Anterior Cervical Discectomy and Fusion for Hirayama Disease: A Case Report and Literature Review.

Hirayama disease, a juvenile muscular atrophy of the distal upper extremity, is a rare form of cervical flexion myelopathy characterized by insidiously progressive weakness of the hands and forearm muscles (i.e., painless amyotrophy). The pathognomonic finding is a markedly forward-shifted spinal cord during neck flexion, demonstrated by dynamic magnetic resonance imaging (MRI), as in a young man with muscle atrophy in the bilateral distal upper extremities. In this report, the authors describe a 31-year-old man who had the classic radiological and clinical presentations of Hirayama disease. S

Classic neurogenic thoracic outlet syndrome in a competitive swimmer: a true scalenus anticus syndrome.

Neurogenic thoracic outlet syndrome (TOS) is caused by compression of the lower brachial plexus usually by a cervical rib or a fibrous band. We describe a 16-year-old girl with weakness and wasting of her right hand, which progressed over the ensuing years. She had been a competitive long distance freestyle and butterfly swimmer since age 8 years. A neurological exam at age 20, revealed severe atrophy and weakness of all intrinsic right hand muscles, more so of the thenar muscles, and hypesthesia along the ulnar aspect of the hand and forearm. EMG, which showed a severe chronic axon loss lower

Guillain-Barré syndrome in a patient with pancreatic cancer after an epidural-general anesthetic.

Guillain-Barre syndrome is a rare occurrence in medicine and is probably rarer still as a postoperative complication. We report an uneventful operative course, during epidural-general anesthesia, in a patient undergoing pancreatectomy who presented with acute paralysis mimicking an acute cervical spinal cord syndrome or brachial plexus neuropathy. The signs and symptoms of right upper extremity paralysis occurred within 2 h postoperatively. Immediate work-up, which included magnetic resonance imaging, electromyography, and nerve conduction velocity studies, provided the diagnosis.

Abduction extension cervical nerve root stress test: anatomical basis and clinical relevance.

PURPOSE: While the Lasègue straight leg raising test is an established test for lumbar nerve root compression, an established equivalent for cervical nerve root compression is missing. The aim of this bi-modal study was to find the most effective way to stretch the cervical nerve roots anatomically in cadavers and to assess its value in the clinical setting. METHODS: Three positional maneuvers of the upper limb were tested on three cadavers to determine the displacement by stretch of the nerve roots C5, C6 and C7. The maneuver which was most efficient in nerve root displacement was applied in

Localization of motoneurons that extend axons through the ventral rami of cervical nerves to innervate the trapezius muscle: a study using fluorescent dyes and 3D reconstruction method.

It has been suggested that in addition to motor axons, which extend directly into the spinal accessory nerve (SAN), ventral rami-associated motor fibers of cervical nerves also innervate the trapezius muscle. Using fluorescent dye labeling and 3D reconstruction in adult rats, this study clarifies the localization of motoneurons, which extend axons either directly through the SAN or through the ventral rami of cervical nerves to innervate the trapezius. DiI or DiI and DiO were used to label the ventral rami of cervical nerves entering the SAN, as well as branches of the SAN. We show that motone

Cervical epidural arteriovenous fistula with radiculopathy mimicking cervical spondylosis.

A 65-year-old woman presented with a rare case of cervical epidural arteriovenous fistula (AVF) manifesting as radiculopathy of the right upper extremity that mimicked cervical spondylosis. She had a 2-month history of gradually progressive right-hand motor weakness and sensory disturbance. The initial diagnosis was cervical disk herniation. However, computed tomography with contrast medium showed abnormal enhancement at the right C5-6 and C6-7 intervertebral foramina. Magnetic resonance (MR) imaging with gadolinium disclosed an enhanced abnormal epidural mass at the dorsal surface of the dura

Congenital segmental spinal muscular atrophy: a case report.

Spinal muscular atrophies are genetic disorders in which anterior horn cells in the spinal cord and motor nuclei of the brainstem are progressively lost. We present a patient with arthrogryposis due to congenital spinal muscular atrophy predominantly affecting the upper limbs. Spinal muscular atrophies with onset at birth may be a cause of arthrogryposis. Localized forms of neurogenic arthrogryposis have been divided into cervical and caudal forms. Our case is similar to the cases described by Hageman et al (J Neurol Neurosurg Psychiatry 1993;56:365-368): severe symmetric lower motor neuron de

Persistent cough associated with osteophyte formation and vagus nerve impingement following cervical spinal surgery: case report.

Persistent cough due to irritation of the vagus nerve by osteophytes resulting from cervical spinal surgery is a very rare condition. The authors report the case of a 49-year-old woman who presented with a persistent cough subsequent to cervical spinal surgery. One year after the initial operation, the patient underwent surgery to free the larynx from the prevertebral fascia and cut the pharyngeal plexus, but her symptoms persisted. In order to control the cough, she used a soft cervical collar with padding inserted in the left side so that the larynx would be pushed to the right, a solution s

Ulnar fascicle to brachialis branch of musculocutaneous nerve for restoration of elbow flexion associated with spinal cord tumor and radiation-induced lower motor neuron disease.

Transfer of the ulnar fascicle to the biceps branch of the musculocutaneous nerve, or Oberlin transfer, has been widely used for the treatment of elbow flexion weakness in the setting of upper trunk brachial plexus palsy. The authors present a modified application of this technique for restoration of functional elbow flexion in a 30-year-old woman with a history of recurrent upper cervical spinal cord pilocytic astrocytoma, complex spinal deformity, and radiation-induced lower motor neuron disease. The video can be found here: https://stream.cadmore.media/r10.3171/2022.10.FOCVID2299.

Sprengel deformity and Klippel-Feil syndrome leading to cervical myelopathy presentation in old age.

Klippel-Feil syndrome is a rare condition characterized by the congenital fusion of two of the seventh cervical vertebrae. A 50-year-old woman presented with a 2-year history of neck pain and ataxia for 1 year. She had not urinary incontinence. She was referred to a neurosurgeon by a neurologist because of her progressive gait ataxia. Risk for brachial plexus injury because of compression or stretching by the clavicle accelerate with age. Therefore, the surgical approach of adults' patients with Sprengel's deformity can intend suitable surgical conclusions.

[Scar neuroma following tumor operations of the head and neck. Immunohistologic studies, differential diagnosis, therapy].

Amputation neuromas occasionally follow division of the cervical plexus during classical neck dissection. They present as painful, subcutaneous, and fixed tumours. They must be removed to exclude recurrence of the primary disease. The differential diagnosis includes neurilemmoma, solitary neurofibroma, neurofibromatosis, neuro-muscular hamartoma, and benign myxoid tumours. Histological diagnosis is particularly easy especially if immunohistochemistry with markers against neural tissues is used. However, to prevent recurrence, the central nerve stump should be exposed, ligated after removal of …

Flexion MRI in a case of Hirayama disease.

We report the case of an 18-year old male with a history of asymmetric weakness and amyotrophy of distal upper extremities, suggestive of Hirayama disease. Magnetic resonance imaging (MRI) of the cervical spine was obtained both in flexion and neutral position. Flexion MRI showed forward displacement of the dura and subsequent cord compression, with associated marked enlargement and postcontrast enhancement of posterior epidural plexus. These findings are pathognomonic of the disorder. On neutral MRI abnormalities may be subtle: in our case, they included loss of physiological lordosis, asymme

[Ultrasonic diagnosis of nodal lesions of the axillary-jugular lymphatic plexus and their evolution].

Axillary lymph nodes of three levels may be adequately differentiated on the basis of the TNM classification by application of layer-by-layer tomographic examination of axillary fossa and thoracic and clavicular-thoracic triangles in the axillary cavity. Echographs show musculus thoracis minor as a distinct point of reference. Topographic features of deep lateral cervical lymph nodes may be used in anatomical identification of internal jugular nodes, transverse cervical artery, accessory nerve and suprajugular lymph nodes. In cases of multiple involvement of lymph nodes, their shape (chain or

Fulminant Cervical Epidural Hematomas: Why Do They Happen, How Can We Minimize Their Occurrence, and What Can We Do When They Do Occur? A Perspective.

BACKGROUND: Epidural hematomas after appropriately performed cervicothoracic interlaminar epidural injections have been associated with the rapid onset of neurological symptoms and devastating outcomes, despite prompt identification and treatment. Anticoagulation issues were initially felt to be the problem, but the occurrence of fulminant hematomas in patients without coagulation forced a reassessment of the causes and responses to this problem. OBJECTIVES: To evaluate why fulminant epidural hematomas occur after cervicothoracic epidural injections, with a literature review to survey knowled

[The use of pelvic packing for intraoperative hemorrhage during pelvic exenteration--case reports].

This is a report of two cases of haemorrhage after pelvic exenteration in patients with vulvar cancer and cervical cancer treated by temporary pelvic packing at the Department of Gynaecology of the Medical University in Gdańsk. The packing was successful and the sponges were removed after 24 and 48 hours. There were no major postoperative complications in both cases and one wound dehiscence. Both patients recovered after surgery. In cases with severe intraoperative haemorrhage, intraabdominal packing can be effective mode of treatment, especially in bleeding which originates from the sacral ve

Brachial plexus neuropathy.

Brachial plexus neuropathy is a common syndrome that, although a distinct clinical entity, is often unrecognized. The illness is characterized by the acute onset of shoulder girdle pain without constitutional symptoms. The pain persists for 1 to 2 weeks, then subsides, with the development of weakness and atrophy in the affected muscles. Good recovery usually occurs but may take up to three years. There is no effective treatment. Since this condition may masquerade as bursitis, cervical disc disease, or other conditions commonly seen by general physicians, it is important to recognize this dis

Catheter cervical vertebral venography.

Cervical vertebral venography using catheterzation via the femoral vein was carried out in 78 cases. In disc disease the internal vertebral venous plexus (IVVP) are interrupted at single or multiple levels, and sometimes completely blocked. In ossification of posterior longitudinal ligament, the IVVP are separated bilaterally and interrupted at multiple levels due to centrally situated ossification and associated osteophytes. Venography helps to determine the upper and lower limits of laminectomy, and the necessary sites of facetectomy. The IVVP in the thoracic region were clearly opacified in

Imaging spectrum and diagnostic insights in Hirayama disease: A case series.

Hirayama disease is a rare, self-limiting juvenile motor neuron disease predominantly affecting young males, characterized by progressive muscular atrophy and weakness in the distal upper extremities due to dynamic compression of the cervical spinal cord during neck flexion. This case series of 10 patients emphasizes the critical role of dynamic flexion MRI in diagnosis and management. All patients underwent clinical evaluation and detailed MRI imaging including dynamic flexion studies. Anterior displacement of the dural sac was observed in all 10 patients on flexion MRI, with engorgement of t

Cervical sympathetic chain schwannoma masquerading as a carotid body tumour with a postoperative complication of first-bite syndrome.

Carotid body tumours (CBT) are the most common tumours at the carotid bifurcation. Widening of the bifurcation is usually demonstrated on conventional angiography. This sign may also be produced by a schwannoma of the cervical sympathetic plexus. A 45-year-old patient presented with a neck mass. Investigations included contrast-enhanced CT, MRI and magnetic resonance arteriography with contrast enhancement. Radiologically, the mass was considered to be a CBT due to vascular enhancement and splaying of the internal and external carotid arteries. Intraoperatively, it was determined to be a cervi
