Isolated Phlegmon of the Round Ligament of the Liver: Clinical Decision-Making in the Context of Lemmel's Syndrome-A Case Report.

Background and Clinical Significance: The pathology of the round ligament (RL) is rare and often remains in the shadow of common surgical emergencies. The preoperative diagnosis is challenging, leaving the surgeon perplexed as to whether and when to operate. The presented case deserves attention due to the difficult decision to operate based solely on the clinical picture, despite negative imaging diagnostic results. Case presentation: A 76-year-old woman was admitted to the Emergency Department with 6 h complaints of epigastric pain, nausea, and vomiting. She was afebrile with stable vital si

Monitoring of the right ventricular responses to pressure overload: prognostic value and usefulness of echocardiography for clinical decision-making.

Regardless of whether pulmonary hypertension (PH) results from increased pulmonary venous pressure in left-sided heart diseases or from vascular remodeling and/or obstructions in pre-capillary pulmonary vessels, overload-induced right ventricular (RV) dysfunction and its final transition into right-sided heart failure is a major cause of death in PH patients. Being particularly suited for non-invasive monitoring of the right-sided heart, echocardiography has become a useful tool for optimizing the therapeutic decision-making and evaluation of therapy results in PH. The review provides an updat

The added value of conventional defecography and MRI defecography in clinical decision making on treatment for posterior compartment prolapse.

INTRODUCTION AND HYPOTHESIS: Conventional defecography and MRI defecography can be requested as an additional test for diagnosing and differentiating the type of posterior compartment prolapse and/or obstructive defecation disorders. The objective of this study was to determine the added value of conventional defecography, conventional defecography and MRI defecography for clinical decision-making on treatment for patients with posterior compartment prolapse. METHODS: Four gynecologists were asked to fill in their treatment plan per patient for 32 cases for three different steps. Step 1 consi…

Is a Patient with Paget's Disease of Bone Suitable for Living Kidney Donation?-Decision-Making in Lack of Clinical Evidence.

Living donor kidney transplantation is a widely performed medical procedure. Living kidney donation requires an in-depth health assessment of candidates. The potential living kidney donor must remain healthy after kidney removal. A consequence of donation can be a decrease in glomerular filtration rate (GFR), and donors can become at risk of developing chronic kidney disease (CKD). We present a rationale for potential living kidney donor withdrawal due to Paget's disease of bone (PDB) based on a literature review. The treatment for PDB includes the use of, for example, non-steroidal anti-infla

Dorsal Metacarpophalyngeal Dislocations of Thumb in Children-Clues to the Clinical Decision Making and a Dorsal Approach to Open Reduction.

Dorsal Metacarpophalyngeal joint dislocation of thumb in children is a rare occurrence. All the three different types, namely Simple Complete, Complex Complete and Incomplete dislocations have been described in this article with clinical and radiological clues to differentiate the closely reducible (simple) dislocations from irreducible ones (complex). A simple approach to treatment decision making has been adopted for all cases, starting with attempt of closed reduction which if fails, moving to open reduction. First case is a simple dorsal dislocation presenting after fall injury with hypere

Emergent stenting to treat patients with carotid artery dissection: clinically and radiologically directed therapeutic decision making.

BACKGROUND AND PURPOSE: Carotid artery dissection (CAD) is a common cause of stroke in young patients. Clots formed at the low-flow zone in the false arterial lumen may give rise to distal emboli, and the mural hematoma may eventually occlude the artery. Anticoagulation is currently the accepted treatment, but it is unknown which patients will improve spontaneously, which will respond to anticoagulation, and which will have an exacerbation of ischemic symptoms despite therapy. Endovascular treatment of CAD may be an attractive alternative to anticoagulation, and methods of identifying patients

Medical education is transforming due to new technologies such as artificial intelligence (AI). Although AI is being considered for medical education, its integration into the curriculum remains limited. Furthermore, few studies evaluate how these tools affect students' diagnostic accuracy during clinical rotations. This study aims to evaluate available evidence regarding AI tools in medical students, identifying their effects on clinical education and clinical decision-making, as well as their implications for medical practice during clinical rotations. For this purpose, a scoping review was …

Serum neurofilament light chain in multiple sclerosis: from biological signal to clinically informed decision-making.

Serum neurofilament light chain (sNfL) is an analytically robust blood-based biomarker of recent neuroaxonal injury in multiple sclerosis (MS), supported by validated ultrasensitive immunoassays suitable for routine measurement. These assays now enable reliable measurement in routine care, and evidence links higher levels and rising trajectories to inflammatory activity, treatment response, and-at a population level-future tissue loss and disability risk. Clinical implementation is constrained by overinterpretation of single measurements: sNfL is not MS-specific, is strongly age-dependent, is

Who makes the decisions? Uncovering the evolutionary implications and clinical applications of Toxoplasma gondii's Fatal Feline Attraction.

Here I recount my research journey on the coccidian protist Toxoplasma gondii (Nicolle et Manceaux, 1908), a ubiquitous parasite capable of infecting all warm-blooded animals as intermediate or secondary host, but with only members of the Felidae as its definitive host. I describe my initial studies into its epidemiology and persistence within the UK, and how this led on to a series of biologically and ethically appropriate studies into T. gondii's apparent specific manipulation of its rat intermediate host to facilitate transmission to its feline definitive host. I then describe how this prom

The clinical experience of fetoscopic repair of myelomeningocele in Taiwan: The dilemma in prenatal decision-making and first successful case.

OBJECTIVE: Objective: Myelomeningocele (MMC) is the most severe type of spina bifida, with an incidence of 1.87 per 10,000 live births in Taiwan. Exposure of the lesion to amniotic fluid exacerbates neurological outcomes, while fetal surgery for MMC repair, now a routine practice, improves postnatal outcomes. However, Asian women and their families often find it difficult to accept prenatal defects, leading nearly all pregnancies with fetal MMC to opt for termination without considering fetal surgery. MATERIALS AND METHODS: In Taiwan's first approved trial of fetoscopic MMC repair, we prospec
