PURPOSE OF REVIEW: Although blood transfusion has been felt to be a beneficial therapy for sickle cell disease (SCD) since the 1950s, associated complications initially limited this therapy for these patients. With advances now reducing the side effects of transfusion and several landmark studies over the last decade clearly defining the efficacy for decreasing sickle cell morbidity, the indications for transfusion have increased. This review will discuss the indications, methods and goals of transfusion as well as complications and recent changes in transfusion therapy for SCD.

Stroke is a major cause of neurological disability across all countries in the world. Functional recovery after stroke is limited because of neuronal death and degeneration. Although early reperfusion therapy may improve outcome, thrombolysis does not reverse ischaemic neuronal death and carries the risk of cerebral haemorrhage. Early trials of stem cell therapy in promoting functional recovery after completed stroke remain inconclusive. Based on experimental data, we propose human umbilical cord blood transfusion as a possible therapy for ischaemic cerebral stroke to aid functional recovery.

A retrospective study of 1,402 patients who underwent primary total knee arthroplasty (TKA) (1,194 unilateral, 208 bilateral) was performed. The strongest predictors for allogenic transfusion after surgery were advancing age (P<.001), low preoperative hemoglobin (P<.001), and the use of low-molecular-weight heparin postoperatively (P<.01). Pre-donation of 1 unit of autologous blood before TKA decreased the allogenic transfusion rate from a baseline of 38% to 11%, whereas pre-donating 2 units lowered the rate of breakthrough transfusion of allogenic blood to 7%. A patient with a preoperative he

OBJECTIVE: Blood conservation has become one of the most important issues in cardiac surgery. We clarified preoperative predictors of the need for blood transfusions during coronary artery bypass graft surgery.
METHODS: Subjects were 89 patients--66 men (74%) and 23 women (26%) 40 to 84 years old (mean: 66.2 +/- 8.3 years)--undergoing isolated coronary artery bypass surgery from September 1997 to December 1999. Of these, 66 patients (74%) received transfusion during hospitalization and 23 (26%) did not. Nine risk factors detected by univariate study were entered in a multivariate logistic reg…

Although home transfusion programmes are relatively common in the USA (Anon, 1990, Home Care in the 1990s. Council on Scientific Affairs. Journal of the American Medical Association, 263, 1241-1244), this type of treatment has only recently been considered in Britain and, where it is in operation, is generally supervised by trained nursing staff or via a hospice. North Middlesex Hospital now has 3 years experience of a home transfusion programme operating for beta-thalassaemia major patients, in which relatives are trained and responsible for supervision of the red-cell transfusions at home. A chronically ill, 80-year-old man received over three fourths of a unit of CPDA-1 red blood cells contaminated with Yersinia enterocolitica serotype 0:3. He experienced rigors, high fever, elevated blood pressure, and increased pulse rate. He was treated with antibiotics and discharged in stable condition 6 days later. The blood donor, an Air Force recruit, was asymptomatic at the time of donation but recalled having had diarrhea 4 days earlier. In addition, several other flight members were reported to have had diarrhea at the same time. Serologic evidence suggested that the donor had had a

OBJECTIVE: To present an up-to-date review of the literature encompassing all important aspects of fetal transfusion for red blood cell alloimmunization in pregnancy.
DATA SOURCES: A MEDLINE computer data base search was conducted for pertinent articles through August 1995. Additional publications were identified by cross-referencing.
METHODS OF STUDY SELECTION: All pertinent references were reviewed by the authors, and their clinical significance in the fetal treatment of red blood cell alloimmunization was summarized.
TABULATION, INTEGRATION, AND RESULTS: Fetal intraperitoneal transfusion

The risks associated with homologous blood transfusion necessitates the development of strategies for reducing the need for it. The most effective method is certainly preoperative donation of autologous blood, which leads to an increase in the absolute numbers of erythr789789 by the time surgery is performed. Depending on the type of preparation and storage, erythrocytes may be viable for between 49 days (liquid storage) and many years (deep frozen). By employing preoperative plasmapheresis, high-quality autologous fresh frozen plasma can be made available for use during surgery. Donation of a…

The prevalence of transmissible viruses, human immunodeficiency (HIV) and hepatitis B (HBV) in blood donors, recipients and blood bank staff in a Calcutta (eastern India) based blood bank and transfusion centre has been studied from 1987-93. HIV seropositivity of blood donors was of recent emergence and was low. Recipients of blood and blood components frequently i.e., haemophilics showed a progressive increase in HIV seropositivity since 1988 whereas in thalassaemics the emergence of HIV seropositivity was noted only in 1992. Blood bank staff were seronegative. HBV which has a similar portal

Blood transfusions are reported to impair the cell-mediated immune response. Because both T lymphocyte and macrophage function are important for wound repair, the authors investigated the effect of blood transfusions on anastomotic repair. Lewis rats underwent resection of both ileum and colon, followed by the construction of either an everted or an inverted end-to-end anastomosis. Immediately after operation, they received either 3 mL saline intravenously, or 3 mL heparinized blood from Lewis or Brown Norway donors. The animals were killed 3 or 7 days after operation, and anastomotic strength…

Children undergoing major craniofacial surgery (MCFS) often require transfusion in excess of one blood volume. Therefore they were the subject of a retrospective review which looked at the longitudinal trend of plasma potassium concentration [K+] during surgery. Ten of eleven children had a statistically significant increase in plasma potassium concentration during their intraoperative course and in five the potassium concentration exceeded 5.5 mmol.L-1. This was in contrast to the stable intraoperative plasma [K+] observed in a control group which did not receive blood transfusion. All MCFS c…

Circulatory behavior in chronic, severely anaemic patients on volume loading is not precisely known. Twenty young male subjets with hook-worm anaemia, (Hb 2 to 5 gm %), without any complications were transfused with 300 or 600 ml of whole blood at 3 6 or ml/mt. Haemodynamic study was done before and immediately after. Blood volume was low, intracardiac pressures normal or minimally abnormal, cardiac output raised and vascular resistances low. After transfusion, there was a small but significant rise in arterial and mixed venous oxygen content, oxygen transport, heart rate, pulmonary wedge and
