Spontaneous fluctuation in the size of a midline posterior fossa arachnoid cyst.

Arachnoid cysts of the posterior fossa are rather uncommon compared with their supratentorial counterparts. Spontaneous disappearance of middle cranial fossa arachnoid cysts has been reported but there are none in the English literature on the spontaneous fluctuation in size of a posterior fossa arachnoid cyst. We present a 41-year-old male, with a midline posterior fossa arachnoid cyst, which appeared to lessen in size spontaneously with complete disappearance of symptoms and then enlarged with a worsening in neurological status.

Cauda equina syndrome due to lumbosacral arachnoid cysts in children.

We describe the clinical, neuroradiological and surgical aspects of two children in whom symptoms attributable to cauda equina compression were caused by spinal arachnoid cysts. The first patient presented with recurrent urinary tract infections due to neurogenic bladder dysfunction, absent deep tendon reflexes and sensory deficit in the lower limbs. The second child presented with unstable gait as a result of weakness and diminished sensation in the lower extremities. Spinal magnetic resonance imaging revealed a lumbosacral arachnoid cyst in both patients. During surgery the cysts were identi

Twins with different temporal lobe malformations: schizencephaly and arachnoid cyst.

The etiology and relationships between different forms of malformations of cortical development are poorly understood. Schizencephaly is generally regarded as unrelated to arachnoid cysts. As part of a systematic study of epilepsy in twins we observed a monozygotic twin pair discordant for temporal lobe epilepsy where the twin with epilepsy had unilateral temporal schizencephaly and periventricular heterotopia. The twin without epilepsy had an arachnoid cyst in the same temporal lobe. Although an incidental association is possible, this observation, together with occasional reports of schizenc

Progesterone receptors in arachnoid cysts. An immunocytochemical study in 2 cases.

We report 2 cases of arachnoid cysts, one with a retrocerebellar and the other with a left temporal localization, in which immunohistochemical studies had been conducted. The results of the immunohistochemistry on the presence of carcino-embryonic antigen (CEA) and glial fibrillary acidic protein (GFAP), and of the scanning- and transmission electron microscopy revealed the cyst lining to be identical to subdural neurothelium. Progesterone receptors were found in the nuclei of cells lining the cyst, which also suggests the similarity of the cyst lining to arachnoid granulations and meningiomas

Microsurgical anatomy of the arachnoidal trabecular membranes and cisterns at the level of the tentorium.

A clear and thorough understanding of the neuroanatomical structures of the subarachnoid cisterns is important because they provide natural pathways to intracranial arteries, veins, and nerves during microvascular procedures without disturbing surrounding important brain structures. Using a surgical microscope, we examined the microsurgical anatomy of the trabecular membranes and subarachnoid cisterns in 20 adult cadaver brains. The brains were immersed in Ringer's solution and air was injected into the subarachnoid cisterns while the brains remained submerged in solution. We identified seven

Fluctuating hemiparesis caused by a quadrigeminal arachnoid cyst: case report.

A 42-year-old woman presented with intermittent hemiparesis mimicking a reversible ischemic neurologic deficit. Magnetic resonance imaging (MRI) demonstrated a compression of the left side of the midbrain by a quadrigeminal arachnoid cyst without hydrocephalus. After the cystic wall was widely excised under operating microscope, the patient made a good recovery and remained well at a follow-up of 18 months. The absence of hydrocephalus in a symptomatic patient with a quadrigeminal cyst was exceptional, which might result from the early diagnosis with MRI. The use of microsurgical technique mad

Asymptomatic hypothalamic hamartoma associated with an arachnoid cyst--case report.

A 42-year-old male presented with the complaint of mild left facial numbness. Magnetic resonance imaging demonstrated a solid tumor in the interpeduncular cistern and a huge arachnoid cyst in the left middle cranial fossa. The tumor appeared isointense to the surrounding cerebral gray matter on T1-weighted images and hyperintense to that on T2-weighted images. The tumor was partially resected. Histological findings were characteristic of hamartoma. The mild left facial numbness was probably due to compression of the left trigeminal nerve by the arachnoid cyst. Asymptomatic hypothalamic hamarto

[The radiological findings in surgically and conservatively treated intracranial arachnoid cysts].

PURPOSE: To determine which appearances on CT or MRI are indications for surgical treatment of intracranial arachnoid cysts. METHOD: The images obtained from 26 patients treated either by surgery or conservatively were compared retrospectively. The features evaluated were the size of the cyst, evidence of a space-occupying lesion, cerebral abnormalities near the cyst and skull deformities. RESULTS: In 6 patients (23%), surgical treatment was indicated because of the presence of obstructive hydrocephalus. The presence of all other radiological findings did not differ significantly between the

Successful epidural anesthesia in a patient with an arachnoid cyst, preeclampsia, and triplets.

BACKGROUND AND OBJECTIVES: Arachnoid cysts are a frequent finding on magnetic resonance imaging; however, the use of major conduction anesthesia in patients with these cysts has not been reported. METHODS: Epidural anesthesia was used in a parturient with a triplet pregnancy preeclampsia, and a documented arachnoid cyst extending from TII to LI. RESULTS: Epidural anesthesia was successfully used in this high-risk patient, and airway manipulation, which would have been difficult in this patient, was thereby avoided. CONCLUSIONS: Arachnoid cysts are not a contraindication to major conduction

Arachnoid cysts: does surgery improve epileptic seizures and headaches?

During the period from 1985 to 1992 we treated 43 patients with intracranial arachnoid cysts (ACs). In this retrospective study we assessed the outcome of these patients with regard to non-specific symptoms such as headaches and epileptic seizures. Twelve patients had headaches of obscure origin and a mostly temporal located AC. Six of these 12 underwent surgery. After the operation 4 patients (4/6) had no further headaches, two remained unchanged. The other 6 conservatively treated patients (6/12) had further headaches. Ten of the 43 patients had epileptic seizures. One patient dropped out of

[A case of suprasellar arachnoid cyst followed up for a long time].

We followed a case of suprasellar arachnoid cyst for 12 years. The patient was a sixteen-year-old girl without particular problems in her general condition. She showed optic atrophy in both eyes and optic nerve hypoplasia with an inferotemporal quandranopsia in the left eye. A suprasellar arachnoid cyst communicating with the tubarachnoid space was found to extend into the sella turcica as an empty sella. A cyst wall was resected and a cyst-peritoneal shunt performed. After 12 years from the operation, sensitivity was slightly depressed in the visual field where it had already been disturbed.

Petrous apex arachnoid cyst: radiographic confusion with primary cholesteatoma.

The advent of combined computed tomography (CT) and magnetic resonance imaging (MRI) for the evaluation of petrous apex lesions has improved the otologist's ability to differentiate among many disease processes. Temporal bone CT details osseous anatomy, whereas MRI delineates soft tissue signal characteristics. The employment of these two imaging modalities is often successful in differentiating between cholesterol common entities encountered in the petrous apex. The finding of a smoothly marginated, expansile, bone eroding lesion on CT, coupled with hypointensity on T1-weighted and hyperinten

Primary intracranial arachnoid cyst in the elderly: a survey on 39 cases.

Thirty-nine cases of primary intracranial arachnoid cyst in the elderly, including one case of our own, were analyzed. Clinical characteristics in these patients were as follows. (1) The number of patients decreased with age, but there were no differences according to sex. (2) Clinical manifestations were similar to those of chronic subdural haematoma or normal pressure hydrocephalus including dementia, urinary incontinence, and hemiparesis. General symptoms such as headache and seizures were also present. (3) Surgery was performed in most patients with generally good outcome regardless of ope

Primary intracranial arachnoidal cysts. A study of 67 childhood cases.

Sixty-seven cases (41 males and 26 females) of arachnoidal cysts in children under 11 years are reported. About 53% of cases were diagnosed before 1 year of life. Thirty-one (42.2%) were supratentorial (interhemispheric 9, temporal fossa 10, convexity 5, sylvian fissure 3, supra- and/or retrosellar 4); 31 (46.2%) infratentorial (supra- and/or retrocerebellar 22, foramen of Magendie 3, quadrigeminal cistern 5, pontocerebellar 1); 5 (7.5%) supra- and infratentorial. Macrocephaly was the presenting symptom in 48 cases (71.5%). Associated features were frequent: cranial asymmetry in 24; aqueductal

Stereotactic decompression of a prepontine arachnoid cyst with resolution of precocious puberty.

A case of a prepontine (clival) arachnoid cyst is reported. The patient presented with isosexual precocious puberty. Treatment was initially with fenestration by suboccipital craniectomy, however, definitive treatment was via stereotactic transfrontal placement of a cystoperitoneal shunt. Normalization of endocrine function has occurred and has persisted for 1 year. This case is presented as a contrast to the two approaches for the treatment of arachnoid cysts. The case will be described and the literature reviewed.

[Extradural arachnoid cyst of the dorsal region with relapsing and recurrent development].

An extradural arachnoid cyst of the thoracic spine was diagnosed and operated in a 39 year-old woman with a history of two regressive attacks of spastic paraplegia during a 19 year-period. The diagnosis was established by magnetic resonance imaging and myeloscan, which demonstrated the cyst and its consequences on the spinal cord and the spine. Theories of etiology and development of extradural arachnoid spinal cysts are reviewed.

Clinical and magnetic resonance features of primary intracranial arachnoid cysts.

The clinical features of 33 patients with primary intracranial arachnoid cysts are reviewed. Magnetic resonance imaging was performed in 9 patients and was particularly useful in the multiplanar definition of complex cysts and in differentiating arachnoid cysts from other lesions that are hypodense on computed tomographic scans. Hydrocephalus, cranial nerve palsies, or symptomatic brain compression was considered an indication for surgical intervention in 21 patients. Outcome was uniformly good: mild focal weakness and seizure disorders constituted the sequelae.
