Diagnosis and treatment of obscure GI bleeding at double balloon endoscopy.

BACKGROUND: Double balloon endoscopy (DBE) and videocapsule endoscopy (VCE) have been useful in managing obscure GI bleeding (OGIB). OBJECTIVE: This study evaluated the usefulness of DBE for diagnosis, treatment, and prognosis of OGIB and compared diagnostic yield between DBE and VCE in Japan. METHODS: Detection rates of abnormalities and diagnostic yields between VCE and DBE were compared in 74 patients at 5 centers. Of 244 patients who underwent DBE at Nagoya University Hospital, 130 (53%) with OGIB were enrolled for investigation of therapeutic procedures. SETTING: Seven Japanese medical

Unmasking a High-Grade Malignant Neoplasm: A Rare Case of Small Bowel Melanoma.

Malignant melanoma of the small bowel is a rare form of gastrointestinal malignancy. This case report describes a 70-year-old woman who presented with symptomatic iron deficiency anemia, progressive abdominal pain, weight loss, and other nonspecific symptoms, prompting an extensive diagnostic workup. Initial endoscopic evaluations were unremarkable, leading to further investigations, including capsule endoscopy and radiologic studies, which identified a jejunal mass. Histopathological and immunohistochemical analyses confirmed the diagnosis of malignant melanoma, and molecular testing identifi

Improvement of Surgical View and Working Space at the Lower Pole by Three-Dimensional Exoscope-Assisted Coblation Tonsillectomy: A Case Series.

Advantages of hot devices for tonsillectomy, represented by coblation, have been highlighted in recent years. During hot technique tonsillectomy it is important to identify and coagulate the vessels of the tonsillar capsule, especially at the lower pole of the tonsil. Hot technique tonsillectomy under microscope or endoscope has therefore been recommended to achieve accurate identification of the microstructure of the surgical field. We have applied ORBEYE, a three-dimensional surgical exoscope system, to coblation tonsillectomy. Advantages of using ORBEYE include high definition and high magn

Olmesartan-associated sprue-like enteropathy diagnosed by capsule endoscopy and double balloon endoscopy.

Sprue-like enteropathy associated with olmesartan is characterized by villous atrophy in the duodenum. We report the case of an 81-year-old woman diagnosed with olmesartan-associated sprue-like enteropathy with no villous atrophy in the duodenum. The patient had been taking olmesartan for 10 years and complained of diarrhea and weight loss. Despite undergoing general treatment for 2 months, her symptoms showed no improvement. Gastrointestinal endoscopy and pathological findings showed no villous atrophy in the duodenum. However, villous atrophy was observed in the small intestine by capsule en

Splenocystojejunostomy for the treatment of a symptomatic splenic pseudocyst.

Pancreatic pseudocyst is a widely recognised local complication following acute pancreatitis. Typically occurring more than four weeks after acute pancreatitis, a pseudocyst is a mature, encapsulated collection found within the peripancreatic tissues manifesting as abdominal pain, structural compression, gastroparesis, sepsis and organ dysfunction. Therapeutic interventions include endoscopic transpapillary or transmural drainage, percutaneous catheter drainage and open surgery. We present our management of idiopathic chronic pancreatitis complicated by a pancreatic pseudocyst extending to the

[Utility of capsule endoscopy in pediatric gastroenterology].

Wireless endoscopy is a new noninvasive diagnostic method that is able to visualize small bowel lesions. The instrument is small and carries a battery and microcamera that takes two photographs per second. It is indicated in cases of bleeding of unknown origin and for the diagnosis of inflammatory bowel disease, among other disorders. To date, it has mainly been used in adults. We believe that this instrument could play an important role in the pediatric age group since it is noninvasive and can be used to diagnose small bowel lesions, thus avoiding unnecessary diagnostic tests. We report the

[A case report of Crohn's disease in a child with trisomy 8].

The patient is a 12-year-old male who has experienced recurrent perianal abscesses for over 10 years, along with recurrent oral ulcers and deformities in the joints of hands and feet. Gastrointestinal endoscopy and capsule endoscopy revealed multiple ulcers in the digestive tract. Combined with his histopathological examinations, the patient was diagnosed with Crohn's disease. Whole exome sequencing and peripheral blood karyotype analysis indicated a karyotype of 47,XY,+8. The patient was treated with a "step-up" strategy. His clinical symptoms were under control, with significant improvement

[Ganglioneuroma of the Small Intestine Mimicking Crohn's Disease].

INTRODUCTION: Intestinal ganglioneuromatosis is a rare benign neoplastic disease. It is usually described in children, in association with multiple endocrine neoplasia type 2b and neurofibromatosis type 1. Ganglioneuromatosis is rare in adults, where it usually occurs sporadically and in isolation. CASE REPORT: We present a patient with intestinal ganglioneuromatosis in whom the initial clinical and radiographic findings simulated Crohn's disease. A small bowel resection was performed due to endoscopic capsule occlusion. The immunohistochemical study of the resected segment suggested the diag

Recurrent Gastrointestinal Pseudo-obstruction Because of Well-Differentiated Duodenal Neuroendocrine Tumor.

A 56-year-old man presented with recurrent gastrointestinal obstruction. Computed tomography showed fluid-filled, distended stomach, small intestine, and large intestine. Extensive workup including esophagogastroduodenoscopy, colonoscopy, magnetic resonance enterography, push enteroscopy, and video capsule enteroscopy showed no mechanical obstruction. Endoscopic ultrasound-guided biopsy of peripancreatic nodes detected on 18F-fluorodeoxyglucose positron emission tomography revealed a duodenal neuroendocrine tumor. The lesion showed intense uptake on gallium-68 DOTATOC positron emission tomogra

Role of enteroscopy in the diagnosis of whipple's disease.

Whipple's disease is a rare chronic systemic infection caused by Tropheryma whipplei. The widespread infection by this rod is responsible for the protean clinical manifestations of the disease, although its classical form is notable for the prevalence of abdominal symptoms such as chronic diarrhea and abdominal pain. Whitish-yellow patches, suggestive of lymphangiectasia, are typically observed in the duodenum during upper endoscopy. The diagnosis of this condition is supported by the identification in duodenal biopsies of Periodic acid-Schiff staining within lamina propria macrophages. Nevert

[Endoscopic biopsy of the small intestine in children].

As from 1983, endoscopy with biopsy in general anesthesia was used instead of the capsule suction method in children with suspected coeliac disease in a county hospital. A study was carried out to evaluate mean endoscopy time, duration of general anesthesia, complications and quality of biopsies. An ordinary Olympus GIF-Q endoscope (diameter 11 mm) was used in 17 patients, eight boys and nine girls. Mean age was 5.9 years (range 11 months-14 years). Mean endoscopy time was 5.3 min. and of duration of general anesthesia 13.6 min. No serious complications such as perforation or bleeding were rec

Dieulafoy disease with gastric MALT lymphoma: A case report.

RATIONALE: Dieulafoy lesion (DL), a rare cause of gastrointestinal bleeding, is easily covered by blood scab formation on the mucous membrane for its small size, which makes it difficult to be identified under endoscope. In clinical practice, it is also very easy to miss gastric mucosa-associated lymphoid tissue (MALT) lymphoma that exhibits atypical early manifestations under gastroendoscope and is difficult to be diagnosed by routine superficial biopsy. Most patients only experience nonspecific dyspepsia symptoms. PATIENT CONCERNS: A 68-year-old man suffering from repeated melena for 6 year

Combined use of capsule endoscopy and double-balloon enteroscopy in patients with obscure gastrointestinal bleeding.

BACKGROUND: Most of the sources of bleeding in patients with obscure gastrointestinal bleeding are located in the small bowel. Capsule endoscopy (CE) and double-balloon enteroscopy (DBE) are innovative modalities to examine the small intestine. This study evaluated the combined use of CE and DBE in patients with obscure gastrointestinal bleeding. METHODS: From October 2005 to November 2006, ten patients with obscure gastrointestinal bleeding who underwent CE followed by DBE were included in this study. The insertion route for DBE was determined according to the site of obscure gastrointestina

The small bowel diseases detected by capsule endoscopy in patients with chronic abdominal pain: A retrospective study.

Chronic abdominal pain (CAP) remains a particular challenge because of its complicated causes, especially when the disorders involve the small bowel, where it is quite difficult to intubate the flexible endoscopes. This study was to investigate the small bowel diseases detected by capsule endoscopy (CE) in CAP patients to evaluate the role of CE on CAP, and analyzed the relationship among the clinical characteristics of CAP patients and the positive rates of CE findings to search for the indications of CE for CAP patients. This retrospective study included 341 patients with CAP defined as recur

Endoscopic stapes surgery for otosclerosis: clinical, radiological, and audiological outcomes in a retrospective series of 40 patients.

BACKGROUND: Otosclerosis is a primary osteodystrophy of the otic capsule, characterized by abnormal bone remodeling, leading to stapes fixation and progressive conductive or mixed hearing loss. Although microscopic stapes surgery remains the historical standard, endoscopic ear surgery has gained increasing interest due to improved visualization of middle ear anatomy and its minimally invasive transcanal approach. This study aimed to evaluate the clinical, radiological, and audiological outcomes of exclusive endoscopic stapes surgery in a retrospective cohort of patients with otosclerosis. MET

The findings of capsule endoscopy in patients with common variable immunodeficiency syndrome.

BACKGROUND/AIMS: Common variable immunodeficiency syndrome (CVIS) is a primary immunodeficiency disorder characterized by reduced serum immunoglobulins and heterogeneous clinical features. Parasitic and bacterial infections are very common complications of the syndrome. Capsule endoscopy (CE) represents a new and highly innovative method of demonstrating the small intestinal diseases. We evaluated the practical usefulness and diagnostic yield of CE in three patients with CVIS. METHODOLOGY: Between January 2003 and September 2004 CE was performed in 31 patients for different indications includ

[Use of intraoperative enteroscopy as a diagnostic method in patients with obscure gastrointestinal bleeding. Initial experience].

BACKGROUND: Bleeding of dark origin is a rare entity and it is reserved for cases in which exploration of the small intestine has already been performed (endoscopic / radiological) without finding the cause. Currently, the gold standard is the videocapsule which has been described as a safe, useful and effective technique for the diagnosis and treatment of diseases of the small intestine. OBJECTIVE: The objective was to describe demographic variables, indications, findings, explored segment, treatment and time of the surgical procedure of the intraoperative enteroscopy in 15 patients with a d
