Multimodal sensor dataset for monitoring older adults post lower limb fractures in community settings.

Lower limb fractures (LLF) significantly impact older adults, leading to reduced mobility, prolonged recovery, and impaired independence. During recovery, older adults frequently face social isolation and functional decline, complicating rehabilitation and adversely affecting their physical and mental health. Multimodal sensor platforms that continuously collect data and analyze it using machine learning algorithms can remotely monitor this population and infer health outcomes. These platforms can also alert clinicians to individuals at risk of social isolation and functional decline. This pap

External Fixator Pin Placement in the Lower Extremity Without Fluoroscopy.

INTRODUCTION: External fixation is a critical method for temporizing musculoskeletal injuries. Limited guidance exists on how to achieve appropriate external fixator pin depth without image guidance. We sought to validate a standardized technique for placing external fixation pins in the lower extremity without fluoroscopy. METHODS: Adults undergoing external fixation of lower extremity injuries at a level I trauma center were prospectively enrolled over 12 months. A standardized technique was used for pin placement, and pins were placed by attendings or trainees at PGY2 to PGY6 levels. Compl

Operative Treatment and Outcomes of Pediatric Patients with an Extremity Bone Tumor: A Secondary Analysis of the PARITY Trial Data.

BACKGROUND: Osteosarcoma and Ewing sarcoma are the 2 most common primary bone sarcomas, occurring predominantly in pediatric patients, with the incidence of osteosarcoma correlating with periods of peak bone-growth velocity. Although survival outcomes have plateaued over the past several decades, ongoing treatment advances have improved function, decreased infection rates, and improved other clinical outcomes in patients with bone tumors. Recently, the Prophylactic Antibiotic Regimens in Tumor Surgery (PARITY) trial addressed the serious problem of surgical site infection (SSI) and the lack of

Costs and 30-day readmission after lower limb fractures from motorcycle crashes in Queensland, Australia: A linked data analysis.

BACKGROUND: Lower limb trauma is the most common injury sustained in motorcycle crashes. There are limited data describing this cohort in Australia and limited international data establishing costs due to lower limb trauma following motorcycle crashes. METHODS: This retrospective cohort study utilised administrative hospitalisation data from Queensland, Australia from 2011-2017. Eligible participants included those admitted with a principal diagnosis coded as lower extremity or pelvic fracture following a motorcycle crash (defined as the index admission). Multiply injured motorcyclists where

[Efficacy of treatment of lower-limb critical ischaemia by methods of indirect revascularization].

AIM: The study was aimed at comparing the efficacy of implantation of autologous bone marrow cells with that of revascularizing osteotrephination in treatment of lower limb chronic critical ischaemia in patients with a poor distal vascular bed. PATIENTS AND METHOD: We analysed the results of comprehensive examination and treatment of a total of 60 patients presenting with lower limb chronic critical ischaemia due to atherosclerotic lesions of the femoropopliteal-tibial segment. According to the technology of treatment, the patients were divided into two statistically homogenous groups of 30 p

Failure of tension band plating: a case series.

Growth modulation with tension band plates (TBP) has been shown to be a very useful method for the treatment of angular deformities in growing children. Recently, we have observed cases of failure where the epiphyseal screw was drawn through the physis into the metaphysis. This study describes a series of children who developed this complication. Patients who developed TBP failure after operative treatment of lower limb angular deformities were identified from the databases at four institutions over a 5-year period. The medical records were reviewed to record demographics, primary diagnoses, d

Bilateral Total Knee Arthroplasty in a 15 Year Old With Skeletal Dysplasia and Open Physes.

There are limited reports of total knee arthroplasty (TKA) in the pediatric population. The use of constrained, stemmed components is particularly unusual in the setting of open physes due to concerns with growth arrest and implant survivorship. The current authors describe a 15-year-old boy with open growth plates and an epiphyseal dysplasia-like condition affecting the knees bilaterally. He had no other significant medical or orthopedic conditions and his genetic workup was negative. He had severe knee pain and a bilateral 40° valgus with severe lateral compartment arthritis. Conservative tr

Orthopaedic trauma mythbusters: is limb salvage the preferred method of treatment for the mangled lower extremity?

Orthopaedic education is replete with unsubstantiated recommendations (myths) from predecessors in the field of orthopaedics. Even in the presence of sound evidence, some of these myths can be perpetuated through generations. One such recommendation is that if a mangled lower extremity can be saved, it should be saved. Recent technical and biologic advances allow the salvage of limbs that often required amputation in the past. Today's physicians must decide whether the physical, emotional, and financial costs of limb salvage can be justified by the expected functional outcome. Accumulated evid

Biological osteosynthesis as the treatment mode for multifragmental extra-articular fractures of lower limb long bones.

The concept of stable-rigid osteosynthesis developed by AO ASIF was subjected to basical changes recently, with regard to the treatment strategy of multifragmental extra-articular fractures. The principle of biological osteosynthesis was worked out, which, in contrast to AO original concept, implies the indirect (closed) reduction and "bridge-like" (elastic) fixation, without intervening on fracture area, and additionally, with maximally preserved vascularization of intermediate fragments. The study objective implied the consideration and analysis of the results of biological osteosynthesis, p

Hypertrophic osteoarthropathy masquerading as lower extremity cellulitis and response to bisphosphonates.

Hypertrophic osteoarthropathy (HOA) is characterized by clubbing of digits and, in more advanced stages, by periosteal new bone formation and synovial effusions. Secondary HOA typically occurs in the setting of intrathoracic malignancy, infections or lung metastases. We report a case with an atypical presentation of HOA. The prominent systemic signs, presentation limited to lower extremities only and an excellent response to bisphosphonates make this case unusual.

Effect of chemotherapy and surgery timing on mortality in upper and lower extremity osteosarcoma.

BACKGROUND: Surgery with neoadjuvant and adjuvant chemotherapy represents the standard of care for extremity osteosarcoma despite a lack of high-quality evidence for its use, and trial evidence that suggests upfront surgery may result in better outcomes. This study estimated the difference in overall survival for the standard of care (neoadjuvant first) vs upfront surgery first followed by adjuvant chemotherapy (surgery first). METHODS: Using Surveillance, Epidemiology, and End Results data, we identified patients aged 5-29 years diagnosed with a primary cancer of upper or lower extremity ost

Personalized tourniquet pressure versus uniform tourniquet pressure in trauma orthopedic surgery of extremities: a prospective randomized controlled study.

BACKGROUND: Tourniquets are widely used in limb fracture surgeries. Controversies still exist about the pressure inflated, including unified tourniquet inflation pressure (UTIP) and personalized tourniquet inflation pressure (PTIP). This study evaluated the hemostatic effect between UTIP and PTIP based on systolic blood pressure (SBP) in extremity fracture patients. MATERIALS AND METHODS: Patients with fresh extremity fractures requiring tourniquets during surgeries were prospectively enrolled and randomly assigned to the UTIP and PTIP groups. The inflation pressure was set to 250 mmHg for th

The muscle-bone relationship in X-linked hypophosphatemic rickets.

CONTEXT: We recently found that patients with X-linked hypophosphatemic rickets (XLH) have a muscle function deficit in the lower extremities. As muscle force and bone mass are usually closely related, we hypothesized that patients with XLH could also have a bone mass deficit in the lower extremities. OBJECTIVE: The study objective was to assess the muscle-bone relationship in the lower extremities of patients with XLH. SETTING: The study was carried out in the outpatients department of a pediatric orthopedic hospital. PATIENTS AND OTHER PARTICIPANTS: Thirty individuals with XLH (6 to 60 y;
