Progesterone administration and progressive sleep apneas.

Nine adult patients with obstructive sleep apnea syndrome were administered medroxyprogesterone acetate, 60 to 120 mg/day, and the effect of medroxyprogesterone on clinical symptoms and obstructive apneas during sleep was assessed. Four patients responded with resolution of daytime sleepiness, disappearance of pedal edema, and a decrease in the number of obstructive apneas during sleep. Cessation of therapy led to a return of daytime sleepiness in three and an increase in obstructive apneas in all four subjects. Two responders experienced side effects of alopecia or decreased libido. Before me…

Chemoreceptor function and sleep state in apnea.

Resting ventilation and ventilatory responses to 100% oxygen and to 5% carbon dioxide in air were measured in REM and non-REM sleep in post-neonatal infants. Normal controls were compared to infants with prolonged apnea and to siblings of sudden infant death victims. No significant differences in ventilatory responses were found between the groups. We conclude that apnea may occur in infants whose central and peripheral chemoreceptor activity is normal while they are breathing.

Is adenotonsillectomy effective in improving central apnea events in patients with obstructive sleep apnea? A systematic review and meta-analysis.

PURPOSE: To evaluate the effects of adenotonsillectomy on improving central sleep apnea events in children with obstructive sleep apnea (OSA). METHODS: We searched four online databases for relevant articles published from inception until October 2022. We included studies that measured the number of central apnea events per sleep and central apnea-hypopnea index (CAHI) or central apnea index (CAI) scores in children with OSA before and after adenotonsillectomy. Our primary outcomes were changes in CAI scores, the number of central apnea events per sleep, and CAHI scores after surgery. Our sec…

Effects of hyperventilation on oxygenation, apnea breaking points, diving response, and spleen contraction during serial static apneas.

PURPOSE: Hyperventilation is considered a major risk factor for hypoxic blackout during breath-hold diving, as it delays the apnea breaking point. However, little is known about how it affects oxygenation, the diving response, and spleen contraction during serial breath-holding. METHODS: 18 volunteers with little or no experience in freediving performed two series of 5 apneas with cold facial immersion to maximal duration at 2-min intervals. In one series, apnea was preceded by normal breathing and in the other by 15 s of hyperventilation. End-tidal oxygen and end-tidal carbon dioxide were me…

Proteomic Biomarkers of the Apnea Hypopnea Index and Obstructive Sleep Apnea: Insights into the Pathophysiology of Presence, Severity, and Treatment Response.

Obstructive sleep apnea (OSA), a disease associated with excessive sleepiness and increased cardiovascular risk, affects an estimated 1 billion people worldwide. The present study examined proteomic biomarkers indicative of presence, severity, and treatment response in OSA. Participants (n = 1391) of the Stanford Technology Analytics and Genomics in Sleep study had blood collected and completed an overnight polysomnography for scoring the apnea−hypopnea index (AHI). A highly multiplexed aptamer-based array (SomaScan) was used to quantify 5000 proteins in all plasma samples. Two separate interv…

Western University Protocol for Obstructive Sleep Apnea.

Background/Objectives: Obstructive sleep apnea (OSA) is a prevalent disorder in adults, characterized by recurrent upper airway obstruction during sleep, resulting in intermittent hypoxia, sympathetic activation, and sleep fragmentation. It is linked to significant cardiovascular, metabolic, neurocognitive, and psychosocial morbidity. There is increasing evidence that continuous positive airway pressure (CPAP) adherence remains suboptimal in many patients, and in those patients, surgery is often indicated. Methods: This protocol report presents an updated and protocol-driven surgical approach …

Community prevalence of positional obstructive sleep apnea.

STUDY OBJECTIVES: The prevalence of positional obstructive sleep apnea (POSA) in community populations warrants further investigation. Further, more research is needed into the clinical characteristics of its subtypes such as supine predominant OSA (spOSA) and supine isolated OSA (siOSA). METHODS: A cross-sectional analysis was performed on 1,870 Sleep Heart Health Study participants. OSA was defined by an apnea-hypopnea index (AHI) of ≥ 5 events/h of sleep. Participants with OSA were classified as POSA if the supine AHI was ≥ 2 times the nonsupine AHI. Participants with OSA who did not meet …

Pontine hyperperfusion associated with ictal central apnea.

We report ictal SPECT findings in a patient with focal epilepsy and ictal central apnea (ICA). We describe the history, EEG findings, and SPECT analysis of a 38-year-old female with left posterior quadrant epilepsy post intracerebral hemorrhage due to ruptured aneurysm, whose seizures are accompanied by ICA. The patient underwent continuous EEG monitoring as a presurgical candidate for her epilepsy, during which several typical seizures associated with ICA were captured. Ictal SPECT was performed during a typical seizure with ICA, with tracer administration 15 s after the ICA onset, 14 s after…

Transient radiation-induced severe obstructive sleep apnea.

UNLABELLED: Radiotherapy for head and neck cancer can trigger or worsen obstructive sleep apnea (OSA) due to factors such as neurological impairment, muscle atrophy, edema, and xerostomia. We present a case in which a patient developed severe OSA 3 months after undergoing radiochemotherapy and neck dissection for squamous cell carcinoma of the right lingual tonsil with regional metastasis. Polysomnography confirmed severe OSA. Unfortunately, the patient did not adhere to the recommended treatment. The symptoms persisted for 6 months but then, unexpectedly, resolved completely. A follow-up poly…

Apnea Detection in newborns using abdominal IMU.

Over recent years, there has been an increase in cases of sudden postnatal collapse in apparently healthy newborns within the first 24 hours of life. Current monitoring systems struggle to identify episodes of apnea. This study proposes a solution: an electronic device designed to monitor abdominal movements associated with breathing in newborns, complemented by a user interface for information tracking. Experimental tests were conducted to validate the system's performance in various scenarios, including supine and prone positions, as well as during mother-infant contact. Overall, the results…

'Personalized medicine': phenotyping pediatric obstructive sleep apnea.

PURPOSE OF REVIEW: Obstructive sleep apnea (OSA) is common in children. Phenotyping pediatric OSA has a crucial role in personalized diagnosis and treatment to improve outcomes for this population. This review sets forth a clinical approach that allows for phenotyping pediatric OSA. RECENT FINDINGS: The emerging concept of phenotyping pediatric OSA is based on identifying a primary cause, which leads to a more precise understanding of the pathogenesis in any individual patient. Phenotyping enables treatment focusing on the primary cause, but does not exclude the need for supplemental manageme…

Obstructive Sleep Apnea in Coronary Artery Disease.

Obstructive sleep apnea (OSA) is a common sleep-related breathing disorder, characterized by obstruction of upper airways during sleep, resulting in repetitive breathing pauses accompanied by oxygen desaturation and arousal from sleep. OSA patients commonly suffers from poor sleep quality and reduced quality of life. Further, OSA is associated with cardiovascular risk factors and linked independently to both structural coronary artery disease (CAD) as well as functional CAD. Structural CAD is depicted by atherosclerosis (either obstructive or non-obstructive) of the epicardial coronary arterie…

Obstructive sleep apnea -consideration of its pathogenesis.

The pathogenesis of obstructive sleep apnea (OSA) is characterized not only by obstruction of the pharynx, but also by repeated obstruction. OSA onset is thought to involve four phenotypic traits: pharyngeal muscle responsiveness, respiratory center instability (loop gain), arousal threshold, and anatomical factors. Patients with lower muscle responsiveness are likely to have OSA, whereas those with higher responsiveness are not. When the loop gain is relatively high, reaction and suppression of the respiratory drive are repeated, decreasing ventilation and pharyngeal muscle activity and leadi…
