Ultrasonography and magnetic resonance imaging changes in patients with polymyalgia rheumatica treated by tocilizumab.

BACKGROUND: This study assessed inflammatory changes using ultrasound (US) and magnetic resonance imaging (MRI) in patients taking tocilizumab for polymyalgia rheumatica (PMR). METHODS: Eighteen patients were included in the prospective open-label TENOR study and received three tocilizumab infusions, without corticosteroids. B-mode and power Doppler US and MRI (T1 and T2-short time inversion recuperation weighted sequences) of the hips and shoulders were performed at weeks 0, 2, and 12. Subacromial, trochanteric, and iliopsoas bursitis and intraarticular glenohumeral and coxofemoral effusions…

Magnetic Resonance Imaging of T2-weighted Hypointense Nodular Lesions of the Foot and Ankle: A Pictorial Review of Rheumatoid Nodules, Tophaceous Gout, and Tenosynovial Giant Cell Tumor.

ABSTRACT: Nodular soft tissue abnormalities are common in the foot and ankle and often demonstrate overlapping magnetic resonance imaging fe … peated mechanical stress, and usually present as multiple lesions with accompanying intermetatarsal bursitis. In contrast, tophaceous gout is rarely associated with interdigital bursitis, but more commonly it shows diffuse fatty muscle infiltration. Tenosynovial giant cell tumors are typically limited to a single joint space or tendon sheath and usually no …

The management of greater trochanteric pain syndrome: A systematic literature review.

Greater trochanteric pain syndrome (GTPS) is a common cause of lateral hip pain. Most cases respond to conservative treatments with a few refractory cases requiring surgical intervention. For many years, this condition was believed to be caused by trochanteric bursitis, with treatments targeting the bursitis. More recently gluteal tendinopathy/tears have been proposed as potential causes. Treatments are consequently developing to target these proposed pathologies. At present there is no defined treatment protocol for GTPS. The purpose of this systematic literature review is to evaluate the cur…

Diagnostic and therapeutic use of sonography-guided iliopsoas peritendinous injections.

OBJECTIVE: Our objective was to review our experience performing sonography-guided iliopsoas bursal/peritendinous injections as a diagnostic and therapeutic tool in the workup and treatment of patients with hip pain. CONCLUSION: Sonography-guided iliopsoas bursal/peritendinous injections are useful in determining the cause of hip pain. They can provide relief to most patients with iliopsoas tendinosis/bursitis after hip replacement. The results of injection alone are not as successful in cases of idiopathic iliopsoas tendinosis/bursitis, but the technique can help determine which patients may…

Endoscopic resection of the inflamed bicipitoradial bursa extended around the radial neck.

The bicipitoradial bursa lies at the insertion of the biceps tendon on the radial tuberosity. It is an unusual site for chronic bursitis and most often, results from repetitive mechanical trauma or overuse. Other causes include tuberculosis, immunological complications of the rheumatological disease, for example, psoriatic arthropathy, rheumatoid arthritis and synovial chondromatosis. Unlike ganglion cyst arising from the elbow joint, resection of the bursa through the elbow arthroscopy is not possible as the bursa is not communicated with the joint. We reported a patient with rheumatoid arthr…

Soft-tissue rheumatism: diagnosis and treatment.

Soft tissue rheumatism is one of the most common and most misunderstood categories of disorders facing the primary care physician. Among the more common types are subacromial bursitis, epicondylitis, trochanteric bursitis, anserine bursitis, and fibromyalgia. The keys to the diagnosis of soft-tissue rheumatism are the history and, more importantly, the physical examination. Extensive laboratory testing and radiographs are not as helpful in evaluating patients with these complaints. Treatment consists of nonsteroidal anti-inflammatory drugs (NSAIDs) and nonnarcotic analgesics. Especially in pat…

Clinical anatomy of the knee.

The clinical anatomy of several pain syndromes of the knee is herein discussed. These include the iliotibial tract syndrome, the anserine syndrome, bursitis of the medial collateral ligament, Baker's cyst, popliteus tendon tenosynovitis and bursitis of the deep infrapatellar bursa. These syndromes are reviewed in terms of the structures involved and their role in knee physiology. All of the discussed structures can be identified in their normal state and more so when they are affected by disease. The wealth of information gained by cross examination of the medial, lateral, posterior and anteri…

[Tendinitis of the hip region].

Tendinitis and bursitis are less common around the hip than around the shoulder. Nevertheless, they must be recognized to avoid unnecessary and costly diagnostic errors. Their various clinical forms are studied in detail. Tendino-bursitis of the gluteus medius muscle is the most frequent in its subacute form, but it is rare in its acute, pseudo-gouty form. Calcification of the reflected tendon of the rectus femoris muscle often closely resembles arthritis of the hip. Synovial cysts of the psoas bursa and rupture of the gluteus medius tendon are rare but must be known. Local injections of corti…

Arthroscopic Excision of Pigmented Villonodular Synovitis of the Trochanteric Bursa.

Trochanteric bursitis is a common disorder affecting middle-aged adults and usually presents with lateral-based hip pain and swelling. It usually responds to conservative measures, including adductor stretching, abductor strengthening, and select injections of corticosteroid or platelet-rich plasma. For refractory cases, excision, open or arthroscopic, is usually recommended. We observed a 55-year-old woman who had lateral hip pain and longstanding swelling consistent with refractory trochanteric bursitis. Her persistent symptoms, coupled with atypical findings on imaging, prompted an arthrosc…

Unilateral painful, swollen and erythematosus knee. Case report.

Musculoskeletal ultrasound (US) is gaining an increasing role in the assessment of a variegate set of joint and periarticular soft tissues abnormalities in rheumatology. In addition, US represents a useful technique in guiding local procedures such as aspiration of fluid collections and injections within the joints and periarticular musculoskeletal structures. We recently performed US in a patient with unilateral painful, swollen and erythematosus knee. Pre-patellar bursitis was demonstrated, without any signs of intra-articular abnormalities. US-guided aspiration of local effusion was perform…

Structure and function, injury, pathology, and treatment of the medial collateral ligament of the knee.

The medial collateral ligament (MCL) is the most commonly injured ligament of the knee. There is a spectrum of injury severity, and injuries may be acute or chronic. The MCL is also frequently injured in conjunction with other knee structures. Clinical evaluation of the knee is important to assess the degree of surgical acuity, but magnetic resonance imaging can provide details about the injury that may not be obvious clinically. In addition to injury, MCL bursitis can occur and may be treated with needle aspiration and corticosteroid injection. This review article covers the anatomy and biome…

Tendon Sheath, Bursae, and Clinically Relevant Peritendinous Structures of the Hand and Wrist.

Imaging is essential for evaluating tenosynovitis, bursitis, and pulley lesions. Both ultrasound (US) and MR imaging detect fluid accumulation and synovial thickening, key for diagnosing tenosynovitis and bursitis. They also reveal A1 pulley thickening in trigger fingers and A2-A4 pulley strain or tears in closed flexor pulley injuries (climber's fingers). US enables dynamic assessments in real-time and detects hypervascularity, while MR imaging provides detailed insights into the extent of inflammation in the soft tissues and bone through fluid-sensitive sequences and contrast enhancement.
