Self-stigma in women with borderline personality disorder and women with social phobia.

Little is known about how women with borderline personality disorder (BPD) and women with social phobia react to mental illness stigma. The goal of this study was to assess empirically self-stigma and its correlates in these groups. Self-stigma and related constructs were measured by self-report questionnaires among 60 women with BPD and 30 women with social phobia. Self-stigma was inversely related to self-esteem, self-efficacy, and quality of life and predicted low self-esteem after controlling for depression and shame-proneness. Stereotype awareness was not significantly correlated with sel…

Evidence of abnormal amygdala functioning in borderline personality disorder: a functional MRI study.

BACKGROUND: Intense and rapidly changing mood states are a major feature of borderline personality disorder (BPD); however, there have only been a few studies investigating affective processing in BPD, and in particular no neurofunctional correlates of abnormal emotional processing have been identified so far. METHODS: Six female BPD patients without additional major psychiatric disorder and six age-matched female control subjects underwent functional magnetic resonance imaging (fMRI) to measure regional cerebral hemodynamic changes following brain activity when viewing 12 standardized emotio…

Long-Term Psychodynamic Psychotherapy With a Patient With Dissociative Identity Disorder, Major Depressive Disorder, and Borderline Personality Disorder.

Dissociative identity disorder is a complex condition associated with severe trauma and disrupted attachment. This case report describes a young woman initially diagnosed with borderline personality disorder and major depressive disorder, later rediagnosed with comorbid dissociative identity disorder. This report details her treatment course with psychodynamic psychotherapy complemented by secondary strategies such as breathing and relaxation exercises. Prognostic implications are discussed, emphasizing how a diagnosis of dissociative identity disorder reshaped her treatment plan. The authors …

Is 'subthreshold' bipolar II disorder more difficult to differentiate from borderline personality disorder than formal bipolar II disorder?

Recent research indicates that borderline personality disorder (BPD) can be diagnostically differentiated from the bipolar disorders. However, no studies have attempted to differentiate participants with sub-threshold bipolar disorder or SubT BP (where hypomanic episodes last less than 4 days) from those with a BPD. In this study, participants were assigned a SubT BP, bipolar II disorder (BP II) or BPD diagnosis based on clinical assessment and DSM-IV criteria. Participants completed self-report measures and undertook a clinical interview which collected socio-demographic information, a mood h…

Attention-Deficit/Hyperactivity Disorder as a Risk Factor for Suicidality in Patients With Borderline Personality Disorder.

Borderline personality disorder (BPD) and attention-deficit/hyperactivity disorder (ADHD) are each associated with impulsivity, emotion dysregulation, and an increased prevalence of suicide attempts. It is unclear whether comorbid ADHD heightens suicide risk in individuals with BPD. This study examined suicide attempts among psychiatric outpatients diagnosed with either comorbid BPD and ADHD (n = 54) or BPD without ADHD (n = 330). Patients were assessed with several semistructured interviews. Contrary to our hypothesis, the BPD-only group reported significantly more lifetime suicide attempts, …

Associations between borderline personality disorder symptomatology and major depressive disorder: Insights from a general population study.

Understanding factors influencing the onset and course of major depressive disorder (MDD) is critical due to its frequently unfavorable prognosis. While Borderline Personality Disorder (BPD) seems to negatively affect the course of MDD, the role of subthreshold BPD symptoms remains unclear. This study investigated the impact of subthreshold BPD symptoms on the onset and persistence of MDD over a three-year follow-up in a general population sample of 4618 participants from the Netherlands Mental Health Survey and Incidence Study. MDD was assessed using the Composite International Diagnostic Int…

Social cognition impairment associated to a borderline personality disorder subtype linked to attention deficit hyperactivity disorder.

INTRODUCTION: Social cognition (SC) impairment in patients with BPD could be increased by comorbidity with attention deficit hyperactivity disorder (ADHD). Comorbidity in neurodevelopmental disorders and its influence on SC impairments could result in greater severity, worse response to treatment and worse evolution of the disorder. The aim of this work is to study SC deficits, through Theory of Mind (ToM) errors associated with BPD linked to ADHD. METHODS: 87 BPD patients with and without ADHD associated were assessed with the Movie for the Assessment of Social Cognition (MASC). Statistical

Complex posttraumatic stress disorder and borderline personality disorder: a truly complex relationship or a diagnostic artefact?

PURPOSE OF REVIEW: This review examines the relationship between borderline personality disorder (BPD) and complex posttraumatic stress disorder (CPTSD) in light of recent claims that the former might be replaced by the latter. RECENT FINDINGS: The stigma associated with BPD is not a convincing reason to suggest that CPTSD is more adequate than BPD, despite the fact that there is currently less stigma attached to it. BPD and CPTSD share some core symptoms, such as affective dysregulation and interpersonal difficulties. However, several features, including impulsivity, identity disturbance, an…

Prevalence and Clinical Impact of Attention Deficit Hyperactivity Disorder in Patients With Severe Borderline Personality Disorder.

OBJECTIVE: This study aims to estimate the prevalence and assess the clinical impact of ADHD in patients with severe borderline personality disorder (BPD) treated in a specialized unit. METHODS: Cross-sectional study of a sample of 145 patients diagnosed with BPD according to DSM-5 criteria, consecutively referred by outpatient psychiatrists to a Personality Disorders Unit. ADHD symptoms were assessed using the self-reported long version of the Conners Adult ADHD Rating Scale (CAARS-S:L). Groups were compared based on ADHD presence. Hypothesis testing was performed and variables with statisti…

A cluster analysis of attachment styles in patients with borderline personality disorder, bipolar disorder and ADHD.

BACKGROUND: Insecure adult attachment has been associated with psychiatric disorders characterized by emotional dysregulation, such as borderline personality disorder (BPD), bipolar disorder (BD) and attention deficit/hyperactivity disorder (ADHD). However, little is known about the differences in attachment patterns between these diagnostic groups. The aim of this study was to identify clusters of adult attachment style in a cross-diagnostic group of patients with BDP and/or BD and/or ADHD and explore the characteristics of these clusters based on temperament profile, childhood trauma and psy…

Impulse control under emotion processing: an fMRI investigation in borderline personality disorder compared to non-patients and cluster-C personality disorder patients.

Impulsivity is a characteristic syndromal and neurobehavioral feature of borderline personality disorder (BPD). Research suggests an important interaction between high negative emotions and low behavioral inhibition in BPD. However, knowledge about the generalizability across stimulus categories and diagnosis specificity is limited. We investigated neural correlates of hypothesized impaired response inhibition of BPD patients to negative, positive and erotic stimuli, by comparing them to non-patients and cluster-C personality disorder patients. During fMRI scanning, 53 BPD patients, 34 non-pat…

[Psychological disorders in the oral healthcare practice. Borderline and other personality disorders].

Personality disorders are prevalent and increase the risk of a range of serious dental problems. In the oral healthcare practice, however, this diagnosis is often not known and the disorder is not always immediately recognizable. Trial and error may bring it to the fore. It may cause tension in the therapeutic relationship and the development of trust. The largest problems in the therapeutic relationship are encountered in patients with severe (borderline) personality disorder, often attributable to a history of (early) childhood emotional neglect and traumatization. It is important to recogni

Longitudinal description and prediction of physical inactivity among patients with borderline personality disorder and personality-disordered comparison subjects.

BACKGROUND: The physical and psychological benefits of physical activity are well-known, and physical activity has been proven to be a helpful adjunct to psychotherapeutic treatment for many symptomatic disorders, including mood and anxiety disorders. The current study explores physical inactivity levels in patients with borderline personality disorder (BPD). The first aim of this study is to describe the 12-year course of physical inactivity in patients with BPD. The second aim is to examine predictors of physical inactivity, including adversity experiences, comorbid symptomatic (formerly axi…

Compatibility of Linehan's biosocial theory and the DSM-5 Alternative Model of Personality Disorders for borderline personality disorder.

Borderline personality disorder (BPD) is characterized by affective, interpersonal, and identity instability, as well as marked impulsivity. There is evidence that BPD may be best operationalized dimensionally using models such as the Alternative Model for Personality Disorders (AMPD) described in Section III of the Diagnostic and Statistical Manual for Mental Disorders (DSM). Moreover, biosocial theory is a well-known etiological theory of BPD emphasizing emotion dysregulation, inherited impulsivity, and development within invalidating contexts as key etiological mechanisms. Given that curren…

The 24-year course of major depression in patients with borderline personality disorder and personality-disordered comparison subjects.

BACKGROUND: This study had two main objectives. The first was to detail the prevalence of major depressive disorder over 24 years of follow-up for both patients with borderline personality disorder (BPD) and comparison subjects with other personality disorders (OPD). The second was to determine time-to-remission, recurrence, and new onset of major depression among these two groups of patients. METHODS: The SCID-I was administered to 290 borderline inpatients and 72 personality-disordered comparison subjects during their index admission. It was also re-administered at 12 contiguous two-year fo…

Longitudinal associations between plasma oxytocin levels and borderline personality disorder symptoms in adolescents.

BACKGROUND: Borderline personality disorder (BPD) is a complex disorder associated with severe interpersonal instability and common relationship problems. Oxytocin has emerged as a candidate neuropeptide, with cross-sectional studies showing reduced levels in individuals with BPD. Longitudinal data - particularly in adolescents - remain scarce, leaving unresolved whether changes in oxytocin reflect treatment effects, developmental processes, or reciprocal influences with symptom trajectories. The present study examines the longitudinal associations of oxytocin and the number of fulfilled crite…

Wise Moments: A four-session Acceptance and Commitment Therapy for Borderline Personality Disorder.

BackgroundBorderline Personality Disorder (BPD) is a complex mental health condition. Wise Moments is a four-session Acceptance and Commitment Therapy (ACT)-themed intervention for the preliminary treatment of BPD. It focuses on the ACT concept of the 'self-as-context' (the 'observing self'), encouraging the person diagnosed with BPD to notice and observe their internal mental states without judgement. This study assessed the acceptability and efficacy of an ultra-brief, individual intervention based on ACT principles, designed for clients experiencing symptoms of BPD.MethodSelf-administered p…

Long-term stability of perceived childhood trauma in individuals with borderline personality disorder.

Background: The Childhood Trauma Questionnaire (CTQ) is a widely used tool for the retrospective assessment of childhood trauma (CT). However, evidence on its long-term stability remains limited, especially in diagnostically complex populations such as individuals with borderline personality disorder (BPD). Given the disorder's characteristic emotional instability and altered self-perception, assessing the stability of CTQ scale scores in this group is particularly important. Methods: In a longitudinal design, CTQ and the Borderline Symptom List-23 (BSL-23) scores were collected at two timepoin…
