[Electrophysiological action of disopyramide].

The paper presents data on an electrophysiologic action of disopiramid injected intravenously in a dose of 2 mg/kg on the function of the atrioventricular conduction system in 11 patients with paroxysmal supraventricular tachycardias. Disopiramid reduced the length of the spontaneous cardiac cycle and slightly affected the corrected sinus node recovery time and sinoatrial conduction time. The drug prolonged the intraatrial conduction time, increased the refractoriness of the His-Purkinje system and accessory conduction pathways. It produced no significant action on the refractory period and th

Differential effects of anesthetics and sex on supraventricular electrophysiology and atrial fibrillation substrate in rats.

Rodents are increasingly used in atrial electrophysiology research, yet such studies are often performed under anesthesia owing to technical … tal (PEN). Pacing protocols determined various parameters, including sinoatrial node recovery time, atrioventricular node effective refractory period and atrial effective refractory period. Arrhythmic substrate was assessed after 20 triggering bursts per condition, and arrhythmic tendency was analyzed manually and through the complexity ratio, an unbiased measure recen …

Less Is More: A Case of Delayed Return of Normal Conduction.

Conduction disturbances are a well-recognized complication of acute myocardial infarction (MI), particularly when the infarct involves the inferior wall and jeopardizes blood flow to the atrioventricular node. An 82-year-old woman admitted for symptomatic anemia developed an inferior myocardial infarction complicated by 2:1 atrioventricular block, later progressing to complete heart block. She underwent percutaneous intervention for a critical distal right coronary artery lesion and required temporary transvenous pacing. Although permanent pacemaker implantation was initially considered, the p…

Atrial inhibited pacing in the sick sinus node syndrome: clinical value and the demand for rate responsiveness.

A consecutive series of 66 patients (males = 32; mean age +/- SD = 71 +/- 9 years) given atrial inhibited pacemakers for sick sinus nodes were followed to study the incidence of lead failures, chronic atrial tachyarrhythmias, and atrioventricular conduction disturbances. The need for rate responsive pacing was also assessed. Pre and postoperative investigation could include carotid sinus massage, Holter monitoring, exercise testing, and invasive electrophysiology. The mean follow-up time +/- SD was 32 +/- 29 months (median = 26 months). Three patients (5%) had their pacemakers replaced due to

A Rare Case of Dual, Decrementally Conducting, Pathway-to-Pathway-Mediated Tachycardia.

We report a rare case of broad complex atrioventricular reentrant tachycardia (AVRT) in an 18-year-old female, mediated by two distinct decrementally conducting accessory pathways (APs). The reentrant circuit consisted of antegrade conduction via an atrio-fascicular (AF) fiber and retrograde conduction through a concealed posteroseptal AP. Both APs exhibited decremental properties, and the circuit was independent of the atrioventricular (AV) node and His-Purkinje system, representing a classic example of pathway-to-pathway reentry. Sequential ablation of the retrograde and then the antegrade l

A Curious Change in QRS Width: Timing Is Everything.

CASE BACKGROUND: A 61-year-old man with a mechanical aortic valve developed Staphylococcus aureus bacteremia. His electrocardiogram showed sinus rhythm with atrioventricular Wenckebach and phase 4 left bundle branch block. He was subsequently found to have a peri-aortic root abscess that was likely eroding not only into the atrioventricular node, but into the left bundle fibers as well. TAKE-HOME MESSAGES: Phase 4 block is a pause-dependent conduction block that reflects underlying His-Purkinje system disease. During a critical pause, spontaneous phase 4 depolarization in a diseased His-Purki…

An Unpredictable Atrioventricular Accessory Pathway: Low-Risk or High-Risk?

A 66-year-old man with known asymptomatic preexcitation was admitted to our department for symptomatic complete infranodal atrioventricular block and scarce intermittent anterograde conduction through the atrioventricular accessory pathway. The accessory pathway was not further stratified due to evidence of intermittent conduction with a sinus node heart rate of 60 bpm, and a bicameral pacemaker was implanted. Surprisingly, after 4 months, the patient developed preexcited, rapidly conducted atrial fibrillation and ultimately underwent successful accessory pathway catheter ablation. Even when i

Spontaneous Transition of Double Tachycardias with Atrial Fusion in a Patient with Wolff-Parkinson-White Syndrome.

Among patients with Wolff-Parkinson-White syndrome, atrioventricular reciprocating tachycardia (AVRT) and atrioventricular nodal reentrant tachycardia (AVNRT) can coexist in a single patient. Direct transition of both tachycardias is rare; however, it can occur after premature atrial or ventricular activity if the cycle lengths of the two tachycardias are similar. Furthermore, persistent atrial activation by an accessory pathway (AP) located outside of the AV node during ongoing AVNRT is also rare. This article describes a case of uncommon atrial activation by an AP during AVNRT and gradual tr

[Acute lymphoblastic leukemia with invasive fungal infection causing complete atrioventricular block].

A 62-year-old female was diagnosed as having acute lymphocytic leukemia in October 2001. She achieved complete remission after induction therapy with L-asparaginase, doxorubicin, vincristine and prednisolone. However, an invasive fungal infection developed in the lung, followed by a sudden complete atrioventricular block. Her heart rate was brought back to normal by the administration of isoproterenol. In spite of anti-fungal therapies, the patient finally succumbed to pneumonia in January 2002. Autopsy revealed an invasion of fungi into the lung, mitral valves and atrioventricular node. We ne…

[Familial junctional ectopic tachycardia: clinical and pathological findings].

We describe two brothers with a neonatal diagnosis of junctional ectopic tachycardia. The first brother presented hydrops fetalis secondary to narrow QRS tachycardia at a rate of 230-300 beats/min with atrioventricular dissociation. Although the ventricular rate was controlled with intravenous amiodarone the baby died a few hours after initiation of this treatment from ventricular fibrillation and electromechanical dissociation. Histological examination of the conduction system showed diffuse hemorrhage and necrosis of the atrioventricular node and His' bundle. The second brother presented fet…

Role of the surface electrocardiogram in the diagnosis of patients with supraventricular tachycardia.

In this era of interventional electrophysiology, the accuracy of the electrocardiogram in diagnosis of supraventricular tachycardia could be improved by detailed endocardial mapping and confirmed by results of radiofrequency catheter ablation. This article describes the electrocardiographic characteristics for different types of supraventricular tachycardia: atrial fibrillation, atrial flutter, atrial tachycardia, atrioventricular reciprocating tachycardia using an accessory pathway, and atrioventricular node reentrant tachycardia. Several limitations, including the identification of P wave mo…

Atrial echo beats as a novel mechanism of periodic fluctuation in cycle length of atrial tachycardia.

Atrial echo beats due to a dual-atrioventricular nodal pathway are a possible cause of fluctuating tachycardia in cycle length (CL) but have not been reported so far. Here, we present a case of an 82-year-old man with symptomatic atrial tachycardia (AT), which was accompanied by periodic fluctuations in atrial sequence at the coronary sinus. Electrophysiological study (EPS) for atrioventricular conduction and three-dimensional (3D) electro-anatomical mapping system explained that the mechanism of periodic fluctuations was due to atrial echo beats through a dual-atrioventricular nodal pathway.

First-degree atrioventricular block in patients with atrial fibrillation and atrial flutter: the prevalence of intra-atrial conduction delay.

PURPOSE: PR interval prolongation > 200 ms resulting in the diagnosis of first-degree atrioventricular block (AVB1) is caused by a delay in the AV nodal/His conduction and/or the right intra-atrial conduction (RIAC). The aim of the study was to assess the prevalence of AVB1 due to RIAC delay (AVB1 with normal AH and HV) in patients with atrial fibrillation (AF) and atrial flutter (AFlu). METHODS: We included 1067 consecutive patients (33% female, age 63 ± 13 years) referred for catheter ablation of AF (AF-group) (453 patients), AF and AFlu (136 patients), AFlu (292 patients), and AVNRT/AVRT (

The effect of atrial pacing site on electrophysiological properties of the atrioventricular junction and induction of atrioventricular nodal reentry in patients with typical atrioventricular nodal reentrant tachycardia.

BACKGROUND: Clinical studies in humans have shown the site of atrial stimulation to influence atrioventricular (AV) conduction times and refractory periods, the demonstration of dual AV nodal (AVN) pathways, and induction of AVN reentry. These studies often found conflicting results. Moreover, among enrolled patients a minority of them were found to have AVN reentrant tachycardia (AVNRT). AIM: The purpose of this study was to investigate the effect of right and left atrial pacing on the electrophysiological properties of the AV junction in the typical AVNRT population. METHODS: Ninety-two co
