[Simultaneous digestive and cardiological studies in asymptomatic chagasic patients].

Prognosis in patients with Chagas disease is related to systemic involvement. 43 asymptomatic chagasic subjects, 28 males and 15 females aged 28 to 85 years, were subjected to electrocardiograms and upper and lower gastrointestinal radiological studies. 23 patients (53%) had evidence of involvement of at least one organ. Multiple involvement was demonstrated in 5 individuals (12%): one patient had megaesophagus, megacolon and abnormal EKG; 2 had megaesophagus and abnormal EKG; 1 had megaesophagus and dolicocolon and 1 had dolicocolon and abnormal EKG. Dilated esophagus was found in 6 patients

[Diagnostic value of the endomyocardial biopsy in the cardiologic therapeutic clinic].

Literature and authors' own data on the diagnostic value of endomyocardial biopsy in cases with clinical diagnosis of dilatation cardiomyopathy are analysed. It is shown that due to this biopsy the diagnosis of more than 20 heart diseases becomes possible. Myocarditis, the diagnosis of which is particularly difficult, is most frequently masked under dilatation cardiomyopathy. Ultrastructural studies are of great importance allowing to diagnose a number of heart diseases including alcoholic cardiomyopathy and viral myocardial lesions. The authors advocate wider use of endomyocardial biopsy whic

Drug related admissions to a cardiology department; frequency and avoidability.

Three hundred and sixty-six consecutive patients admitted to a department of cardiology were evaluated for drug events as a cause of admission. The drug events considered were adverse drug reactions (ADR) and dose-related therapeutic failures (DTF). 'Definite' or 'probable' drug events accounted for 15 admissions (4.1%, 95% confidence limits 2.3-6.7%), of which eleven were ADR and four were DTF. With the inclusion of six 'possible' drug events, the rate of drug-related hospitalizations (DRH) was 5.7%. DRHs were characterized by a preponderance of acute admissions and elderly patients. Hypokala…

[The status of echocardiography within the scope of routine cardiologic diagnosis].

In particular since the introduction of the two-dimensional approach, echocardiography as a non-invasive and easily repeatable bedside-technique without side-effects plays a central role within the diagnostic tools of cardiology. The following applications of cardiac ultrasound are commonly accepted and sufficiently validated: Assessment of the size of cardiac chambers including qualitative and quantitative evaluation of left ventricular function, detection of pericardial effusion, diagnosis of intra- and extracardiac tumors and thrombi, analysis of complex congenital diseases. Without the use…

Cardiovascular malformations in lecanosomatopagus conjoined twins: a cardiologic curiosity.

Female conjoined twins were delivered after 42 weeks' gestation, but they died within a few minutes of birth. They were dicephalus, dibrachius and dipus conjoined twins with two separate spines and fusion of the trunk and the pelvis. The pericardial sac was common, and the heart was a single structure. The atrial complex was a common chamber with an attempt at division into two parts by a circular ridge of tissue; the ventricular complex was formed by three chambers which were all communicating between each other in the superior margin of their muscular interventricular septum.

[Cardiologic evaluation in a family with Emery-Dreifuss muscular dystrophy].

Emery-Dreifuss muscular dystrophy is an X-linked recessive condition characterized by mild muscular weakness predominantly in a humero-peroneal distribution with variable facial involvement. Onset is in childhood with slow progression of weakness. The disease is often associated with cardiac involvement, mainly with bradyarrhythmias which might be responsible for sudden death. The most striking finding derived from the literature is the high incidence of sudden death; in the 7 large families described, out of the 79 reported patients 32 died suddenly at a young age (between 25 and 56 years). [The prompt squatting in the ambulance of pediatric cardiology (author's transl)].

The result of prompt squatting is an increase of the venous return and of the arterial resistance. Consequences are increasing of the end-diastolic and systolic chamber pressure, stroke volume, average arterial blood pressure as well as decreasing of the frequency of the pulse controlled by baro-receptors. The regulatory mechanism both change the intensity of heart murmurs of various cardiopathies and of functional heart murmurs in children. Only the systolic murmur of muscular subvalvular aortic stenosis softens. The experiment is most valuable for the recognition of mild insufficiency of sem

[Experience with conducting transcutaneous kidney biopsy in a cardiology clinic].

The analysis of 450 closed puncture biopsies of the kidneys permits to conclude that the employment of this method in a cardiological hospital provides rather valuable information on the state of the renal tissue in cases of various diseases entailing the arterial hypertension syndrome. The incidations for kidney biopsy include arterial hypertension, developing against the background of the nephrotic syndrome of varying genesis, clinically distinct glomerulonephritis (to precise its morphological variant), various forms of renovascular hypertension, and in some cases -- hypertensive form of py

Quality of life in heart failure. The heart of the matter. A scientific statement of the Heart Failure Association and the European Association of Preventive Cardiology of the European Society of Cardiology.

For most patients with chronic, progressive illnesses, maintaining good quality of life (QoL), with preserved functional capacity, is just as crucial as prolonging survival. Patients with heart failure (HF) experience much worse QoL and effort intolerance than both the general population and people with other chronic conditions, since they present a range of physical and psychological symptoms, including shortness of breath, chest discomfort, fatigue, fluid congestion, trouble with sleeping, and depression. These symptoms reduce patients' capacity for daily social and physical activity. Usual

Cardiological rehabilitation, prehabilitation, and cardiovascular prevention in adults with congenital heart defects: tasks and services of the German Pension Insurance-part 2: cardiological rehabilitation.

Congenital heart defects (CHD) represent the most common inborn organ anomaly, with more than a million newborns affected annually. Advances in diagnostics and treatment have led to significantly improved survival rates, resulting in a growing population of an estimated 50 million adults with congenital heart defects (ACHD) worldwide. As these individuals age, they often face a high burden of morbidity and complex long-term health challenges that require specialized, lifelong care. In this context, cardiological rehabilitation (CR) becomes increasingly important, not only to reduce morbidity b

CONCLUSION: In a broad international cardiology community, the 'historical approach' to HFrEF therapies remains the preferred sequencing approach. However, accelerated introduction and uptitration are also major treatment goals. Strategy trials in treatment guidance are needed to further change p …

Diagnosis and Management of Rare Cardiomyopathies in Adult and Paediatric Patients. A Position Paper of the Italian Society of Cardiology (SIC) and Italian Society of Paediatric Cardiology (SICP).

Cardiomyopathies (CMPs) are myocardial diseases in which the heart muscle is structurally and functionally abnormal in the absence of coronary artery disease, hypertension, valvular disease and congenital heart disease sufficient to cause the observed myocardial abnormality. Thought for a long time to be rare diseases, it is now clear that most of the CMPs can be easily observed in clinical practice. However, there is a group of specific heart muscle diseases that are rare in nature whose clinical/echocardiographic phenotypes resemble those of the four classical morphological subgroups of hype

Appropriate use criteria for optical coherence tomography guidance in percutaneous coronary interventions : Recommendations of the working group of interventional cardiology of the Netherlands Society of Cardiology.

INTRODUCTION: Optical coherence tomography (OCT) enables detailed imaging of the coronary wall, lumen and intracoronary implanted devices. Responding to the lack of specific appropriate use criteria (AUC) for this technique, we conducted a literature review and a procedure for appropriate use criteria. METHODS: Twenty-one of all 184 members of the Dutch Working Group on Interventional Cardiology agreed to evaluate 49 pre-specified cases. During a meeting, factual indications were established whereupon members individually rated indications on a 9-point scale, with the opportunity to substanti…
