In man, high doses of the "group 1" inhalation anesthetics (diethylether, cyclopropane, and fluroxene) produce relatively minor depression of ventricular function, although it is possible to depress the heart if the dose is great enough. The "group 2" drugs (halothane, methoxyflurane, etc.) produce dose-related depression in cardiac function, but reasonable caridac outputs and blood pressure can be maintained at light anethetic levels. Much the same can also be said for the intravenous barbiturates and other hypnotics. If ventilation is supported and hypovolemia avoided, large doses of the nar

Vein of Galen malformation (VoGM) is a rare congenital vascular anomaly. It represents a well-recognized extracardiac cause of high-output cardiac failure in the neonatal period. We report a case of a 3.5-month-old boy who presented for routine vaccination and was incidentally found to have macrocephaly (head circumference 44.5 cm, >99th percentile) with early sun-setting eyes. He was otherwise well-appearing with a normal neurological examination. Bedside cranial ultrasound revealed bilateral lateral and third ventricular dilation with a cystic mass compressing the aqueduct, and brain MRI con

When added to endurance training, dynamic strength training leads to significantly greater improvements in peripheral muscle strength and power output in patients with cardiovascular disease, which may be relevant to enhance the patient's prognosis. As a result, dynamic strength training is recommended in the rehabilitative treatment of many different cardiovascular diseases. However, what strength training intensity should be selected remains under intense debate. Evidence is nonetheless emerging that high-intensity strength training (≥70% of one-repetition maximum) is more effective to incre

We report the first Australian use of the Impella LP 5.0, a minimally invasive, high-output left ventricular assist device. The device was inserted intra-operatively for cardiac failure following coronary artery grafting. Although it facilitated a temporary post-operative recovery, the patient ultimately succumbed due to left ventricular failure more than 30 days after the original procedure. This device has several advantages over existing forms of left ventricular support, and has the potential for widespread use in Australian centres.

New Doppler ultrasound technology has caused a change in the approach to the diagnosis of congenital heart disease. Pulsed Doppler ultrasound continues to be of great value for measuring cardiac output ventricular performance and for calculating pulmonary to systemic flow ratios. Continuous wave Doppler ultrasound by virtue of its ability to detect high velocity accurately remains the premier technique for determining pressure drop across valves or across shunts. Pulsed Doppler ultrasound remains a valuable test for defining many lesions including patent ductus arteriosus.

Buprenorphin, 0,3 mg i.v., was used in 20 patients in the age of 51 to 75 years for analgesia after surgery for aortic aneurysm. Haemodynamic parameters of peripheral and pulmonary circulation, cardiac output, as well as blood gases were determined during routine postoperative controlled ventilation. The main results were an initial vasodilation and a decrease in heart rate, both of which reduce myocardial oxygen consumption. This effect is beneficial in the presence of sufficient volume substitution and optimal oxygenation in high-risk patients.

A 26-year-old woman developed severe postpartum cardiomyopathy in early puerperium. Ligation of the inferior vena cava prevented recurrent pulmonary embolism but did not affect her hemodynamic condition, which deteriorated constantly. During continuous hemodynamic monitoring only vigorous volume replacement with an increase of the left ventricular filling pressure to 32 mm Hg improved cardiac output significantly. The presence of a high titer of antiactin antibodies 9 months after the delivery supports the theory that the peripartum cardiomyopathy was of autoimmune etiology.

Congestive heart failure in neonatal thyrotoxicosis is attributed to sympathetic overstimulation of the myocardium with resulting high-output cardiac failure. An additional case of neonatal thyrotoxicosis with congestive heart failure is discussed; three possible causes (thyrotoxicosis, maternal propranolol therapy, and ventricular septal defect) were present. Along with the usual procedures, the echocardiogram is of value in separating these factors. In addition, we discuss the potential dangers to the newborn of a mother receiving long-term propranolol hydrochloride therapy during pregnancy.

Severe hyperthyroidism can cause cardiac complications, such as severe rhythm disturbances, heart failure and angina. Gestational trophoblastic disease (GTD) is a rare complication of pregnancy, ranging from benign hydatidiform mole to malignant form. Clinical hyperthyroidism may occur in GTD, as human chorionic gonadotropin (hCG) secreted by molar tissue is structurally similar to thyroid-stimulating hormone. Cardiothyreosis in this context is exceptional. We report the case of a nulligravida 42-year-old woman without thyroid or cardiac history who presented to the emergency department for dy

When the heart is subjected to an increased workload, mechanical stretch together with neurohumoral stimuli activate the "fetal gene program" and induce cardiac hypertrophy to optimize output. Due to a lack of effective methods/models to quantify and modulate cardiac mechanical properties, the connection between these properties and the development of cardiac hypertrophy remains largely unexplored. Here, we utilized an atomic force microscope (AFM) to directly measure the elastic modulus of the hypertrophic myocardium induced by pressure overload. Additionally, we investigated the effects of e

AIM: Levels of plasma markers of myocardial fibrosis (galectin-3), stretch (B-type natriuretic peptide (BNP)) and injury (high-sensitivity troponin T (hs-TnT)) are affected by haemodialysis, residual renal function (RRF) and cardiac pathology. We aimed to determine the association of RRF, urine output and haemodialysis itself on cardiac biomarkers in haemodialysis patients.
METHODS: Adult haemodialysis patients underwent venesection pre- and post-haemodialysis then echocardiography and inter-dialytic urine collection to calculate RRF (mL/min per 1.73m2 ) and urine output (mL/day). Galectin-3,

Kidney disease patients have a high prevalence of cardiovascular morbidity and mortality. It can be challenging to adequately assess their cardiovascular status based on physical examination alone. Cardiac ultrasound has proven to be a powerful tool to accomplish this objective and is increasingly being adopted by noncardiologists to augment their skills and expedite clinical decision-making. With the advent of inexpensive and portable ultrasound equipment, simplified protocols, and focused training, it is becoming easier to master basic cardiac ultrasound techniques. Hemodynamic changes have been studied in 19 patients (30 observations) with acute respiratory failure of various genesis during transition from conventional controlled lung ventilation, performed using PO-6 and Phase-3C devices, to intermittent high-frequency jet ventilation ("Spiron 601"). Impedance plethysmography and rheopulmonography have been used. It has been established that intermittent high-frequency jet ventilation during 1 hour can have both positive and negative effect on systemic and pulmonary hemodynamics in patients with lung damage. It has been demonstrated that the type of rea

BACKGROUND: In a cross-sectional study we examined whether the haemodynamic response to upright posture could be divided into different functional phenotypes, and whether the observed phenotypes were associated with known determinants of cardiovascular risk.
METHODS: Volunteers (n = 470) without medication with cardiovascular effects were examined using radial pulse wave analysis, whole-body impedance cardiography, and heart rate variability analysis. Based on the passive head-up tilt induced changes in systemic vascular resistance and cardiac output, the principal determinants of blood press

In atrial and ventricular tachyarrhythmias, reduced time for ventricular filling and loss of atrial contribution lead to a significant reduction in cardiac output, resulting in cardiogenic shock. This may also occur during catheter ablation in 11% of overall procedures and is associated with increased mortality. Managing cardiogenic shock and (supra) ventricular arrhythmias is particularly challenging. Inotropic support may exacerbate tachyarrhythmias or accelerate heart rate; antiarrhythmic drugs often come with negative inotropic effects, and electrical reconversions may risk worsening circu

BACKGROUND: Little is known about the autonomic control of the native sinus node (NSN) after heart transplantation because its electrical activity is difficult to detect with skin electrodes.
METHODS AND RESULTS: The intracardiac electrogram of the NSN and the donor heart electrocardiogram were measured simultaneously. A total of 351 recordings of 107 heart transplant patients (age 53.6+/-12.3 years) were obtained up to 1 year after transplantation. In a subgroup of 41 patients, consecutive recordings were made. Eleven recordings of heart failure patients (age 56.2+/-6.5 years) awaiting cardi

Poudel, Sangeeta, Sandesh Gautam, Purushottam Adhikari, and Ken Zafren. Physiological effects of sildenafil versus placebo at high altitude: a systematic review. High Alt Med Biol. 25:16-25, 2024. Introduction: High altitude pulmonary edema (HAPE), a life-threatening condition that affects individuals ascending to high altitude, requires the development of pulmonary hypertension. Sildenafil can be used to prevent and treat HAPE, presumably by decreasing pulmonary artery pressure (PaP). We compared the physiological effects of sildenafil versus placebo at high altitude (above 2,500 m), includin
