Evaluation of risk factors in developmental dysplasia of the hip in children from multiple pregnancies: results of hip ultrasonography using Graf's method.

BACKGROUND: The risk factors for developmental dysplasia of the hip (DDH) in infants from unifetal pregnancies are commonly known. Many of these factors also apply to multiple pregnancies. The aim of this paper was to evaluate the influence of selected, widely recognized DDH risk factors in unifetal pregnancy on the development of hip joints in infants from multiple pregnancies. The following risk factors were examined: family history of DDH, breech presentation, duration of pregnancy, birth weight and sex. MATERIAL AND METHODS: The study included 200 newborns (400 hip joints) from multiple p…

Is routine cesarean section necessary for vertex-breech and vertex-transverse twin gestations?

Ninety-three vertex-breech and 42 vertex-transverse twin gestations were managed at Yale-New Haven Medical Center during a 5-year period. Antepartum diagnosis of twin gestation occurred in 93% of the cases, and diagnosis was made before delivery of the second twin in 97% of the cases. Seventy-two (78%) of the vertex-breech and 22 (53%) of the vertex-transverse twins were delivered vaginally. Breech extraction was used for delivery of 76 second twins. Below a birth weight of 1,500 gm, there were six neonatal deaths, four cases of documented intraventricular hemorrhage, and a 67% occurrence of d

Term breech delivery. Early and late complications.

Three hundred and sixty-six consecutive singleton breech presentations were analyzed for clinical factors associated with fetal complications related to vaginal delivery, and for the impact of different cesarean section rates on the incidence of fetal complications. The condition at birth of the vaginally delivered (243 cases) infants was found to correlate with maternal pre-pregnancy weight, and size of the fetal head. During the period of study the cesarean section rate changed from approximately 20 per cent of term breech presentations at the beginning to almost 50 per cent at its end. This…

[Selective caesarean section for breach presentation - bearings on morbidity of survivors neonatal period (author's transl)].

The authors have continued their own analysis of deliveries from breech presentation by studying childhood morbidity of 1,094 single infants who had survived the neonatal period, between 1966 and 1975. Particular attention was given to the impact of selective caesarean section on primiparae, beyond completion of the 36th week of pregnancy. The two reported periods between 1966 and 1971 as well as between 1972 and 1975 were compared for that purpose. -The frequency of caesarean sections went up from 11.6 to 52.2 per cent. Severe parturitional trauma, such as fractures or plexal paralysis, were …

Compound presentation following external version.

Two cases of compound presentation following external version are presented. The frequent combination of the feet with the vertex following this procedure and its association with a higher fetal mortality, interference rate, and obstructed labour than the hand and vertex combination are highlighted. The aetiology, problems in diagnosis, and management are discussed. Measured to prevent and deal with this complication are suggested.

Unsuspected hepatic injury in the neonate--diagnosis by ultrasonography.

Three cases of severe neonatal hepatic injury were investigated with ultrasonography. The injury is often associated with antenatal factors (fetal hepatic enlargement, maternal trauma), perinatal factors (breech presentation, pre- or post-maturity, difficult delivery), or postnatal factors (resuscitation).

[Breech extraction of the second twin with or without version by internal maneuvers].

A retrospective study was done to appraise the recent practice of breech extraction of the second twin (with or without internal version) in our unity. From January 1st, 1988 to April 30, 1991 23 patients with a twin gestation were delivered in our unity (0.5% of all deliveries). Nineteen patients (83%) were delivered vaginally. Each procedure was done by a resident under control of a senior obstetrician and in the presence of two midwives, a pediatrician, and an anesthesiologist. Ten patients having had a breech extraction (GE group) were studied, 13 patients were excluded for not having had

Singleton breach presentation planned for vaginal delivery.

A retrospective study of 156 consecutive cases of singleton breach presentation planned for vaginal delivery is presented. The incidence of caesarean section was not increased when compared with that for all Hospital and Clinic confinements. The perinatal mortality and prematurity rates were high. The incidence of cord prolapse was increased especially with the footling breech. The need for an experienced Obstetrician and Anaesthetist to conduct all deliveries was demonstrated. Factors influencing the assessment of feto-pelvic proportion and the occurrence of trauma and asphyxia in the managem

Collective Forgetting of American Vaginal Breech Delivery.

When asked why nearly all doctors refer their breech cases to surgery, despite non-surgical breech birth being permitted throughout the United States, an obstetrician will likely cite the Term Breech Trial (TBT). This study, conducted in 2000, decisively concluded that planned cesarean delivery is safer than vaginal breech delivery. However, a review of the literature suggests that the decline of vaginal breech deliveries was a long time in the making. From the 1950s, once the perceived risks of breech births were accepted as a fact, numerous studies advocated more liberal use of cesarean deli

Platelet storage pool disorder: multidisciplinary planning in pregnancy.

A 32-year old primigravida woman presented for antenatal care giving a history that her mother had platelet storage pool disorder (PSPD). The patient was subsequently diagnosed with PSPD during her pregnancy and had a caesarean delivery for breech presentation at 39 weeks. In this paper, we discuss the basic science, inheritance pattern, symptoms and management of this condition, alongside the antenatal and intrapartum and postnatal management specific to it, highlighting the need for a multidisciplinary approach to care. PSPD refers to a group of rare conditions involving defects in platelet …

A retrospective cohort study of the impact of In Time obstetric simulation training on management of vaginal breech deliveries.

BACKGROUND: Following the Term Breech Trial, vaginal breech deliveries are rarely undertaken in Australia. Some women choose to have a breech delivery following counselling, while others will present in labour with an undiagnosed breech. Clinicians need to be skilled in vaginal breech delivery despite this being a rare clinical situation. Simulation training provides a means by which uncommon clinical situations can be practised. AIM: This study aims to determine if the introduction of a simulation-based training course is associated with an improvement in the management of vaginal breech del

[Case report of 39-Year-Old Gravida III, Para III with Inverted Uterus after Breech Birth].

An inverted uterus is defined as the eversion of the uterus into the vagina. It is a very rare event but has to be treated as an emergency. A woman with an inverted uterus is in serious danger of developing atony with high blood loss in a short time. This can lead to hypovolemic shock and circulatory failure. Therapy includes urgent uterine repositioning under narcosis and curettage after manual abruption of the placenta. Afterwards, uterotonics such as prostaglandins should be administered. A disorder of the coagulation system is very common due to blood loss. Therefore, substitution of plasm

Training program for the management of two obstetric emergencies within a French perinatal care network.

OBJECTIVES: To evaluate the effectiveness of an interdisciplinary team training program based on simulated scenarios and focusing on two obstetrical emergency situations: shoulder dystocia and complicated breech vaginal delivery (CBVD). These situations are rare, so there are few opportunities for real-life training, yet their competent and efficient management is crucial to minimizing the risks to mother and child. STUDY DESIGN: The target population for training comprised the 450 professionals working in the French regional perinatal care network ELENA. An expert committee defined the topic

Obstetric brachial plexus palsy associated with breech delivery. A different pattern of injury.

Most obstetric brachial plexus palsies are due to rupture of the upper roots in babies whose delivery was complicated by shoulder dystocia. If treated by early exploration and grafting, they have a favourable prognosis. We reviewed 36 babies who had had an obstetric brachial plexus palsy after a breech delivery and found that they had a different pattern of injury; 81% had avulsion of the upper roots. This injury cannot be treated satisfactorily by exploration and microsurgical grafting and carries a considerably worse prognosis for shoulder function.
