Experiences of people living with HIV in low- and middle-income countries and their perspectives in self-management: a meta-synthesis.

INTRODUCTION: Availability of anti-retroviral treatment has changed HIV in to a manageable chronic disease, making effective self-management essential. However, only a few studies in low- and middle-income countries (LMICs) reported experiences of people living with HIV (PLWH) on self-management. METHODS: This meta-synthesis of qualitative studies investigated perspectives of PLWH in LMICs on self-management. Various databases, including PubMed, EMBASE, EBSCO, and CINHAL, were searched through June 2022. Relevant additional articles were also included using cross-referencing of the identified

Integrated care improves mental health in a medically underserved U.S.-Mexico border population.

INTRODUCTION: Chronic conditions, particularly diabetes, and related health conditions continue to be a major concern in the United States, especially in Hispanic populations. This study evaluated the effect of an integrated behavioral health care model, including promotoras(es), on a primarily Hispanic population living with diabetes. METHOD: Seven hundred fifty-six participants were enrolled in an intervention (n = 329) or comparison group (n = 427) and followed up for 12 months. We used a quasiexperimental design to compare participants who received coordinated integrated behavioral health

Juvenile dermatomyositis: a Roy nursing perspective.

With current medical treatment, juvenile dermatomyositis (JDMS) is rarely fatal. However, for most families, it is a chronic illness that interrupts life for both the affected child and the rest of the family. This article provides an overview of the disease, presents a case history of a 9-year-old affected girl, and discusses nursing care using the Roy Adaptation Model and nursing diagnoses.

Health outcomes of patients with chronic disease managed with a healthcare kiosk in primary care: protocol for a pilot randomised controlled trial.

INTRODUCTION: The rising prevalence of chronic disease is leading to an increase in the demand for primary care services and a shortage of primary care physicians globally. Addressing these challenges calls for innovations in the healthcare delivery model with greater use of healthcare technology tools. We previously examined the feasibility of using an automated healthcare kiosk for the management of patients with stable chronic disease in the primary care setting. The aim of this follow-up study is to evaluate the health outcomes of patients with chronic disease who are on kiosk management c

Detection and follow-up of chronic health conditions in Rio de Janeiro - the impact of residency training in family medicine.

BackgroundThere is a need for evidence that residency training in family medicine can benefit the care of patients in primary care in low- and middle-income countries. We tested the hypothesis that two years of residency training in family medicine enables doctors to better detect chronic health conditions while requesting fewer laboratory tests and providing more follow-up visits.MethodsWe performed a retrospective longitudinal observational analysis of medical consultations from 2013 to 2018 in primary care in Rio de Janeiro, comparing doctors without residency training in family medicine (G

Design and rationale for Exploring Predictors of successful healthCare Transition for adolescents with chronic conditions in a longitudinal explorative cohort.

BACKGROUND: Transitioning from paediatric to adult healthcare is a pivotal phase for adolescents and young adults (AYAs) with chronic conditions. Effective healthcare transition (HCT) supports continued healthcare engagement and stable psychosocial outcomes, reducing risks for patients, their families, and the healthcare system. Yet the determinants of a smooth transition are complex, shaped by the interplay of various biopsychosocial constructs. OBJECTIVE: The ExPeCT study (acronym: Exploring Predictors of a successful healthCare Transition) seeks to: A) examine the relationship between biop

Shared decision making among parents of children with mental health conditions compared to children with chronic physical conditions.

High quality care in pediatrics involves shared decision making (SDM) between families and providers. The extent to which children with common mental health disorders experience SDM is not well known. The objectives of this study were to examine how parent-reported SDM varies by child health (physical illness, mental health condition, and comorbid mental and physical conditions) and to examine whether medical home care attenuates any differences. We analyzed data on children (2-17 years) collected through the 2009/2010 National Survey of Children with Special Health Care Needs. The sample cons

Exposure to high stress in the intensive care unit may have negative effects on health-related quality-of-life outcomes after cardiac surgery.

OBJECTIVE: Up to 20% of patients do not show improvements in health-related quality of life (HRQL) after cardiac surgery, despite apparently successful surgical procedures. We sought to determine whether failed improvements in HRQL after cardiac surgery are associated with the development of traumatic memories and chronic stress states as a result of high perioperative stress exposure. DESIGN: Prospective cohort study. SETTING: A 10-bed cardiovascular intensive care unit of a tertiary care university hospital. PATIENTS: A total of 148 cardiac surgical patients. INTERVENTIONS: None. Machine Learning Applications Within the Earlier Medicine Framework for Stroke: A Scoping Review.

This scoping review explores how machine learning (ML) has been applied to stroke research within the Earlier Medicine framework, which promotes proactive and personalized care through primary (preventive care), secondary (acute care), and tertiary (chronic care) prevention levels. Following PRISMA-ScR guidelines, studies were identified from PubMed and Scopus, yielding 105 eligible studies. Most studies focused on tertiary prevention, such as predicting in-hospital mortality, complications, and functional outcomes, while fewer addressed early risk prediction or acute-phase detection. Common p

Advocating for a patient- and family centered care approach to management of short bowel syndrome.

Patient- and family centered care (PFCC) is a model of providing healthcare that incorporates the preferences, needs, and values of the patient and their family and is built on a solid partnership between the healthcare team and patient/family. This partnership is critical in short bowel syndrome (SBS) management since the condition is rare, chronic, involves a heterogenous population, and calls for a personalized approach to care. Institutions can facilitate the practice of PFCC by supporting a teamwork approach to care, which, in the case of SBS, ideally involves a comprehensive intestinal r
