A preliminary experience with use of magnetic resonance angiography in assessment of failing lower extremity bypass grafts.

BACKGROUND: This study evaluates the ability of magnetic resonance angiography (MRA) to detect hemodynamic failure of lower extremity bypass grafts and questions whether conventional angiography is necessary for this purpose. METHODS: A prospective surveillance protocol with segmental plethysmography and color flow Doppler scanning has been used to detect hemodynamic graft failure in 110 patients with 120 lower extremity bypass grafts (90 vein, 30 polytetrafluoroethylene). Twenty (17%) bypass grafts fit the criteria for impending failure (drop in ankle/brachial index determination greater tha

Internal Carotid Artery Hypoplasia Misidentified as Internal Carotid Artery Dissection.

Agenesis or hypoplasia of the internal carotid artery (ICA) may easily be confused with dissection or occlusion. We report a case of a 24-year-old female with complaint of acute left-hand hypoesthesia and a history of occasional intermittent numbness of her right hand with myoclonic jerking. Because previous imaging studies over 2 years were interpreted as occlusion of the left ICA secondary to carotid dissection, the treating physician had prescribed anticoagulant therapy. During transcranial Doppler (TCD) examination, the spectral waveform was unexpectedly normal, prompting a repeat review o

Coexisting filum terminale arteriovenous fistula and filum terminale lipoma treated with single-stage surgery: illustrative case.

BACKGROUND: Both filum terminale arteriovenous fistulas (FTAVFs) and filum terminale lipomas (FTLs) are rare. Because of this rarity, there is a paucity of data regarding concomitant FTAVF and FTL, and the optimal treatment remains to be defined. The authors describe a patient with coexisting FTAVF and FTL treated with single-stage surgery. OBSERVATIONS: A man in his 70s was referred to the authors' department because of a suspected spinal vascular malformation seen on magnetic resonance imaging that was performed to investigate lower limb weakness, intermittent claudication, and urinary inco

Clinical Image-Vertebral artery beading in a child with Beta Thalassemia major.

Vertebral artery beading is an uncommon, vascular complication associated with various systemic conditions. We describe the vascular imaging of a 11-year-old boy with Beta Thalassemia Major who developed acute ataxia following a splenectomy. MR angiography revealed a notable finding: symmetric, bilateral vertebral artery beading; while Digital Subtraction Angiography (DSA) was normal. This case highlights an intriguing imaging feature that mimics vasculopathy, underscoring the importance of careful interpretation in such contexts.

CT angiographic demonstration of a mesenteric vessel "whirlpool" in intestinal malrotation and midgut volvulus: a case report.

Although the color Doppler ultrasonography diagnosis of intestinal malrotation with midgut volvulus, based on the typical "whirlpool" appearance of the mesenteric vascular structures is well-defined in the peer-reviewed literature, the combination of both the angiographic illustration of these findings and the contemporary state-of-the-art imaging techniques is lacking. We report the digital subtraction angiography and multidetector computed tomography angiography findings of a 37-year-old male with intestinal malrotation.

Intraprocedural Rupture During Coil Embolization of a Ruptured, Rapidly Thrombosing Anterior Cerebral Artery Aneurysm: A Case Report.

Acute thrombosis is often observed following the rupture of an intracranial aneurysm, and optimal coil selection during endovascular coiling is challenging. A 75-year-old woman presented with a subarachnoid hemorrhage. Computed tomography angiography revealed aneurysms at the right anterior cerebral artery A2/3 junction and the right middle cerebral artery. Based on vessel wall imaging, the anterior cerebral artery aneurysm was diagnosed as the rupture site. Initial digital subtraction angiography demonstrated a 4.4 × 3.4 × 3.4 mm aneurysm with a 1.6 mm neck. Coil embolization was performed 1

Neuroimaging in Moyamoya angiopathy: Updated review.

Moyamoya angiopathy (MMA) is a rare condition characterized by steno-occlusion of terminal portions of the internal carotid arteries and/or their proximal branches with the formation of collateral vessels network (moyamoya vessels). These changes result in chronic ischemia of the brain parenchyma with subsequent serious cerebrovascular accidents. The prognosis of MMA could be dramatically improved if the diagnosis and assessment are prompt and accurate. It is also crucial to define the indications, optimal timing, volume, and tactics of revascularization surgery, as well as longitudinal observ

Volume subtraction three-dimensional CT angiography for cerebrovascular disease: report of two cases.

We report two cases in which volume subtraction three-dimensional CT angiography (VS-3DCTA) was used for cerebral aneurysm and vascular stenosis with intramural calcification. Case 1: VS-3DCTA with volume rendering clearly showed carotid cave aneurysm of the internal carotid artery. The location and size of the aneurysm was confirmed by digital subtraction angiography (DSA). In evaluation of the aneurysm, VS-3DCTA was equal to DSA and endovascular findings. Case 2: VS-3DCTA with volume rendering clearly showed stenosis of the middle cerebral artery, and intramural calcification with the cause

A Computational Framework for Pre-Interventional Planning of Peripheral Arteriovenous Malformations.

PURPOSE: Peripheral arteriovenous malformations (pAVMs) are congenital lesions characterised by abnormal high-flow, low-resistance vascular connections-the so-called nidus-between arteries and veins. The mainstay treatment typically involves the embolisation of the nidus, however the complexity of pAVMs often leads to uncertain outcomes. This study aims at developing a simple, yet effective computational framework to aid the clinical decision making around the treatment of pAVMs using routinely acquired clinical data. METHODS: A computational model was developed to simulate the pre-, intra-,

A Treatable Cause of Pulsatile Tinnitus: A Case of Sigmoid Sinus Dehiscence.

Tinnitus is a symptom of an underlying condition that can be neurological, ontological, or infectious in origin. This case report describes a patient with pulsatile tinnitus caused by sigmoid sinus dehiscence, which was successfully treated by sigmoid sinus dehiscence repair. We recommend computed tomography angiography/magnetic resonance angiography or digital subtraction angiography to rule out vascular malformation, such as arteriovenous fistula, prior to surgical intervention. In addition, we recommend imaging of the brain and formal evaluation by an ophthalmologist and lumbar puncture pri
