Improving Quality in the Treatment of Pediatric Forearm Fractures: Minimizing the Need for Repeat Intervention With Long-arm Extension Casting.

PURPOSE: Repeat intervention for pediatric forearm fractures after closed manipulation is as high as 46% with flexed elbow (FE) long-arm casts. Casting with the elbow extended (EE) has been advocated as an alternative technique. We documented outcomes of patients treated with closed manipulation and casting with EE long-arm casts for displaced, diaphyseal both-bone forearm fractures. METHODS: We retrospectively reviewed charts for patients who had sedated manipulations and casting for closed, diaphyseal forearm fractures. Open fractures, immediate surgical intervention, metabolic bone disease

Use of emergency room external fixator in provisional reduction of posterior malleolar fractures.

Closed reduction is an important initial step in managing ankle fractures. Although the majority of ankle fractures can be managed with closed reduction, the posterior malleolar fracture is often unstable. Posterior malleolar fractures may result in persistent posterior ankle dislocation with compromise of the soft tissue surrounding the joint. Persistent dislocation is best treated with urgent open reduction and fixation to protect the cartilage and surrounding soft tissues and to allow for ease of reduction at the time of surgery. We describe a technique for placing an emergency room externa

Intramedullary nailing for the treatment of dislocated pediatric radial neck fractures.

INTRODUCTION: Pediatric radial neck fractures represent 5-10% of all elbow fractures. Open reduction allows anatomic fracture fixation but c … ing. The average age of the patients was 8 years. Thirty-eight patients (90%) could be treated with closed fracture reduction and retrograde nailing. Open fracture reduction was required in 4 (10%) patients, and in 2 patients necrosis of the radial head occurred. Loss of reduction was seen in 7 (17%) patients. Excellent results were found in 38 patients, good results in 2 …

Variation in Treatment of Young Adult Distal Radius Fractures by Pediatric and Adult Orthopaedic Surgeons.

INTRODUCTION: There remains a lack of consensus on the optimal treatment of isolated distal radius fractures in young adults. The primary aim of this study was to identify differences in treatment of isolated distal radius fractures in patients aged 17 to 21 years treated by adult versus pediatric orthopaedic surgeons. The secondary aim was to identify whether there is a variation in utilization of open reduction and internal fixation (ORIF) versus closed reduction and percutaneous pinning when treated surgically by adult versus pediatric orthopaedic surgeons. METHODS: Patients aged 17 to 21

Displaced Radial Neck Fractures: What Are My Options?

Pediatric radial head and neck fractures are uncommon injuries. Fractures are classified using the Judet system based on fracture angulation and displacement. Judet type I and II fractures can be managed nonsurgically with a short course of immobilization in a cast or splint without closed reduction. Most of these patients have an excellent prognosis and functional outcomes. Judet type III and IV injuries, as well as injuries that demonstrate a mechanical block to motion, should be closed reduced with the patient under conscious sedation or general anesthesia. Patients who undergo an unsuccess

Carpometacarpal dislocation of the thumb associated with ipsilateral fracture of the distal radius.

An acute carpometacarpal (CMC) dislocation of the thumb is itself an extremely rare injury, but one associated with a fracture of the distal radius has never before been reported. In our case, the fracture of the distal radius was treated by closed reduction and immobilization with a plaster cast, whereas the dislocation of the CMC of the thumb was treated with closed reduction, percutaneous fixation with Kirschner wires, and immobilization with a plaster cast. At the patient's two-year follow-up examination, the distal radius fracture displayed successful functional and radiographic results,

Knee dislocations and associated fractures: risk factors for surgical reduction.

INTRODUCTION: Dissociation of the knee joint, or knee dislocations (KD), can lead to severe complications, often resulting in multiligament injuries. A subset of these injuries are irreducible by closed reduction and require open reduction. Identifying KDs that necessitate surgical intervention is crucial for optimal outcomes. While previous studies have explored various risk factors, the influence of associated fractures is less understood. MATERIALS AND METHODS: We queried the Trauma Quality Improvement Program (TQIP) database from 2017 to 2021, for non-congenital closed knee dislocations r

Surgical Management for Posterior Atlantoaxial Dislocation without Fracture and Atlantoaxial Dynamic Test to Confirm the Integrity of the Transverse Ligament: A Case Report.

BACKGROUND: Traumatic posterior atlantoaxial dislocation (PAAD) without fracture of the odontoid process is a rare injury. Closed reduction by skull traction under C-arm fluoroscopic guidance and open reduction have been reported previously for the treatment of PAAD. OBJECTIVE: To report a rare case of PAAD without fracture treated by closed manual reduction and posterior fixation. To provide a new method-atlantoaxial dynamic test-for confirming the integrity of the transverse ligament after reduction and evaluate the ideal treatment strategy for traumatic PAAD without fracture of the odontoi

Femoral neck fracture in a child with autosomal-dominant osteopetrosis: failure of spica cast treatment and successful outcome by internal fixation.

A case report of a 6-year-old child with autosomal dominant osteopetrosis who sustained a closed femoral neck fracture initially treated with a spica cast with subsequent fracture displacement is presented. Further treatment included closed reduction and operative fixation, first, with a guide pin placement for a cannulated screw insertion that was abandoned because of the hardness of the bone. Three Kirschner wires were then inserted to maintain the closed reduction, followed by immobilization in a spica cast. At the 2-year follow-up, the child was completely free of any complications resulti

Triplane fractures of the distal tibial epiphysis.

Triplane distal tibial fractures can occur as two-, three-, or four-part fractures with or without a fibular fracture. Diagnosis of the particular anatomy of each fracture is ascertained by plain radiographs; if the fracture is displaced 2 mm or more on any view, anteroposterior and lateral tomograms and, if possible, a limited computerized tomography (CT) scan should be done. A plaster cast in situ for non-displaced fractures or closed reduction for displaced fractures should be attempted first by internal rotation and anterior movement of the fibular metaphyseal piece. Failure to obtain and/

Bado III Monteggia in children - treatment options and outcome: an ultrasound control study.

The Bado III Monteggia lesion is an uncommon fracture in children. Radial head dislocation can occur due to intra- or extra-articular fractures of the olecranon. In extra-articular ulnar fractures, heterogeneous operative methods are proposed in different case reports. The objective of this study is to determine if closed reduction of the ulnar fracture, either without osteosynthesis or by using TEN, is a safe treatment option. In a multicenter study, we retrospectively analyzed 26 children who were treated for an extra-articular proximal metaphyseal ulnar fracture with dislocation of the radi

Gartland type III extension supracondylar humerus fracture in a 10-year-old child. A surgical case report of an infrequent technique of medial and lateral column stabilization.

INTRODUCTION AND IMPORTANCE: Supracondylar humeral fractures in children are the most common fractures of the elbow accounting for 16 % of all pediatric fractures. The treatment depends on age, the degree of displacement, and the presence of additional injuries. PRESENTATION OF CASE: A case reports a 10-year-old girl with a Gartland type III supracondylar humeral fracture accompanied by anterior interosseous nerve neurapraxia preoperatively. The patient was treated operatively with medial and lateral column cross-pinning using four K-wires due to unsatisfactory closed reduction and lateral pi

Recommendations for Early Surgical Intervention in Adolescents With Salter-Harris II (SH2) Distal Radius Fractures.

INTRODUCTION: Nonsurgical management is the standard of care for most distal radius fractures involving the epiphyseal plate. However, because of the lack of remodeling potential in adolescents, nonanatomic reduction can result in a symptomatic malunion necessitating late complex surgical correction. Guidelines outlining optimal management strategies for SH2 distal radius fractures in the adolescent population are needed. METHODS: This study is a retrospective review of isolated SH2 distal radius fractures from two, large, regional, referral pediatric ACS trauma level 1 hospitals between 2017

A Method for Precise Reduction of Facial Bone Fractures Using Intraoperative Ultrasound With a Solid Gel Pad as a Coupling Medium.

This study introduces a method to overcome technical challenges in using intraoperative ultrasound for the closed reduction of nasal bone and zygomatic arch fractures. The curvature of the face makes it difficult to apply an ultrasound probe to the facial area. We used a solid gel pad as a coupling medium during surgery to improve the scanning of facial bone fractures. The results show that the fracture sites observed on preoperative computed tomography scans can be easily visualized using intraoperative ultrasound, and real-time manipulation confirms successful reduction. The solid gel pad is

Bilateral Bennett's fracture: A case report.

Bennet's fracture represents one of the most common injuries of the hand district, involving the base of the thumb, and it is affects children and elderly patients the most. The fracture is caused by direct axial trauma to a partially flexed first metacarpal and it is always intra-articular: the fracture line separates the palmar ulnar aspect of the first metacarpal base from the remaining first metacarpal. The fracture pattern is such that the first metacarpal shaft moves dorsally, proximally, and radially due to the pull of the abductor pollicis longus, extensor pollicis longus, extensor pol

Traumatic Obturator Dislocation Following Total Hip Arthroplasty Managed with Closed Reduction: A Case Report and Review of the Literature.

CASE: A 30-year-old man sustained an obturator dislocation of a left total hip prosthesis with a concomitant ipsilateral fracture of the greater trochanter following a high-energy motor-vehicle accident. With the patient under general anesthesia, the hip was successfully treated with closed reduction, as confirmed with intraoperative fluoroscopy and postoperative radiographs. Postreduction, the patient was able to mobilize with abduction restrictions and had no additional instability. Short-term follow-up showed evidence of healing of the greater trochanter. CONCLUSION: Obturator dislocation

Comparison of outcomes of ORIF versus bidirectional tractor and arthroscopically assisted CRIF in the treatment of lateral tibial plateau fractures: a retrospective cohort study.

BACKGROUND: Lateral tibial plateau fractures (TPFs) are often treated with conventional open reduction and internal fixation (ORIF) through standard anterolateral sub-meniscal arthrotomy. There has been increasing support for "bidirectional rapid redactor" device-assisted closed reduction and internal fixation (CRIF) for treating TPFs. The aim of the present study is to compare the clinical and radiological outcomes between CRIF and ORIF procedures. METHODS: We performed a retrospective cohort study of 55 lateral TPF patients (Schatzker types I-III) who accepted surgical treatment at our trau
