[May Medically Assisted Procreation (MAP) be relevant for homosexual women? Study among 147 gynaeco-logists involved in MAP techniques].

The second part of the twentieth century has seen societal modifications as well as evolution of medical techniques allowing now thinking hu … ders - and 5% deny good maternal abilities to homosexual women. Before the so-called french laws of bioethics in 1994, none of them had practiced a donor insemination for a lesbian couple, though 4% had realized some for single homosexual women. Clinical, Ethical, and Financial Considerations With Red Blood Cell Transfusion Support at the End of Life in Hematologic Malignancies.

Patients with advanced hematological malignancies often have red blood cell (RBC) transfusion needs at the end of life (EOL) to improve debi … ons at the EOL for patients with hematological malignancies by drawing on literature in hematology, bioethics, and palliative care. One notable clinical consideration is that RBC transfusions improve anemia-related symptoms, such as fatigue, weakness, and dyspnea. At the same time, ethical considerations include the scarcity of blood. Moreover, some financi …

From designer babies to edited food: comparing public attitudes toward CRISPR technology and gene editing in Japan and the United States.

The perception of bioethics is said to vary based on culture and religion, and research has often been confined to specific topics such as genome-edited babies. In this study, four different scenarios using CRISPR technology and gene editing (designer babies, de novo embryo production, crop improvement, and malaria mosquito control) were presented to measure the attitudes of people in Japan and the United States. The most influential demographic factor was age, with older individuals in both countries expressing greater concern. In the United States, the higher the educational level and annual…

Ethical and Legal Considerations in Posthumous Oocyte Retrieval for Minor Girls.

Posthumous gamete retrieval has emerged as a complex issue at the intersection of reproductive technology, bioethics, consent, and law. While posthumous sperm retrieval has received small amounts of legal, ethical, and clinical attention, posthumous oocyte retrieval, particularly in minors, remains largely unaddressed. This review examines the clinical, legal, and ethical considerations surrounding posthumous oocyte retrieval in minor girls. First, the paper explores the medical processes involved in posthumous gamete retrieval, drawing a significant distinction between sperm and oocyte retrie…

Ritualized relational autonomy in Chinese end-of-life care: balancing Confucian benevolence and individual rights.

This article develops a hypothesis-and-theory framework for ethical tensions in contemporary Chinese end-of-life care and family mourning. I … rsect: in Confucian ethics, the body is the medium of care, devotion, and mourning, while in modern bioethics it is the bearer of autonomy and consent. On this basis, the article proposes a framework of ritualized relational autonomy, structured through three stages: care agreements, family meetings, and care-termination rituals. This framework aims to prev …

Emphasizing Patient-Centricity Through a Tailored Training Program to Empower Patients, Advocates, and Ethics Committees in Good Clinical Practice.

ObjectivesGood Clinical Practices (GCP) are essential for patient-centric research. The standard bioethics and GCP training emphasizing a "one-size-fits-all" approach may not adequately equip ethics committee members, especially the lay and social scientist members, towards their critical role in reviewing clinical trials and related documentation. This article explores a patient-centered, patient advocates-driven training program focused on raising awareness about research ethics and GCP among patients, advocates and ethics committee members.MethodsA patient advocates-driven program called Pa…

An International Legal Review of the Relationship between Brain Death and Organ Transplantation.

The "dead-donor rule" states that, in any case of vital organ donation, the potential donor should be determined to be dead before transplantation occurs. In many countries around the world, neurological criteria can be used to legally determine death (also referred to as brain death). Nevertheless, there is considerable controversy in the bioethics literature over whether brain death is the equivalent of biological death. This international legal review demonstrates that there is considerable variability in how different jurisdictions have evolved to justify the legal status of brain death an…

[Altruism and the donor].

On December 20, 1988, the government of France passed a law to protect people who voluntarily participate in biomedical research. This article makes extensive reference to a major study, titled From Biology to Ethics, by Jean Bernard, a well-respected authority in the field of bioethics. The author looks at models proposed by Bernard, as examples for health volunteers, in particular, the blood donor and the self-experimenter. To set the tone of the article, she recalls the concept of altruism, as first proposed by Auguste Comte, then makes a linkage between his philosophy and Bernard's point o…

The Silver Lining in Gain-of-Function Experiments with Pathogens of Pandemic Potential.

Half a decade after the contentious "gain-of-function" (GOF) debate of 2012 that followed experimentation showing that highly pathogenic avian influenza virus could become mammalian transmissible, it is possible to reflect on the arguments for and against this type of research. In this essay we argue that GOF-type experiments have already produced important information not available from any other source while also providing information on pathogenesis and the requirements for optimizing strains for vaccine production. We analyze the moral arguments against GOF and find them less compelling fo

Newborns in crisis: An outline of neonatal ethical dilemmas in humanitarian medicine.

Newborn infants are among those most severely affected by humanitarian crises. Aid organisations increasingly recognise the necessity to provide for the medical needs of newborns, however, this may generate distinctive ethical questions for those providing humanitarian medical care. Medical ethical approaches to neonatal care familiar in other settings may not be appropriate given the diversity and volatility of humanitarian disasters, and the extreme resource limitations commonly faced by humanitarian aid missions. In this paper, we first systematically review existing guidelines relating to

A matter of respect: a defense of the dead donor rule and of a "whole-brain" criterion for determination of death.

Many accounts of the historical development of neurological criteria for determination of death insufficiently distinguish between two strands of interpretation advanced by advocates of a "whole-brain" criterion. One strand focuses on the brain as the organ of integration. Another provides a far more complex and nuanced account, both of death and of a policy on the determination of death. Current criticisms of the whole-brain criterion are effective in refuting the first interpretation, but not the second, which is advanced in the 2008 President's Council report on the determination of death.

The best-interest standard is not applied for neonatal resuscitation decisions.

OBJECTIVE: Legal and ethical standards require resuscitation when it is considered to be in the patient's best interest. We hypothesized that newborn infants might be dealt with according to different standards, compared with older patients. METHODS: An anonymous questionnaire describing 8 currently incompetent patients with potential neurologic sequelae who required resuscitation was administered to groups of physicians and students. Survival and morbidity rates were explicitly described; a very preterm infant, a full term infant and a 2-month-old infant had identical outcomes. Two patients,

[Ethical problems of research on medical-assisted reproduction].

Research in the field of assisted reproduction technologies (ART) is today very active internationally and is aimed both at improving success chances of already consolidated techniques (in fact these chances are still considerably low), and at elaborating new methods like ICSI (intracytoplasmic sperm injection), oocyte cryoconservation, ovary tissue cultures. Some other techniques, connected to ART, are here considered, like preimplantation diagnosis, early sex determination, gene therapy in utero and cloning. All these subjects of research are here briefly mentioned in relation to the ethical

Appropriateness of behavioural scales in the monitoring of pain in the critically ill patient unable to self-report.

Appropriateness is a dimension of quality that evaluates the effective use of technologies, resources or interventions in specific situations or populations, assessing whether our interventions do more benefit than harm. The evidence regarding pain monitoring in the critically ill patient points to the periodic assessment of pain using appropriate tools, with the aim of improving pain management and more efficient use of analgesics in the intensive care unit. The first step would be to assess the patient's ability to communicate or self-report and, based on this, to select the most appropriate

Adult Mesenchymal Stem Cells from Oral Cavity and Surrounding Areas: Types and Biomedical Applications.

Adult mesenchymal stem cells are those obtained from the conformation of dental structures (DMSC), such as deciduous and permanent teeth and other surrounding tissues. Background: The self-renewal and differentiation capacities of these adult stem cells allow for great clinical potential. Because DMSC are cells of ectomesenchymal origin, they reveal a high capacity for complete regeneration of dental pulp, periodontal tissue, and other biomedical applications; their differentiation into other types of cells promotes repair in muscle tissue, cardiac, pancreatic, nervous, bone, cartilage, skin,

Central role of altruism in the recruitment of gamete donors.

This paper explores problems associated with using altruism as the central value in gamete donation, and in doing so draws on empirical data … removing barriers and providing incentives. A final question, triggered by the Nuffield Council on Bioethics' report, is whether the meaning of the payment should be decided on the basis of an a priori definition or on the basis of the donors' subjective experience. It is concluded that there are different legitimate core values in donation, which should b …

Because we can: clashes of perspective over researcher obligation in the failed PrEP trials.

This article examines the relationship between bioethics and the therapeutic standards in HIV prevention research in the developing world, focusing on the closure of the pre-exposure prophylaxis (PrEP) trials in the early 2000s. I situate the PrEP trials in the historical context of the vertical transmission debates of the 1990s, where there was protracted debate over the use of placebos despite the existence of a proven intervention. I then discuss the dramatic improvement in the clinical management of HIV and the treatment access movement, and consider how these contexts have influenced rese…
