Magnetic resonance imaging of lesions of synovial origin.

Three patients with histologically differing lesions of synovial origin and two with synovial cysts, one of which was a dissecting popliteal cyst, were examined by magnetic resonance imaging (MR) and computerized tomography (CT). The three histologically proven synovial lesions were synovial sarcoma, diffuse giant cell tumor of tendon sheath, and synovial chondromatosis. In two of the five patients MR provided better anatomic and morphologic appreciation than CT, while in the others they were of equal value. CT demonstrated calcification in two of the lesions while on MR calcification could be…

Osteochondroma of the femoral neck in Perthes disease.

Three out of 480 hips with Legg-Calve-Perthes disease have been found to have intraarticular osteochondroma-like lesions of the femoral neck. In all three cases, the osteochondroma unexpectedly diminished in size during the last phase of reossification and, therefore, did not require surgical excision. The occurrence of osteochondroma of the femoral neck with Perthes disease is rare, and resolution has not been previously reported.

Imaging features reliably distinguish malignancy in periosteal cartilaginous tumors: a two-center analysis of 89 patients.

OBJECTIVES: Periosteal cartilaginous tumors, comprising periosteal chondroma (PCh) and periosteal chondrosarcoma (PChS), are uncommon bone surface tumors that remain poorly characterized radiologically. This study aimed to identify clinical, and imaging features distinguishing PChS from PCh. METHODS: A retrospective analysis was conducted on patients with histologically-confirmed PCh and PChS from two sarcoma reference centers (2001-2024). Inclusion required at least one imaging modality (X-ray, CT or MRI). Radiological features depicting the tumor dimensions, matrix, and peritumoral changes …

The role of metabolic enzymes in mesenchymal tumors and tumor syndromes: genetics, pathology, and molecular mechanisms.

The discovery of mutations in genes encoding the metabolic enzymes isocitrate dehydrogenase (IDH), succinate dehydrogenase (SDH), and fumarate hydratase (FH) has expanded our understanding not only of altered metabolic pathways but also epigenetic dysregulation in cancer. IDH1/2 mutations occur in enchondromas and chondrosarcomas in patients with the non-hereditary enchondromatosis syndromes Ollier disease and Maffucci syndrome and in sporadic tumors. IDH1/2 mutations result in excess production of the oncometabolite (D)-2-hydroxyglutarate. In contrast, SDH and FH act as tumor suppressors and …

≥5 cm Tumor with Cortical Breach Predicts Recurrence and Distinguishes ACT from Enchondroma in Long Bones.

PURPOSE: This study aimed to validate potential clinical and radiological features for distinguishing enchondromas from atypical cartilaginous tumours (ACT) and to analyse the association between maximum tumour diameter and local recurrence of enchondromas. It is important to note that this research serves as a validation cohort rather than proposing a new diagnostic framework. MATERIALS AND METHODS: This retrospective study reviewed the clinical data of 50 patients pathologically diagnosed with enchondroma or ACT and treated in our center between 1 January 2015 and 1 June 2024. Imaging chara…

Chondromyxoid fibroma of the temporal bone: A rare case report.

RATIONALE: Chondromyxoid fibroma (CMF) is a rare form of benign bone tumor and easily misdiagnosed as fibrosarcoma. Hence, to explore the clinical manifestations, diagnostic tests, and therapeutic procedures for temporal bone cartilage myxoid fibroma, it is important to optimize patient treatment and avoid overtreatment. Previous research has discussed cases of CMF, but this paper presents a systematic, complete, and comprehensive introduction of this disease based on this case and related literature. PATIENT CONCERNS: A 52-year-old male patient presented with pain in his right ear for 2 year

Enchondroma of Tibia as Potential False-positive Finding on 68Ga-DOTATOC PET/CT Scan.

A 70-year-old man with a history of carcinoid tumor of small bowel was referred for Ga-DOTATOC study to evaluate the extent of disease. PET/CT scan revealed known metastatic disease in the liver, with other sites of involvement including pancreas, peritoneum, and bones. In addition, moderately intense uptake was noted in proximal right tibia and further correlation on CT showed metaphyseal lesion with "rings and arcs" calcification suggestive of enchondroma. This case highlights the possibility of overexpression of somatostatin receptors in enchondromas, which has been little explored in liter…

Intraosseous atypical chondroid tumor or chondrosarcoma grade 1 in patients with multiple osteochondromas.

BACKGROUND: The autosomal dominant condition multiple osteochondromas, formerly called multiple hereditary exostoses, is associated with a risk of malignant progression of osteochondroma into secondary peripheral chondrosarcoma. Most patients with multiple osteochondromas have exostosin-1 or exostosin-2 gene mutations. To our knowledge, it has not been previously reported that patients may also harbor intraosseous (central) chondroid neoplasms, enchondromas, or atypical chondroid tumors or central chondrosarcomas. The combination of osteochondroma and enchondromas also exists in patients with …

When should we biopsy a solitary central cartilaginous tumor of long bones? Literature review and management proposal.

Differentiation between benign and low-grade malignant cartilaginous tumors is a radiological and pathological challenge. Based on a literature review, we propose the following guidelines for the management of a solitary central cartilaginous tumor of long bones distinguishing three situations: 1. The tumor is considered to be aggressive and requires surgery if one of the following criteria is present: cortical destruction, Moth-eaten or permeative osteolysis, spontaneous pathologic fracture, periosteal reaction, edema surrounding the tumor on MR images, and soft tissue mass. Tumor biopsy foll

[The results of curettage and bone cementing for enchondromas of the distal femur].

OBJECTIVES: We evaluated patients who were treated with curettage and cementing for enchondromas localized in the distal femur. METHODS: The study included 13 patients (7 females, 6 males; mean age 54 years; range 43 to 65 years) who underwent surgery for enchondromatosis in the distal femur. Enchondromas were detected incidentally in 10 patients; of these, complaints of pain appeared afterwards in seven patients. Surgery involved curettage and cement filling of the defects. Preoperative biopsies were obtained in seven patients. Clinical evaluations were made with a visual analog scale. The m…

Cartilaginous choristomas of the tongue: review of the literature and report of three cases.

Three cases of cartilaginous choristomas of the tongue are presented. Two lesions occurred in women of 71 and 28 years of age whose lesions were located in the middle third of the dorsum. The other lesion was found on the left lateral border in a 27-year-old man. The clinical, microscopic and immunohistochemical features are described. The lesions were easily excised with no recurrence after follow-up periods of at least 6 months. The histogenesis and the differential diagnosis are discussed, and the literature is reviewed.

The treatment of enchondromas in the hand by endoscopic curettage without bone grafting.

Nine patients with enchondromas in the hand were treated by endoscopic curettage of the tumour without bone grafting. The procedure was performed on an out-patient basis using axillary block anaesthesia. New bone formation and remodelling of the lesions were observed in all patients. There were no postoperative fractures, infections, recurrences or other complications. Functional recovery was rapid. We conclude that endoscopic curettage without bone grafting is an effective treatment of enchondroma in the hand.

[Enchondroma of the hand].

Enchondromata are the most frequent benign tumours of the bones of the hand. From 1973 to 1990 77 patients were treated at the trauma- and hand surgery department of the Westfälische Wilhelms-University in Münster because of a histologically confirmed enchondroma. Those 52 patients (32 female, 20 male, average age 32 years), who were operated on until 1986 were reviewed. After an average history of 16 months the diagnosis was made most often because of a painful swelling. In 15% a spontaneous fracture was seen and in 17% the enchondroma was diagnosed by chance. The enchondroma was merely exsti…
