BACKGROUND: Traditional CHA₂DS₂-VASc scoring has well-recognized limitations in thromboembolic risk assessment for atrial fibrillation (AF), particularly in low-score patients who nevertheless thrombose, high-score event-free individuals, cancer patients, and post-ablation cohorts. The left atrial appendage (LAA) is the source of 98% of thrombi in non-valvular AF and exhibits marked inter-individual morphological and hemodynamic variability that traditional scores completely ignore.
AIMS: This narrative review critically evaluates current evidence on LAA morphology, hemodynamics, computationa…

BACKGROUND: Current guidelines recommend oral anticoagulation in atrial fibrillation (AF) patients with a CHA2DS2-VASc score ≥2 for stroke prevention. However, Centers for Medicare & Medicaid Services (CMS) reimburses left atrial appendage occlusion (LAAO) for patients with CHA2DS2-VASc score ≥3. This study aimed to evaluate differences in outcomes and complications of LAAO therapy in AF patients with a CHA2DS2-VASc score of ≥ 3 versus < 3.
METHODS: National Inpatient Sample (NIS) and International Classification of Diseases, Tenth Revision codes were used to identify LAAO device implantation…

The impact of recovered atrial dynamics on pre-existing thrombi within the left atrial appendage (LAA) remains a poorly understood phenomenon, despite its potential clinical implications during spontaneous or induced restoration of sinus rhythm (SR) in patients with atrial fibrillation (AF). While computational studies have investigated thrombus formation during AF, none have examined the fate of established thrombi following SR restoration. This work addresses that gap by modelling thrombus formation and embolisation in the LAA under AF and after the return to normal rhythm. A meshless partic…

BackgroundCancer-associated atrial fibrillation (AF) is increasingly common, posing dual risks of thrombosis and bleeding. Long-term oral anticoagulation is often limited in these patients due to drug interactions, thrombocytopenia, and treatment-related bleeding. Left atrial appendage occlusion (LAAO) offers a non-pharmacological alternative for stroke prevention, yet its optimal use in cancer remains uncertain.MethodsWe conducted a narrative review of studies from 2009 to October 2025 across PubMed, Embase, and Google Scholar, including randomized trials, observational studies, registries, c…

BACKGROUND: Stroke prevention after transcatheter aortic valve replacement (TAVR) in patients with atrial fibrillation remains clinically challenging because thromboembolic risk must be balanced against substantial bleeding vulnerability. Left atrial appendage occlusion (LAAO) has been proposed as a nonpharmacologic alternative to long-term anticoagulation in selected patients, but its comparative role in the TAVR population remains insufficiently defined.
METHODS: We performed a systematic review and meta-analysis of comparative studies evaluating TAVR plus LAAO versus TAVR plus medical ther…

BACKGROUND: Left atrial appendage (LAA) patency after catheter ablation and LAA closure (LAAC) is a key determinant of postprocedure stroke. The effect of LAA electrical isolation (LAAEI) on LAA patency after LAAC in patients with atrial fibrillation (AF) remains unknown. Our objective was to explore the effect of LAAEI on LAA patency after catheter ablation and LAAC in patients with AF.
METHODS: Patients with persistent AF who underwent combined catheter ablation and LAAC were enrolled. LAA patency was detected by cardiac computed tomographic angiography at 3 months after the procedure.

Atrial fibrillation (AF) and atrial flutter (AFL) are common arrhythmias, with incidence increasing with age. Catheter ablation and left atrial appendage closure (LAAC) are guideline-recommended treatments for patient with AF and high thromboembolic risk of stroke. We report the case of a patient with paroxysmal AF/AFL at high thromboembolic risk who developed paroxysmal rapid AF with sinus arrest following combined pulmonary vein isolation (PVI) and LAAC. Notably, both the arrhythmia and sinus node dysfunction gradually resolved over three months without pacemaker implantation. We propose tha…

PURPOSE: Whether de novo organized left atrial tachyarrhythmia (LAT) shows preserved left atrial appendage (LAA) mechanics has not been adequately characterized. We compared LAA peak emptying velocity across atrial fibrillation (AF) phenotypes and de novo LAT.
METHODS: Multicenter retrospective cohort of 634 consecutive first-ablation candidates undergoing pre-ablation transesophageal echocardiography (TEE) at five centers: paroxysmal AF (PAF, n = 146), persistent AF (PeAF, n = 310), long-standing persistent AF (LsPeAF, n = 86), and LAT (n = 92). Markedly reduced LAA function was peak emptyin…

BACKGROUND: It remains unclear whether concomitant or subsequent left atrial appendage closure (LAAC) may affect atrial fibrillation (AF) ablation outcomes.
OBJECTIVES: This study sought to evaluate the impact of LAAC on AF recurrence and its role in clinical outcomes.
METHODS: A subanalysis was conducted on 1,600 patients from the randomized OPTION (Comparison of Anticoagulation With Left Atrial Appendage Closure After AF Ablation) trial. Patients were stratified into 2 groups: anticoagulation (no device) and LAAC. The primary endpoint was AF recurrence, and the secondary endpoint was strok…

Anterolateral thoracotomy is frequently used for the closure of ostium secundum atrial septal defect (ASD). The cosmetic result has become an important feature. There are various complications of anterolateral thoracotomy like persistent postoperative pain, phrenic nerve injury, atelectasis, and bleeding. We report a case of ASD closure through anterolateral thoracotomy who had bleeding of the left atrial appendage (LAA), which is an unusual and rare complication.

BACKGROUND: Left atrial appendage closure (LAAC) combined with radiofrequency catheter ablation (RFCA) as a hybrid procedure is commonly performed to treat atrial fibrillation (AF). Although this treatment carries a low risk of coronary artery spasm (CAS), and has never been observed in LAAC procedure, caution still needed to be taken. We presented a case of CAS that occurred in an AF patient during the hybrid procedure.
CASE PRESENTATION: The patient was a 65-year-old man with paroxysmal AF who developed CAS during RFCA and LAAC. In this case, LAAC was performed ahead of RFCA. After atrial s…

INTRODUCTION: Left atrial appendage (LAA) thrombus in patients with atrial fibrillation is usually detected by transesophageal echocardiography (TEE). Intracardiac echocardiography (ICE) can be a suitable alternative to detect thrombosis. However, the effectiveness of the two methods for detecting LAA thrombus is still unclear, we performed a meta-analysis that compared ICE versus TEE for LAA thrombosis.
METHODS: We searched PubMed, Cochrane Library, and Embase for published abstracts and manuscripts on June 1, 2020. The analysis was performed using RevMan 5.3, STATA 15, and Meta-Disc 1.4.

Many patients with hereditary hemorrhagic telangiectasia (HHT) are unable to sustain oral anticoagulation (OAC) because of severe epistaxis, gastrointestinal (GI) bleeding and the risk of life-threatening bleeding from cerebral arteriovenous malformations (CAVMs) or pulmonary arteriovenous malformations (PAVMs). In patients with atrial fibrillation (AF), most thromboembolic complications arise from the left atrial appendage (LAA) and percutaneous transcatheter LAA closure proved to be non-inferior to OAC at mid-term follow-up. We report our experience with LAA closure in HHT with a follow-up o…

BACKGROUND: Left atrial appendage occlusion devices are cost effective for stroke prophylaxis in atrial fibrillation when compared with dabigatran or warfarin. We illustrate the use of value-of-information analyses to quantify the degree and consequences of decisional uncertainty and to identify future research priorities.
METHODS AND RESULTS: A microsimulation decision-analytic model compared left atrial appendage occlusion devices to dabigatran or warfarin in atrial fibrillation. Probabilistic sensitivity analysis quantified the degree of parameter uncertainty. Expected value of perfect inf…

BACKGROUND: Left atrial appendage (LAA) is implicated in maintenance of atrial fibrillation (AF) and atrial tachycardia (AT) associated with persistent AF (PsAF) ablation, although little is known about the incidence and mechanism of LAA AT.
OBJECTIVE: The purpose of this study was to characterize LAA ATs associated with PsAF ablation.
METHODS: In 74 consecutive patients undergoing stepwise PsAF ablation, 142 ATs were encountered during index and repeat procedures. Out of 78 focal-source ATs diagnosed by activation and entrainment mapping, 15 (19%) arose from the base of LAA. Using a 20-pole…
