Isolated caudate nucleus lesions have only rarely been documented to cause focal extrapyramidal dysfunction. Two cases with possible infarcts in the head of left caudate nucleus presenting with contralateral tremors and blepharospasm with hemifacial spasm are reported. The possible mechanisms for such a presentation are discussed.

Between 1965 and 1976, 95 patients with hemifacial spasm and blepharospasm have been operated according to the method developed by Fisch at the ENT-Clinic of the University of Zurich. 69 of the patients were followed over an interval of 1 to 12 years. 80% of the patients with hemifacial spasm and 65% of the patients with blepharospasm were free of symptoms or significantly improved. These results remained unvaried over an observation period of 6 to 12 years. The motility of the face was preserved to a very high degree in spite of extensive resection of peripheral facial nerve branches.

Thirty-nine patients with the idiopathic blepharospasm-oromandibular dystonia syndrome are described. All presented in adult life, usually in the sixth decade; women were more commonly affected than men. Thirteen had blepharospasm alone, nine had oromandibular dystonia alone, and 17 had both. Torticollis or dystonic writer's camp preceded the syndrome in two patients. Eight other patients developed toritocollis, dystonic posturing of the arms, or involvement of respiratory muscles. PURPOSE: To describe a practical clinical technique that can be used to control the effect of blepharospasm during applanation tonometry.
METHODS: The Volk Superfield NC Lid Lens Adapter is used to keep the eye open and prevent the squeezing effect of orbicularis oculi in blepharospasm.
RESULTS: This helpful tool can be used when blepharospasm becomes a problem that interferes with accurate intraocular pressure measurement.
CONCLUSIONS: The technique is potentially useful and relatively easy to perform.
