum malondialdeheyde, oxidized LDL, lipid profile an

Levels of autoantibodies to oxidized low-density lipoprotein (oxLDL) have been correlated to atherosclerosis; however, contradictory results have been shown. To better understand the role of autoantibodies to oxLDL in atherogenesis, and their potential to predict risk of developing coronary artery disease we investigated the antibody response of unstable angina (UA) patients and healthy controls against chromatographic separated fractions of oxLDL. Five major peaks were detected after chromatographic separation of oxLDL and 10 fractions were collected. Surprisingly, when the response to high m

OBJECTIVE: To identify clinical and angiographic profiles of patients with unstable angina seen at a tertiary hospital and treated with percutaneous coronary intervention (PCI).
METHODS: Study of a consecutive series of 1413 patients, selected from a computerized database, who underwent percutaneous revascularization in the three-year period of 2002-2004. There were no inclusion/exclusion criteria.
RESULTS: Systemic arterial hypertension (74%) and hypercholesterolemia (65%) were the classical risk factors for coronary disease most frequently observed. Coronary artery bypass grafting and hist

BACKGROUND: Improved myocardial blush grade is associated with better MACE outcomes in acute myocardial infarction patients but there are no data on myocardial blush grade (MBG) assessment in unstable angina (UA) patients treated with coronary intervention. We sought to evaluate the use of angiographic MBG assessment in a cohort of UA patients treated with angioplasty.
METHODS: Three hundred and seventy-two consecutive UA patients (mean age 68+/-1 years) treated with PCI were included. No patients had a pre-procedural troponin I (TnI) elevation. Final MBG was recorded for the territory subser

Acute coronary syndromes (ACS) are a major cause of morbidity and mortality in Western society. The commonest underlying pathology is the coronary atherosclerotic plaque complicated by thrombosis. Platelets play a central role. As the glycoprotein IIb/IIIa receptor is involved in the final step of platelet thrombus formation development and investigation of its blockage in ACS is at the fore. Initial hospital management of ACS occur in the ED setting. The present article outlines the above topics and clarifies the place of glycoprotein IIb/IIIa receptor inhibition in ED.

OBJECTIVE: To examine the expression and localisation of adrenomedullin in human coronary atherosclerotic lesions from patients with unstable angina (UAP) and stable angina (SAP), and to study the relation between adrenomedullin expression and plaque instability.
DESIGN: A retrospective observational study.
PATIENTS: Directional coronary atherectomy samples were obtained from 15 patients with UAP and 12 with SAP.
METHODS: The localisation of adrenomedullin was examined by immunohistochemistry, and adreno-medullin mRNA expression was measured by quantitative polymerase chain reaction.

The aim of this study was to investigate the possible association of plasma C-reactive protein (CRP) levels with the presence of angiographically multiple complex lesions (CLs) in patients with primary unstable angina (PUA). For the purpose of this study, 228 consecutive patients with PUA who underwent in-hospital catheterization were evaluated. Plasma CRP levels were measured upon patients' admission. Coronary plaques were classified as CL or non-CL according to Ambrose's criteria. There were 100 (43.9%) patients with no or one CL (</=1) and 128 (56.1%) patients with multiple CLs (>/=2). Tert

The aim of this study was to assess the relations between inflammation, immune response, and coronary angiographic findings in patients with unstable angina pectoris (UAP). Recent studies suggest a role for inflammation in the pathophysiology of UAP. Although activation of neutrophils, monocytes and lymphocytes has been shown in UAP, no studies have correlated the activation findings with clinical and angiographic features of patients with UAP. Seventy-three patients undergoing coronary angiography were classified according to their ischaemic syndrome, stable angina pectoris (SAP) (n = 25) and

Coronary thrombosis is an important determinant of prognosis in patients with acute coronary syndromes (ACS). However, the identification of patients at high-risk for progression of coronary thrombosis is difficult partly because we currently lack clinically meaningful laboratory methods for its detection. The most promising approaches involve the measurement in plasma of markers of fibrin formation and degradation. Thrombin activity, as reflected by plasma or urine concentrations of fibrinopeptide A, is increased in patients with ACS and is associated with adverse outcome. However, the use of

A local cooling test produces a typical flow stop reaction in nailfold capillaries in 88% of patients with Raynaud's phenomenon, but only in 15% of healthy controls. A stop reaction occurred in 9 of 12 patients with Prinzmetal variant angina, exceeding significantly that in 2 of 12 matched control subjects. Also in patients with reversible visual disorders presumably due to vasospasm, the test resulted in a stop reaction in 16 of 25 cases exceeding that in control. In both of these patient groups the cooling test as well as the symptoms reacted favorably to nifedipine. Thus, the local cold exp

BACKGROUND: Unstable angina (UA) is a high-risk presentation of acute coronary syndrome (ACS) that can rapidly progress to myocardial infarction (MI) if not identified and managed promptly. Inflammation plays a key role in plaque instability and thrombotic events, making inflammatory biomarkers useful tools for early risk assessment. The Systemic Inflammatory Response Index (SIRI), derived from peripheral blood cell counts, has emerged as a novel marker of systemic inflammation, but its prognostic utility in UA remains underexplored.
OBJECTIVE: The purpose of this study was to investigate the

BACKGROUND: Coronary artery fistula (CAF) is an atypical vascular communication that drains into a cardiac chamber or vessel and is usually unrelated to atherosclerotic coronary artery disease (CAD).
CASE SUMMARY: An elderly patient with high-risk unstable angina underwent coronary angiography, which revealed 3-vessel CAD and multiple CAFs. Medical treatment was elected.
DISCUSSION: Multiple CAFs, especially a large right CAF draining into the left ventricle in the context of significant 3-vessel CAD in an elderly patient, represent a rare finding with an unusual presentation of unstable ang

BACKGROUND: Unstable angina is a serious heart condition characterized by intense chest pain and anxiety, which substantially reduces patients' well-being.
OBJECTIVE: This study examined the combined effects of rhythmic breathing and aromatherapy on pain and anxiety in unstable angina.
METHODS: This two-arm randomized controlled trial involved 56 participants, who were equally allocated to an intervention group (n = 28) and a control group (n = 28). The intervention group received rhythmic breathing exercises combined with aromatherapy using 40 % diluted damask rose essential oil, administer
