Single-incision laparoscopic appendectomy for treating appendicitis in a patient with gastrointestinal malrotation.

INTRODUCTION: Intestinal malrotation is a rare congenital anomaly, and acute appendicitis associated with intestinal malrotation is extremel … nding structures is easy. CONCLUSION: We conclude that transumbilical single-incision laparoscopic appendectomy is a simple and less invasive method for treating appendicitis associated with intestinal malrotation.

Role of drains in laparoscopic appendectomy for complicated appendicitis at a busy county hospital.

Laparoscopic appendectomy (LA) has become the treatment of choice for acute appendicitis with equal or better outcomes than traditional open appendectomy (OA). LA in patients with a gangrenous or perforated appendicitis carries increased rate of pelvic abscess formation when compared with OA. We hypothesized routine placement of pelvic drains in gangrenous or perforated appendicitis decreases pelvic abscess formation after LA. Three hundred thirty-one patients undergoing LA between January 2007 and June 2011 were reviewed. Patients with perforated or gangrenous appendicitis were included. Grou…

Management of the base of the appendix in pediatric laparoscopic appendectomy: clip, ligate, or staple?

Options for intracorporeal appendiceal stump closure span a variety of techniques including ligation using intra-corporeal knots, extra-corporeal knots, or an endo-loop (EL), closure with endoscopic clips (EC), or endoscopic stapled (ES) closure. The guiding principles are the need for secure, inert closure of the appendiceal base without injury to the appendiceal stump or cecum, with minimal risks of complication attributable to the closure technique. Safety and complication rates, as well as cost data, should guide the techniques used for pediatric laparoscopic appendectomy. Based on the lit…

Analysis of endoloops and endostaples for closing the appendiceal stump during laparoscopic appendectomy.

PURPOSE: An inadequate closure of the appendiceal stump can lead to intra-abdominal surgical site infections. The aim of this study was to assess the efficiency of different closure techniques by focusing on the intraoperative and postoperative complications versus cost. METHODS: From June 2011 to June 2013, 333 patients from two different hospitals undergoing laparoscopic appendectomy were included in this study. The patients were divided into two groups based on the technique used for appendiceal stump closure: there were 104 patients in the stapler group and 229 in the loop group. RESULTS…

Is routine appendectomy at the time of primary surgery for mucinous ovarian neoplasms beneficial?

UNLABELLED: The question whether the appendix should be removed at the time of surgery for apparent early-stage ovarian cancer is controversial. Removal of the appendix in the setting of mucinous histologic type is primarily driven by the existing challenge to distinguish between primary ovarian mucinous neoplasm and metastatic appendiceal carcinoma to the ovary. OBJECTIVES: To evaluate the value of an appendectomy at the time of surgery for ovarian mucinous borderline tumors or carcinoma. METHODS: A retrospective single institute-based study was conducted. We identified patients who were op…

Antibiotics versus appendectomy in the management of acute appendicitis: a review of the current evidence.

BACKGROUND: Acute appendicitis remains the most common cause of the acute abdomen in young adults, and the mainstay of treatment in most centres is an appendectomy. However, treatment for other intra-abdominal inflammatory processes, such as diverticulitis, consists initially of conservative management with antibiotics. The aim of this study was to determine the role of antibiotics in the management of acute appendicitis and to assess if appendectomy remains the gold standard of care. METHODS: A literature search using MEDLINE and the Cochrane Library identified studies published between 1999…

On-table diagnostic accuracy and the clinical significance of routine exploration in open appendectomies.

INTRODUCTION: The current practice of further exploration for other intra-abdominal pathology only when a normal appendix is found may leave other organic causes of acute abdomen undetected if the surgeon's on-table diagnostic accuracy is low. METHODS: In this retrospective study in 518 patients who underwent surgery for acute appendicitis, the on-table operative diagnosis of surgeons was correlated with the histological diagnosis of pathologists. RESULTS: Surgeons were unable to make an accurate on-table diagnosis in 14.3% of cases. The sensitivity for diagnosing normal appendices was also

Laparoscopic appendectomy in paediatric patients: optimization with a new method of port insertion.

BACKGROUND: Conventional methods of port insertion for laparoscopic appendectomy often result in poor surgical access, especially when a pelvic appendix is encountered or when mobilization of the caecum becomes necessary. The relatively small antero-posterior diameter of the peritoneal cavity in children further aggravates this problem. METHODS: We have modified the procedure by inserting only one 10-mm port at the sub-umbilical position, and two 5-mm ports at the lower abdomen beneath the 'bikini line'. The videoscope is inserted via the left lower abdominal port, and the sub-umbilical port …

Right atrial appendectomy reduces the renal response to acute hypervolemia in the rat.

We have shown previously that an extract of atrial tissue from rat heart contains a potent natriuretic factor. In this study anesthetized rats were connected to a respirator and the right atrial appendage was either excised, using a loop ligature (experimental group), or the loop was placed around the appendage and then removed (sham-operated group). After equilibration and control urine collection periods an isooncotic Ringers-albumin solution was infused intravenously (25% of estimated blood volume), and renal function was monitored over the next hour. There were no differences between group

Are we gaining value? A national evaluation of robotic versus laparoscopic appendectomy.

INTRODUCTION: Robotic appendectomy is increasingly used to treat acute appendicitis, yet its clinical benefits and resource implications remain unclear. We compared operative outcomes of robotic appendectomy and laparoscopic appendectomy. METHODS: We conducted a retrospective cohort analysis of adults undergoing minimally invasive appendectomy from 2022 to 2023 using the NSQIP database. The primary outcome was operative time. Secondary outcomes included surgical site infection, conversion to open surgery, and postoperative sepsis. We assessed the statistical interaction among operative approa…

Pediatric Ogilvie Syndrome Following Laparoscopic Appendectomy: A Case Report and Literature Review.

Acute colonic pseudo-obstruction (ACPO), or Ogilvie syndrome, is a rare cause of massive colonic dilation in children, occurring without mechanical obstruction. Its subtle presentation often mimics ileus or bowel obstruction, which may delay diagnosis and increase the risk of ischemia or perforation. We report the case of a seven-year-old boy who developed ACPO two weeks after a laparoscopic appendectomy for perforated appendicitis. Imaging revealed significant colonic distention without a transition point. The patient improved with supportive care, including bowel rest, intravenous fluids, an…

An unexpected giant: incidental discovery of a giant Meckel's diverticulum during appendectomy.

Giant Meckel's diverticula (GMD), characterized by a diameter over 5 cm, are a rare manifestation of the most common congenital gastrointestinal anomaly. GMDs challenge the conventional understanding of Meckel's diverticulum. Their detection may be linked to complications such as inflammation, perforation, or obstruction, which are more frequent in larger lesions. This report presents an unusual case of GMD found incidentally during surgery for acute appendicitis, representing the first documented instance in the English-speaking Caribbean. It emphasizes the diagnostic difficulties associated

Intraluminal Hair Identified During Laparoscopic Appendectomy for Perforated Appendicitis in a Child.

Intraluminal foreign bodies, including hair, have been described in association with appendicitis, sometimes in complicated forms with perforation. It has been suggested that they may contribute to the inflammatory process through luminal obstruction and subsequent progression of inflammation. We present the case of a pediatric patient with intraluminal hair identified in the setting of perforated appendicitis.

Laparoscopic Appendectomy for Early Appendicular Lump in Children: A Single-Institution Experience.

BACKGROUND: The ideal management of appendicular lump in children is still debated, with traditional interval appendectomy being challenged by early laparoscopic intervention. The purpose of this study was to evaluate the safety and efficacy of emergency laparoscopic appendectomy (LA) in children with an early appendicular lump. METHODS: A retrospective chart review of 30 patients (≤15 years) presenting with an early appendicular lump who underwent emergency LA over a five-year period was performed. Outcomes measured included conversion rate, operative time, complications, and length of hospi…

Indications, Technical Advantages, and Debatable Aspects of Laparoscopic Appendectomy: A Comprehensive Review.

Laparoscopic appendectomy has emerged as the preferred surgical approach for treating acute appendicitis, one of the most common abdominal emergencies. This review explores the key indications, technical benefits and ongoing controversies surrounding laparoscopic appendectomy. While diagnosing acute appendicitis is typically straightforward, atypical presentations due to variable appendix locations can complicate management. Advances in imaging and scoring systems have improved diagnostic accuracy, yet timely surgical intervention remains crucial. Compared to open appendectomy, laparoscopic ap…

Unplanned Readmission Rate Before Interval Appendectomy After Nonoperative Management of Complicated Appendicitis.

BACKGROUND: There remains clinical equipoise regarding the preference for upfront appendectomy or nonoperative management for patients with complicated appendicitis. However, the natural history of the appendix after nonoperative management and pending interval appendectomy in children is not well characterized, and the risk of recurrent appendicitis as a function of time from index admission not known. STUDY DESIGN: The Pediatric Health Information System was queried for all patients younger than 18 years with an ICD-10 diagnosis code for complicated appendicitis admitted between 2018 and 20…
