Calcium-sensing receptor in the brain.

Following its cloning through an homology-based method from a rat striatal library, the calcium-sensing receptor (CaR) has been localized in the brains of adult and developing rats by immunocytochemistry and in situ hybridization with CaR-specific antibodies and cDNA probes, respectively. The receptor resides in numerous regions of the brain at widely varying levels. The highest levels are present within the subfornical organ (SFO) and the olfactory bulbs. Substantial levels of expression are also evident within the hippocampus, striatum, cingulate cortex, cerebellum, ependymal zones of the ce…

Long on QT and low on calcium.

Acquired lengthening of the QT interval due to hypocalcaemia is a rare cause of arrhythmia in childhood. Early recognition, rapid institution of appropriate cardiac monitoring, and replacement therapy are essential. An endocrinal work-up may be necessary to exclude primary disorders of calcium metabolism. We report four cases documenting the varied clinical spectrum in which hypocalcaemic-induced prolongation of the QT interval and arrhythmia can occur in childhood.

Calcium and bone health in infants.

Osteopenia, rickets, and fractures from nutrient deficiencies can occur during infancy, particularly in preterm infants. Bone mass accretion during the first year of life is equal to or greater than that achieved at any other stage of life, including adolescence. Optimizing calcium and bone status during infancy can have immediate benefits in maintaining calcium homeostasis and preventing disturbances in bone mineralization and can provide long-term benefits by helping infants to later reach their maximum genetic potential for peak bone mass, a prerequisite for the prevention of osteoporosis a…

Development and application of caged calcium.

Several caged calciums have been synthesized since 1986, and three are commercially available: DM-nitrophen, NP-EGTA, and nitr-5. Each of these caged compounds has uniquely useful properties, making the choice of which cage to use dependent on the specific experiment (i.e., the cell type and divalent cation requirements of the experiments within purview). Significantly, methods have been developed for all three cages that permit their quantitative use inside many cell types, including those with some of the most demanding of requirements for experiments with caged calcium, namely, in relating …

Pitfalls in interpretation: calcium that's not bone.

This is a 65-yr-old woman with dermatomyositis and extensive calcinosis. On physical examination subcutaneous nodules were widely distributed on her chest and extremities. Her current CPK is 163 IU/L (normal range 0-175); however, with a recent flare of her symptoms, the CPK peaked at 411 IU/L. Her current medications are Imuran 25 mg/d, Prednisone 4 mg/d, and conjugated equine estrogen 0.625 mg/d.

Calcium in gravitropism. A re-examination.

For more than a decade it has been assumed that there is a strong relationship between Ca2+ and gravitropism. There is evidence to suggest that the movement of Ca2+ in the wall might regulate extension growth and that free intracellular Ca2+ might mediate signalling in statocytes. However, it is unlikely that changes in either wall Ca2+ or the concentration of free intracellular calcium, [Ca2+]i, act independently of each other. This results in ambiguity in both experimentation and interpretation of experimental information. It is also uncertain that the observed redistributions of wall Ca2+ a…

The biology and medicine of calcium signalling.

Calcium is a second messenger responsible for regulating a wide range of cellular processes. It is normally presented as brief spikes even in non-excitable cells. The necessity of limiting the period of calcium stimulation to brief bursts may depend upon the fact that prolonged elevation of calcium can be toxic. It can act on endonucleases in the nucleus to trigger programmed cell death. It will be argued that non-lethal effects of elevated calcium can lead to a variety of pathological conditions including hypertension, atherosclerosis, transformation, malignant hyperthermia and possible neura…

Management of disorders of calcium homoeostasis.

Severe hypercalcaemia may be life-threatening and requires prompt management. Whatever the cause, the aim of therapy should obviously be to eradicate the source of hypercalcaemic factors. After rehydration, which is an essential first step in the management strategy, and after evaluation of the prevailing pathogenetic mechanism, the acute treatment will be aimed at increasing urinary Ca excretion and inhibiting bone resorption. Among the various pharmacological agents, bisphosphonates appear to be the drugs of first choice, because of their efficacy and their virtual absence of side-effects. T

Calcium and acid rebound: a reappraisal.

We review acid rebound, the seemingly paradoxical increase in acid secretion resulting from administration of an antacid. Primarily a laboratory observation, the demonstration of the phenomenon was a major contributing factor to the swift, and possibly unjustified, fall from grace of calcium carbonate in the therapy of peptic ulcer disease despite years of apparently successful use. Calcium, as carbonate or other salts, causes an increase in gastric acid secretion owing, at least in part, to direct ionic stimulation. Another possible mode of action involves antral alkalinization with subsequen…

Disorders of calcium and magnesium metabolism.

Although relatively common, aberrations in divalent cation homeostasis may be overlooked in Emergency Department patients. The intracellular concentration of ionized calcium is the major regulator of cellular function. Patients may present with signs and symptoms of deranged calcium homeostasis that range from the mild and nonspecific to the truly life threatening. Critically ill patients may develop profound, life-threatening hypocalcemia either as a result of their underlying illness or as a complication of resuscitation. Patients with hypercalcemia may present with symptoms that are so vagu…

Prevention of osteoporosis: the calcium controversy.

The continuing scientific controversy regarding the use of calcium supplements for the prevention of osteoporosis during menopause presents a challenge to the nurse practitioner. There is little doubt that estrogen prevents bone loss following menopause, but there is a growing doubt among many researchers whether calcium supplementation beyond 800 mg/per day produces any benefit at all in preventing postmenopausal osteoporosis. Yet, the medical consensus in this country continues to recommend 1500 mg of calcium per day for menopausal women. The practitioner's clinical questions relate to the e…

Disorders of calcium and bone metabolism.

Calcium levels in blood are maintained within narrow limits by parathyroid hormone, acting on kidney, bone and gut. Our understanding of the mechanisms of these actions has increased greatly in recent years, for example by identification of new hormones (eg 1,25 dihydroxy vitamin D) and by the study of isolated bone cells. Though most clinical disorders of calcium metabolism are now readily diagnosed and many are effectively treated, some serious problems still remain. Foremost amongst these are the early diagnosis of osteoporosis and the treatment of osteoporosis after fractures have occurred…

Calcium and the aging nervous system.

Many aspects of calcium homeostasis change with aging. Numerous calcium compartments complicate studies of altered calcium regulation. However, age-related decreases in calcium permeation across membranes and mobilization from organelles may be a common fundamental change. Deficits in ion movements appear to lead to altered coupling of calcium-dependent biochemical and neurophysiological processes and may lead to pathological and behavioral changes. The calcium-associated changes during aging probably do not occur with equal intensity in all cell types or in different parts of the same cell. T…
