Characteristics of older people who experience high-cost hospital care at the end of life and report low-satisfaction: Evidence of low-value care from bereaved carer surveys in England.

BACKGROUND: A significant proportion of healthcare costs is incurred in hospitals at the end of life (EoL), with a small group of patients accounting for most of the costs. Scarce resources should be allocated based on cost-effectiveness. High costs and low satisfaction with care suggest low-value care at EoL; however, there is little evidence regarding which patients receive such care. OBJECTIVES: To identify the characteristics of older patients who experience high-cost care but report low satisfaction with care at EoL. DESIGN AND METHODS: Secondary data analysis of mortality follow-back s

Psychological stress and hospitalization for childhood asthma-a nationwide cohort study in two Nordic countries.

OBJECTIVE: Exposures to psychological stress in early life may contribute to the development or exacerbation of asthma. We undertook a cohort study based on data from several population-based registers in Denmark and Sweden to examine whether bereavement in childhood led to increased asthma hospitalization. METHODS: All singleton children born in Denmark during 1977-2008 and in Sweden during 1973-2006 were included in the study (N=5,202,576). The children were followed from birth to the date of first asthma hospitalization, emigration, death, their 18(th) birthday, or the end of study (31 Dec…

The Denial of Death: A Three-decade Long Case of Absent Grief.

Bereavement reactions are associated with numerous physical and mental complications. Atypical bereavement reactions have been described but their place in the classificatory system has not been established. We present the case of an eighty-year-old woman who came with the belief that her deceased son was alive. We discuss the diagnostic dilemma she posed and conclude that it may be difficult to differentiate atypical bereavement from other psychiatric illnesses.

Unequal loss: Disparities in relational closeness to a COVID-19 death among U.S. older adults.

BACKGROUND: COVID-19 mortality occurred unevenly across U.S. demographic subgroups, leaving some communities harder hit than others. Black and Hispanic/Latino older adults are among those disproportionately affected by COVID-19 mortality, and in turn, COVID-19 bereavement. Because disparities in COVID-19 mortality may extend to COVID-19 bereavement, it is important to understand the incidence of COVID-19 bereavement among older adults at various degrees of relational closeness (e.g., spouse vs. household member vs. friend). METHODS: We used the National Social Health and Aging Project (NSHAP)…

Factors Affecting Communication Patterns between Oncology Staff and Family Members of Deceased Patients: A Cross-Sectional Study.

OBJECTIVE: Perceptions of the role of oncology medical staff in supporting bereaved families have evolved with the transition to interdisciplinary cancer care. We investigated the interactions between oncology professionals and bereaved families. METHODS: This cross-sectional study involved all oncology medical staff at the Davidoff Center. Participants were given a questionnaire relating to bereavement follow-up. Responses were measured using a 5-point Likert scale. RESULTS: Of 155 staff members, 107 filled questionnaires with <20% missing data and were included in the analysis (α = 0.799; …

Brief report: sedatives for mothers of stillborn infants: views from a national survey of obstetricians.

Although no guidelines support the use of benzodiazepines in bereavement care, clinicians continue to use this medication for surviving family members. An anonymous survey about perinatal death was sent to 1500 United States obstetricians. One question assessed beliefs about prescribing sedatives to bereaved mothers. Almost half (49%) of 804 obstetricians endorsed the use of sedatives, such as benzodiazepines, for acute bereavement. Grieving families may experience mental and physical anguish after a death; however, benzodiazepines are not part of recommended care for bereavement. The surprisi…

Improving Patient-Reported End-of-Life Data Capture: A Pilot Mixed-Methods Evaluation of an Electronic Patient Reported Outcomes Chatbot Platform Using Messaging Applications From Bereaved Caregivers in Chile.

PURPOSE: To evaluate feasibility, death-anchored timeliness, and acceptability of a messaging-based electronic patient reported outcomes (ALVA) for after-death Quality of Dying and Death (QODD) collection from bereaved caregivers in Chile. METHODS: We conducted a single-arm, mixed-methods pilot study. After consent, caregivers completed the Spanish QODD via ALVA on common messaging apps (no installation). The primary end point was completion among registered participants; on-time was prespecified as 14-84 days after death. Proportions include 95% CIs. RESULTS: Of 36 invited, 21 registered; p

What parents of children who have received emergency care think about deferring consent in randomised trials of emergency treatments: postal survey.

OBJECTIVE: To investigate parents' views about deferred consent to inform management of trial disclosure after a child's death. METHODS: A postal questionnaire survey was sent to members of the Meningitis Research Foundation UK charity, whose child had suffered from bacterial meningitis or meningococcal septicaemia within the previous 5 years. Main outcome measures were acceptability of deferred consent; timing of requesting consent; and the management of disclosure of the trial after a child's death. RESULTS: 220 families were sent questionnaires of whom 63 (29%) were bereaved. 68 families

A role for event centrality in prolonged grief disorder.

Having a traumatic or negative event at the centre of one's identity is associated with adverse psychological outcomes including post-traumatic stress, depression, and prolonged grief disorder (PGD). However, direct investigation of the role of centrality of a bereavement-event in the maintenance of PGD symptoms is scarce and has not compared immediate and long-term changes in event centrality nor examined the nature of the loss. Data from bereaved partners and adult children in The Aarhus Bereavement Study at four time points over 26 months post-loss were included in this study. Participants …

Antecedents and sequelae of sudden parental death in offspring and surviving caregivers.

OBJECTIVES: To examine the psychiatric antecedents that put parents at risk for early death, and the psychological sequelae of bereavement in offspring and caregivers. DESIGN: A population-based study. SETTING: Bereaved families were recruited through the coroner's records and by advertisement. Control families were recruited by random-digit dialing and advertisement. PARTICIPANTS: Families with biological offspring from 7 to 25 years of age in which 1 parent died of suicide, accident, or sudden natural death were included (n = 140). Controls (n = 99) had 2 living parents and their biologic…

Maladaptive coping in adults who have experienced early parental loss and grief counseling.

This study compares maladaptive coping, measured as substance use, behavioral disengagement, self-blame, and emotional eating, among adults (>18 years) who have experienced early parental loss ( N = 1465 women, N = 331 men) with non-bereaved controls ( N = 515 women, N = 115 men). We also compared bereaved adults who received grief counseling ( N = 822 women, N = 190 men) with bereaved controls who had not ( N = 233 women, N = 66 men). Bereaved adults reported significantly more substance use, behavioral disengagement, and emotional eating than non-bereaved adults. Counseling participants repo

Prolonged grief disorder in end-of-life care: current literature and future directions.

Prolonged grief disorder (PGD) is a recognized distinct mental health disorder in the DSM-5-TR and ICD-11, characterized by core symptoms such as longing for and preoccupation with the deceased that persist and worsen over time. Family members and caregivers of patients that receive end-of-life care are confronted with imminent bereavement, which emphasizes the importance of bereavement care as a preventive measure and the need for continued support after death beyond the hospital setting. This review discusses bereaved family members' and caregivers' risk factors of developing PGD, screening …

What About the Family? A Content Analysis of International Legislative Frameworks and Guidelines on Assisted Dying.

When a person chooses assisted dying, the impact on their family can be profound. Legislation and professional guidelines not only regulate and standardize practice, but also support the delivery of high-quality care. Within this framework, bereavement care plays a vital role by helping families prepare for the death and potentially reducing the risk of prolonged grief. This study explored how current legislation and clinical guidelines on assisted dying address the role and needs of family members, particularly regarding bereavement care. It examined the frequency of the term "family" and its…

Embodied Grief in the Shadow of Violence: An Integrative Conceptual Review of Widowhood, Trauma, and Social Constraint.

Violent widowhood remains among the least theorized forms of bereavement. While dominant Western psychological models have advanced grief theory, they do not adequately explain grief following intentional spousal loss amid violence, ongoing threat, and gendered social regulation. This integrative conceptual review synthesizes interdisciplinary and cross-cultural scholarship on violent widowhood, traumatic bereavement, embodiment, structural violence, and social constraint to develop a context-sensitive framework for grief after intentional spousal loss. Four interrelated dimensions are identif…

Symptom Structure of Depression and Anxiety in Mothers Following Child Loss: A Network and Bayesian Graph Analysis.

BACKGROUND: Bereavement, especially following the loss of a child, is a profoundly distressing life event associated with heightened risks of depression and anxiety. However, limited evidence exists on the symptom-level structure and statistical interrelations of these conditions among bereaved individuals, particularly in low- and middle-income countries (LMICs). METHODS: Using nationally representative data from the 2022 Bangladesh Demographic and Health Survey (BDHS), we identified 2276 bereaved mothers. We applied psychological network analysis to estimate the partial correlation network …
