Opportunities and challenges in reducing personal inhalation exposure to air pollution among electronic waste recovery workers in Ghana.

BACKGROUND: Informal sector electronic waste (e-waste) recovery produces toxic emissions resulting from burning e-waste to recover valuable metals. OBJECTIVES: To identify high-risk worker groups by measuring relative levels of personal inhalation exposure to particulate matter (PM) of fine (≤2.5 µm) and coarse (2.5-10 µm) fractions (PM2.5 and PM2.5-10, respectively) across work activities among e-waste workers, and to assess how wind conditions modify levels of PM by activity and site location. METHODS: At the Agbogbloshie e-waste site, 170 partial-shift PM samples and time-activity data we

Acute lung injury and multiple organ dysfunction syndrome secondary to intra-abdominal hypertension and abdominal decompression in extensively burned patients.

BACKGROUND: Secondary abdominal compartment syndrome (ACS) is a lethal complication after resuscitation from burn shock, even after abdominal decompression (AD) is performed. This study investigated increased susceptibility to multiple organ dysfunction syndrome (MODS) in extensively burned patients with ACS. METHODS: Patients admitted to our burn unit between 2002 and 2005 with burns affecting 40% or more of the total body surface area without severe inhalation injury were analyzed. Hemodynamic parameters, blood gas analysis, and intrabladder pressure as intra-abdominal pressure were recorde

Comparative analysis of bedside and operating room tracheostomies in critically ill patients with burns.

The objective of this study was to demonstrate that bedside burn intensive care unit tracheostomy is a safe and cost-effective procedure and has advantages over operating room tracheostomy. The charts of all patients who underwent tracheostomies in the burn unit between January 1990 and September 1993 were reviewed retrospectively. All tracheostomies were performed by residents in their second to fourth postgraduate years. The identical operating room technique was used for all bedside procedures including complete instrument tray, electrocautery, and adequate lighting. Standard tracheostomies

Does trauma center designation impact management, complications, and outcomes in burn patients? A National Trauma Data Bank analysis.

BACKGROUND: Burn injuries, particularly those involving large total body surface area (TBSA), present significant challenges due to fluid shifts, increased metabolic demands, and heightened susceptibility to complications. Trauma center designation, ranging from Level 1 (highest acuity) to Level 3, reflects differences in resources, expertise, and care protocols, which may influence patient outcomes. This study evaluates the impact of trauma center designation on transfusion practices, complications, and mortality among burn patients. METHODS: The Trauma Quality Improvement Program (TQIP) dat

Inpatient Complications and Outcomes for Burn Patients Admitted with Opioid-Positive Drug Screens.

Opioid use disorder complicates the management of burn injuries, particularly in achieving adequate pain control. While prior single-institution studies have evaluated associations between opioid use and burn outcomes, multi-institutional analyses of inpatient complications remain limited. Using the American Burn Association Noncommercial Burn Research Dataset, this study compares inpatient outcomes among patients with prehospital opioid-positive screening versus those with negative screening. Among 32 052 patients included, 50.5% screened positive for opioids at the time of presentation. Thes

Hypercapnic respiratory failure and partial upper airway obstruction during high frequency oscillatory ventilation in an adult burn patient.

PURPOSE: To present a case of severe hypercapnic respiratory failure in an adult burn patient and to describe our clinical problem solving approach during support with an unconventional mode of mechanical ventilation. CLINICAL FEATURES: A 19-yr-old male with smoke inhalation and flame burns to 50% total body surface area was admitted to the Ross Tilley Burn Centre. High frequency oscillatory ventilation (HFOV) was initiated on day three for treatment of severe hypoxemia. By day four, the patient met consensus criteria for acute respiratory distress syndrome. On day nine, alveolar ventilation

Respiratory care of the burn patient.

The vast majority of respiratory disorders in thermally injured patients arise from associated inhalation injuries. The major forms of these injuries are carbon monoxide poisoning, injury to the upper airway, and pulmonary parenchymal damage. One hundred per cent oxygen, initiated at the scene of the accident, is the single most effective treatment of carbon monoxide toxicity, which must be assessed by carboxyhemoglobin determinations. Respiratory tract damage is identified by fiberoptic bronchoscopy and xenon ventilation-perfusion scintigrams. The compromised airway is protected by tracheal i

Protective Effect of Nebulized Heparin in the Animal Models of Smoke Inhalation Injury: A Meta-analysis and Systematic Review of Experimental Studies.

The pathophysiological mechanism of abnormal coagulation can result from smoke inhalation injury (SII). Heparin nebulization is a common treatment for lung disorders. This study aimed to use meta-analysis in animal models to examine the effectiveness of atomized heparin on SII. For our online searches, we used the Cochrane Central Register of Controlled Trials, PubMed, Web of Science, Chinese National Knowledge Infrastructure, Chinese BioMedical Literature Database, and Wanfang Database up to January 2022. Data for SII were retrieved and compared to control animals. The studies' findings were

Early detection of extravascular lung water in an inhalation injury animal model.

A rabbit inhalation injury model using a dual tracer radioactive isotope technique (Rowland et al., 1986), has been utilized with small- and medium-sized molecular weight tracers. There is oedema formation at 2 h after smoke inhalation or fluid resuscitation and especially with the combination of inhalation injury and fluid resuscitation [corrected]. This oedema appears to have decreased by 24 h post-injury. There does not appear to be any difference between the small- and medium-sized tracers in the transit times of tracer in the lung vascular compartment. The increases in extravascular water

The use of NexoBrid™ enzymatic debridement and coagulation abnormalities.

OBJECTIVES: Current evidence on how the use of bromelain-based enzymatic debridement techniques (NexoBrid™) affect patient coagulation is limited. A single patient case report [1] suggests that a patient with 15% total body surface area (TBSA) burn developed decreased coagulation activity following debridement with NexoBrid™ enzymatic debridement (ED). Regional Burns Centres in the United Kingdom may be reluctant to use ED, particularly in larger burns, citing concerns regarding coagulation. At our centre we have routinely used ED on deep partial thickness burns since 2017 including on patient

Epinephrine Nebulization Delays Need for Life-Saving Intervention After Smoke Inhalation in Ovine Model.

INTRODUCTION: Smoke inhalation injury, alone or combined with burns and other traumas, remains a serious threat for military members as well as the civilian population. It causes acute respiratory distress syndrome requiring life-saving interventions (LSI), including various respiratory support, for example, oxygen supplementation, intubation, and noninvasive or invasive mechanical ventilation. While potentially scarce in austere environments and/or during mass casualties, LSI require trained personnel, equipment, and resources. In this study, we tested the hypothesis that nebulized epinephrin

Management strategies for perioperative anaemia in the severely burn-injured Jehovah's Witness patients who decline a blood transfusion: A systematic review with illustrative case reports.

BACKGROUND: The management of severe burn-injured Jehovah's Witness patients who decline a blood transfusion poses unique challenges. The literature is scant for guiding perioperative anaemia management in these patients. We present a systematic review of this patient group, along with illustrative, consecutive case reports of our experience. METHODS: A systematic review was performed on Embase, MEDLINE and PubMed databases on articles discussing the treatment of burn-injured Jehovah's Witness patients. Articles were excluded if discussing isolated inhalation injury, or if blood transfusions

[Clinical study on hematocrit used as a predictor for evaluation of resuscitation effect in the early shock stage after burn].

OBJECTIVE: To explore the clinical significance of hematocrit used as a predictor for diagnosis and evaluation of resuscitation effect in the early shock stage after burn. METHODS: Clinical data of 131 severely burned patients admitted to our burn unit from January 2000 to December 2011 were retrospectively analyzed. The burn patients were divided into group A (n = 80) and group B (n = 51) based on the hematocrit level at post burn hour (PBH) 24. The hematocrit levels in group A were less than or equal to 0.50, which in group B were higher than 0.50. There were no statistically significant di

Hyperbaric oxygen therapy: current trends and applications.

Hyperbaric medicine is the fascinating use of barometric pressure for delivering increased oxygen dissolved in plasma to body tissues. Hyperbaric oxygen therapy (HOT) or hyperbaric oxygen (HBO) involves intermittent inhalation of 100% oxygen under a pressure exceeding that of the atmosphere, that is greater than 1 atmosphere absolute (ATA). Therapy is given in special therapeutic chambers which were earlier used primarily to treat illnesses of deep sea divers. There is recently a renewed interest in this field all over the world. Acute traumatic wounds, crush injuries, burns, gas gangrene and

Acute respiratory distress syndrome following cutaneous exposure to Lysol: a case report.

Lysol (mixed cresols) is a brand of popular detergent commonly used to disinfect toilets and floors in Taiwan. We report a patient with acute respiratory failure immediately following chemical burns caused by skin contact with Lysol solution. On admission, chest radiography showed bilateral diffuse pulmonary infiltrates and an arterial blood gas analysis disclosed hypoxemia refractory to a high concentration of oxygen by inhalation. Under the impression of acute respiratory distress syndrome, our patient was admitted to the intensive care unit for respiratory care. Poor clinical improvement wa

Interpretable machine learning model predicts the early gastrointestinal dysfunction risk in severely burned patients: a multicenter-based study.

BACKGROUND: Gastrointestinal (GI) dysfunction is a life-threatening complication following severe burn injury, significantly increasing risks of multi-organ failure and mortality. This study aimed to develop and validate the first machine learning (ML)-based clinical prediction model for GI dysfunction after severe burns by leveraging explainable artificial intelligence (AI) techniques to support early clinical intervention. METHODS: In this retrospective multicenter study, 570 patients with severe burns were enrolled: 469 from Hospital A [randomly split into training (n = 328) and internal v

Long-Term Laryngotracheal Complications After Inhalation Injury: A Scoping Review.

Long-term laryngotracheal complications (LTLC) after inhalation injury (II) are an under-recognized condition in patients with burns. The purpose of this study was to systematically review all available evidence on LTLC after II and identify gaps in knowledge to guide the direction of future research. A scoping review was performed to synthesize all available evidence on LTLC after II, as guided by the question, "What are the LTLC after II, in patients with or without a history of translaryngeal intubation and/or tracheostomy?". MEDLINE, Web of Science, Ovid Embase, Cochrane Library, and Googl

Impact of Wildfires on Cardiovascular Health.

Wildfire smoke (WFS) is a mixture of respirable particulate matter, environmental gases, and other hazardous pollutants that originate from the unplanned burning of arid vegetation during wildfires. The increasing size and frequency of recent wildfires has escalated public and occupational health concerns regarding WFS inhalation, by either individuals living nearby and downstream an active fire or wildland firefighters and other workers that face unavoidable exposure because of their profession. In this review, we first synthesize current evidence from environmental, controlled, and intervent
