Postoperative cortical venous infarction in tumours firmly adherent to the cortex.

It is sometimes difficult to separate extra-axial tumours from the cortical veins in case of tumours attaching tightly to the cortex and the cortical veins. We present two patients having a postoperative cortical venous infarction. A 59 year old female had convexity meningioma above the motor cortex where abnormal cortical anastomotic veins developed. Transient hemiparesis occurred after total removal of the tumour because of venous infarction and cyst formation resulting from sacrifice of these veins which were tightly adherent to the tumour surface. A 15 year old boy with immature teratoma o

Flow quantification in the superior sagittal sinus using magnetic resonance.

To date, the intracerebral veins and venous sinuses have not been amenable to noninvasive study. We describe a magnetic resonance (MR) technique using "bolus tracking" for rapid imaging and measurement of cerebral venous flow. We specifically applied the technique to the superior sagittal sinus, but it can be used for evaluation of other cerebral venous structures. In 10 healthy subjects and 21 patients referred for MR brain studies, mean flow was 420 ml/min. There was a significant inverse correlation between blood flow and age. There were dynamic changes in cerebral blood flow (CBF) during h

Predictive acoustical processing in human cortical layers.

In our dynamic environments, predictive processing is vital for auditory perception and its associated behaviors. Predictive coding formalizes inferential processes by implementing them as information exchange across cortical layers and areas. With laminar-specific blood oxygenation level-dependent imaging we measured responses to a cascading oddball paradigm, to ground predictive auditory processes on the mesoscopic human cortical architecture. Using a modeling approach, we derive putative changes in neural dynamics while accounting for draining effects. We show that the violations of predict

Cognitive Impairment Without Dementia: A Treatable Case of Dural Arteriovenous Fistula.

Dural arteriovenous fistula (DAVF) can present with a wide range of symptoms depending on its location and the degree of arteriovenous shunting. Rarely, they may manifest as cognitive impairment due to chronic venous dysfunction. This is the case of an elderly man evaluated for subacute onset of memory loss without dementia. Magnetic resonance imaging (MRI) of the brain revealed a subtle subarachnoid vascularity in the right occipital lobe, without cerebral edema or parenchymal abnormality. Digital subtraction angiography (DSA) confirmed a low-flow Borden type III DAVF, draining via an occipit

Application of 3D Slicer Combined With Simple Coordinate Method in Operation of Cerebral Arteriovenous Malformations in Functional Areas.

The authors report a case of intracranial arteriovenous malformation in functional areas, initially presenting with symptomatic epilepsy was surgically excised by the Neurosurgery Department of our hospital. The patient's head computed tomography, magnetic resonance imaging, and digital subtraction angiography examination suggested intracranial arteriovenous malformations in the left frontal functional area. A preoperative 3D-reconstruction technique was used to reconstruct the malformed vascular mass, supplying arteries, draining veins, and precise surgical resection was performed. Postoperat

Modification of a biventricular assist device to facilitate preservative infusion and organ recovery in a nonheart-beating donor.

A 46-year-old patient supported by a biventricular assist device (BiVAD) was transferred to our institution for evaluation for heart transplant. The patient was found to have a large intracranial hemorrhage with profound deterioration of neurologic status. The poor prognosis prompted the decision to withdraw care and pursue organ donation. Because the patient did not meet brain death criteria, nonheart-beating donor organ donation was pursued. After the termination of care, the BiVAD was modified: the left side to provide organ preservative solution and the right side to allow drainage. Eight

Retrograde intraventricular hemorrhage caused by a traumatic sacral pseudomeningocele in the presence of spina bifida occulta. Case report.

The authors present the case of a large, posttraumatic sacral pseudomeningocele in the presence of spina bifida occulta. A pseudomeningocele in the sacral region is associated with trauma and with Marfan syndrome, but only one occurrence has been reported in association with spinal dysraphism. Trauma resulted in bleeding into the pseudomeningocele and retrograde passage of blood and fat into the ventricles. An oculomotor nerve palsy subsequently developed in the patient. The authors suspected a subarachnoid hemorrhage caused by a posterior communicating artery aneurysm, although this hypothesi

Chronic granulomatous pleuritis caused by nocardia: PCR based diagnosis by nocardial 16S rDNA in pathological specimens.

Nocardiosis is an uncommon infection caused by the aerobic actinomycete nocardia. Identification of the pathogen is essential for the definitive diagnosis and for an effective treatment. This report describes a case of chronic granulomatous pleuritis caused by nocardia. A 59 year old Japanese man had a history of repeated pyothorax. Right pleural decortication and thoracic drainage were performed. Microbiological examinations of the drained fluid failed to identify a pathogen. Pathological examinations revealed Gram positive filamentous and branching bacilli in the granulomatous lesion of the

[Diagnosis and differential diagnosis of intracranial vascular malformations using proton spin tomography].

Eleven patients are described, nine of whom had angiomas and two/had aneurysms in the brain. Magnetic resonance tomography produced characteristic findings in all these patients. Angiomas typically show irregular margins with little or no surrounding oedema. Thrombosis is common and can be recognised by a reduction of T1 and prolongation of T2. Zones with high blood flow show reduced signal strength. In typical cases the lesion resembles a cauliflower. Enlarged draining veins could be recognised in five of the nine angiomas. Aneurysms, on the other hand, have a smooth configuration and it may

Cranioplasty after decompressive craniectomy: An institutional audit and analysis of factors related to complications.

BACKGROUND: Although a relatively simple procedure, cranioplasties have been associated with high complication rates. Keeping this in perspective, we aimed to determine the factors associated with immediate and long-term complications of cranioplasties at our institution. METHODS: A retrospective review of patient records was carried out for patients having undergone reconstructive cranioplasties at our institution during the last 10 years (2001-2010). All case notes, records, and investigations were reviewed and the data were recorded in a predesigned questionnaire. Complications were record

Brain relaxation and cerebrospinal fluid pressure during craniotomy for resection of supratentorial mass lesions.

Neurosurgery can be complicated by the clinical situation commonly referred to as "tight brain," in which the brain presses against the inner table of the skull or protrudes through the craniotomy site. We report here a retrospective study of 32 patients who had undergone elective craniotomy for resection of supratentorial mass lesions. We determined the relationship between lumbar cerebrospinal fluid pressure (CSFP) and brain relaxation and whether brain relaxation varies with anesthetic technique. Patients had received one of four anesthetic techniques: 1 MAC isoflurane (ISO), 1 MAC desflura

Safety of Indocyanine Green Microdosing for Clinical Imaging of CSF Ventricular Dynamics and Extracranial Outflow.

BACKGROUND AND PURPOSE: Intravenous administration of indocyanine green (ICG) has been approved in brain surgeries for decades, yet concerns about neurotoxicity prevent its direct administration into the cerebrospinal fluid (CSF). Armed with prior animal studies demonstrating the feasibility of using ICG microdosing into the CSF, we sought to evaluate its nonclinical safety profile and obtain surrogate measures in adults prior to its use in human neonates. METHODS: Evaluation of ICG toxicity was conducted in mixed primary CNS cell cultures and in an extended safety study of juvenile rat pups

Cerebro-spinal fluid glucose and lactate concentrations changes in response to therapies in patIents with primary brain injury: the START-TRIP study.

INTRODUCTION: Altered levels of cerebrospinal fluid (CSF) glucose and lactate concentrations are associated with poor outcomes in acute brain injury patients. However, no data on changes in such metabolites consequently to therapeutic interventions are available. The aim of the study was to assess CSF glucose-to-lactate ratio (CGLR) changes related to therapies aimed at reducing intracranial pressure (ICP). METHODS: A multicentric prospective cohort study was conducted in 12 intensive care units (ICUs) from September 2017 to March 2022. Adult (> 18 years) patients admitted after an acute brai

Strengths and weaknesses of frontal versus occipital ventriculoperitoneal shunt placement: A systematic review.

Excessive accumulation of cerebrospinal fluid within the brain ventricles is called hydrocephalus, which results in increased intracranial pressure preventing brain growth or causing damage to intracranial structures due to raised intracranial pressure. One of the most common treatment options for this pathology includes the placement of a ventriculoperitoneal shunt to drain the excess fluid. The location of catheterization is traditionally considered as an important factor affecting shunt survival. In this study, we aimed to systematically review all available documents to determine the advan

Fistula components of brain arteriovenous malformations: angioarchitecture analysis and embolization prior to gamma-knife surgery.

BACKGROUND: Gamma-knife surgery (GKS) is ineffective for high-flow arteriovenous fistula (AVF). The purpose of this study was to present the angioarchitecture of the AVF of brain arteriovenous malformation (BAVM) and report our experience of endovascular embolization of AVF component prior to GKS. METHODS: In the past 10 years, a total of 523 BAVMs had been treated primarily by GKS. Among these, 10 patients with AVF components were identified and referred for embolization prior to GKS. Those patients underwent GKS within 4 weeks after embolization. We analyzed retrospectively the angioarchite

Genesis of a dural arteriovenous malformation in a rat model.

A rat model was developed to determine the role of sinus thrombosis and elevated sinus pressures in the pathogenesis of dural arteriovenous malformations (AVMs). Five protocols were tested to compare various sinus pressures and thrombosis of a sinus: 1) Control I, sham operation (five animals); 2) Control II, occlusion of the right common carotid artery, the right external jugular vein, and the vein draining the left transverse sinus, as well as thrombosis of the sagittal sinus (10 animals); 3) arteriovenous fistula (AVF) I, anastomosis of the right common carotid artery to the external jugula

Hemodynamic modulation of transnidal pressure during transvenous arteriovenous malformation embolization: an experimental porcine model.

BACKGROUND: Optimization of transvenous embolization of brain arteriovenous malformations (AVMs) depends on effective modulation of the transnidal pressure gradient. Pharmacologic flow arrest with adenosine has been proposed as an adjunct, but its hemodynamic equivalence to purely mechanical strategies remains unclear. METHODS: End-to-end carotid-jugular fistula AVM models were created in 10 pigs. Hemodynamic measurements were obtained with pressure-sensing fractional flow reserve guidewires placed in the feeding artery and draining vein. Measurements included feeding artery mean pressure (MA
