Local cartilage trauma as a pathogenic factor in autoimmunity (one hypothesis based on patients with relapsing polychondritis triggered by cartilage trauma).

In the recent years, it has been of great interest to study the binding mechanism between the innate and adaptive immune responses as interrelated processes for the development of multiple autoimmune diseases. Infection has been a well-known trigger of autoimmunity and trauma has been related as well too. Cryptogenic antigens release, recognition of pathogenic structure, and metabolic changes generated by both stimuli begin an inflammatory process which in turn activates the immune system amplifying T and B cell responses. The development of relapsing polychondritis after trauma may have a dir

A new method of costal cartilage harvest for total auricular reconstruction: part II. Evaluation and analysis of the regenerated costal cartilage.

BACKGROUND: The authors established the hypothesis that chest wall deformity can be absolutely avoided by leaving the perichondrium completely intact at the donor site for cartilage regeneration. As described in Part I, on the new method of costal cartilage harvest, the authors were able to confirm regeneration of cartilage visually and with palpation at 6 months postoperatively. METHODS: The regenerated cartilage of four patients at 6, 8, 10, and 12 months, respectively, after the first-stage operation was harvested for morphological and histological evaluation and analyses during the second…

Factors influencing the oxygen concentration gradient from the synovial surface of articular cartilage to the cartilage-bone interface: a modeling study.

OBJECTIVE: There is very little information on the gradients of oxygen concentration from the synovial surface to the subchondral bone in articular cartilage. Cartilage is usually regarded as hypoxic, even though cellular metabolism is inhibited at low oxygen concentrations. We therefore measured rates of cellular consumption of oxygen and used these rates to calculate oxygen tension profiles across articular cartilage. METHODS: The rate of oxygen consumption by bovine articular chondrocytes was measured in vitro, either in intact cartilage slices or in isolated chondrocytes. The oxygen tensi…

Salmon Nasal Cartilage Proteoglycan Ameliorate Joint Pain and Cartilage Degradation by Regulating Catabolic and Anabolic Homeostasis in MIA-Induced Osteoarthritis.

BACKGROUND/OBJECTIVES: Osteoarthritis (OA) is a pervasive chronic joint disease characterized by the triad of persistent articular cartilage degeneration, debilitating synovial inflammation, and sustained chronic pain. Although salmon nasal cartilage proteoglycan (SPG) is recognized for supporting joint health, the precise molecular mechanism underlying its effects during OA progression remains to be fully elucidated. This study evaluated the therapeutic efficacy of SPG using a monosodium iodoacetate (MIA)-induced mouse model. METHODS: A total of 180 male C57BL/6J mice (six-week-old) were uti…

A soft 3D polyacrylate hydrogel recapitulates the cartilage niche and allows growth-factor free tissue engineering of human articular cartilage.

Cartilage degeneration or damage treatment is still a challenge, but, tissue engineering strategies, which combine cell therapy strategies, which combine cell therapy and scaffolds, and have emerged as a promising new approach. In this regard, polyurethanes and polyacrylates polymers have been shown to have clinical potential to treat osteochondral injuries. Here, we have used polymer microarrays technology to screen 380 different polyurethanes and polyacrylates polymers. The top polymers with potential to maintain chondrocyte viability were selected, with scale-up studies performed to evaluat…

Secondary open structural rhinoplasty with costal cartilage grafts.

BACKGROUND: Cleft lip and palate (CLP) represent one of the most prevalent congenital anomalies of the maxillofacial region, resulting in significant structural deformities that impact nasal function, facial aesthetics, and psychosocial well-being. These anomalies often lead to nasal obstruction and asymmetry. Surgical correction of the associated nasal deformities through rhinoplasty is essential to restore nasal airway function, achieve symmetrical nasal contour, and improve overall facial harmony. Autologous costal cartilage serves as an excellent grafting material due to its versatility, b…

New Perspectives on Ankle Cartilage Pathology and Treatment.

This article describes the etiology, diagnosis and current (arthroscopic) treatment options for osteochondral lesions of the talus (OLT) and tibial plafond (OLTP). OLTs and OLTPs should first be treated nonoperatively. In case of failure, arthroscopic techniques such as bone marrow stimulation or fixation can be performed. For larger, and/or secondary lesions, where arthroscopic techniques are not feasible, open techniques are also available.

Articular Cartilage Injury; Current Status and Future Direction.

Today, treatments of cartilage and osteochondral lesions are routine clinical procedures. The avascular and hard-to-self-repair nature of cartilage tissue has posed a clinical challenge for the replacement and reconstruction of damaged cartilage. Treatment of large articular cartilage defects is technically difficult and complex, often accompanied by failure. Articular cartilage cannot repair itself after injury due to a lack of blood vessels, lymph, and nerves. Various treatments for cartilage regeneration have shown encouraging results, but unfortunately, none have been the perfect solution.…
