PURPOSE: To evaluate the clinical outcomes of flattened flanged intrascleral fixation of the intraocular lens (IOL) in cases of pediatric aphakia without adequate capsular support.
METHODS: The medical records of children who underwent flattened flanged intrascleral IOL implantation for aphakia with insufficient capsular support, subluxated cataract, ectopia lentis, and dislocated IOL were reviewed retrospectively. Extended-wear contact lenses (EWCL) are being used increasingly for the correction of aphakia in infants, but there is a lack of data documenting their effects on the corneas of young children. OBJECTIVE: This study aims to describe a novel technique for implanting an intraocular lens (IOL) in the sulcus using a 10-0 nylon suture in cases of intraoperative posterior capsular rupture (PCR) with inadequate capsular support, anterior capsular extension during continuous curvilinear capsulorrhexis extending to the posterior capsule, or posttraumatic cataract.
METHODS: The study included 52 patients who underwent surgery at our hospital, during which an IOL was implanted with the assistance of a 10-0 nylon suture. All patients completed a 1-year follow-up period.

We studied the myopic shift and anisometropia at 10.6 (+/−0.3) years of age in the Infant Aphakia Treatment Study. We found myopic shift continues in the operated eye from 5-10.5 years at a lower rate than that prior to age five years while anisometropia increases proportionally.

METHODS: Eighteen eyes of 18 patients who once had pars plana vitrectomy and lensectomy combined with silicone oil tamponade underwent silicone oil removal using a self-sealing corneal incision.
RESULTS: All of the 18 silicone oil removal procedures performed had clear corneal incisions, which did not require suturing.

A computer-based corneal topographic analysis system was used to evaluate the patterns of power distribution in five patients with at least six months of postoperative follow-up who obtained excellent visual and refractive results after epikeratophakia for aphakia. One hundred fifteen eyes of 62 patients were fit with paflufocon-A (Fluoroperm) rigid gas permeable (RGP) contact lenses. Patients included myopes, hyperopes, aphakes, and keratoconic and astigmatic patients; the latter patients were fit with bitoric designs for large amounts of corneal astigmatism or front toric designs for residual astigmatism. The repeatability and accuracy of an i.r. electronic optometer (the Ophthalmetron) and of retinoscopy performed by experienced practitioners were studied in aphakia by testing a group of 17 patients three times with each method and by subjective techniques. The results show that the Ophthalmetron gives better repeatability than retinoscopy and that accuracy is marginally better for the sphere with the Ophthalmetron and for the cylinder power by retinoscopy when the mean of the three subjective results is taken as the true refractive value.
