Antenatal sonographic diagnosis of congenital clubfoot: a possible indication for amniocentesis.

Eighteen cases of congenital clubfoot were diagnosed sonographically between 16 and 38 weeks. The method of diagnosis, associated anomalies, karyotypes, and obstetric outcomes are described. Fifteen of the 18 fetuses had other major congenital anomalies, and of these, four had abnormal karyotypes. Only three had uncomplicated clubfoot as the only abnormality. Because of the significant incidence of abnormal karyotypes in our series, confirmed in other reported cases, identification of a clubfoot on prenatal sonography may be considered an indication for amniocentesis, particularly when other a…

[Causes of recurrence of congenital clubfoot in children and indications for surgical treatment].

Unsatisfactory results of the conservative treatment of club foot deformity in 220 patients (346 feet) were analyzed. Data of roentgenograms in 86 patients enabled the authors to make a conclusion that an early cause of recurrent club foot is the delayed formation of nuclei of ossification of the navicular and I sphenoid bones, the later cause of the recurrent deformity is early synostosis of the epiphysis of the metatarsal bone. The importance of dispensary observation ad prophylactic measures after the treatment of congenital club foot is emphasized.

[Experience with the surgical treatment of congenital clubfoot in children by the Shturm method].

The authors have studied results of the operative treatment of a typical form of congenital talipes in 220 patients (346 feet) operated by the method of V.A. Shturm Remote results of the surgical treatment were followed in 157 patients (255 feet). Good results were obtained in 120 feet (47,1%), unsatisfactory--in 21 feet (8,2%). Good results were noted in patients operated at the age of 2-3 years.

Congenital arterial malformations associated with clubfoot. A report of two cases.

Arteriograms were examined in three limbs of two patients who had talipes equinovarus. An abnormal arterial pattern characterized by severe diminution in the size of the anterior tibial artery and diminution or absence of the dorsalis pedis artery was evident. Inasmuch as one of these patients was preambulatory, the arterial malformation is congenital and would not have been acquired in walking. The relationship between the abnormal arterial and bony pattern in talipes equinovarus is comparable to that previously described for congenital absence of the fibula. A possible etiologic relationship

Correlation Between Socioeconomic Status and Brace Compliance in Idiopathic Clubfoot Deformities.

BACKGROUND: Congenital talipes equinovarus (CTEV), or clubfoot, is one of the most common foot deformities seen at birth. The Ponseti technique is the most common method to treat clubfoot and consists of gentle manipulation with serial casting, a percutaneous Achilles tendon tenotomy, and bracing for the first few years of life. The purpose of this study was to determine whether socioeconomic factors influence compliance with clubfoot bracing for families with infants who have idiopathic clubfoot treated by the Ponseti method. METHODOLOGY: All patients with clubfoot deformity who began primar…

Muscle force imbalances predict clubfoot recurrence risk: A musculoskeletal modelling approach.

BACKGROUND AND OBJECTIVES: Idiopathic clubfoot, primarily treated with the Ponseti method, recurs in 20-40 % of cases, often within the first two years post-treatment. This recurrence is commonly linked to imbalances in the inverting and everting muscle and ligament forces surrounding the foot, disrupting ankle-foot joint alignment during gait. This study aimed to evaluate the biomechanical factors contributing to clubfoot recurrence using advanced musculoskeletal modeling. METHODS: Experimental data, including 3D segment positions, lower limb muscle activity, and ground reaction forces, were…

First Trimester Fetal Clubfoot: A Novel Presentation of Severe Osteogenesis Imperfecta.

Talipes equinovarus, also called clubfoot, is a relatively common congenital defect affecting approximately one in every 1000 live births. Most cases of clubfoot are expected to be idiopathic and unrelated to an underlying genetic syndrome. In approximately 20% of cases, a clear genetic etiology is identified. Here we present two cases of bilateral clubfoot identified via fetal ultrasound in the first trimester associated with osteogenesis imperfecta diagnosed in the second trimester. Both fetuses presented with multiple fractures and were identified to have loss-of-function variants in COL1A1…

Ponseti Clubfoot Casting: Factors That Affect Trainee Competency (Retrospective Observational Study).

INTRODUCTION: This study investigates how previous simulation training and clinical experience affects trainee performance when manipulating a foot, applying a Ponseti clubfoot cast, and performing an Achilles tenotomy on a clubfoot simulator. METHODS: Sixty-four Accreditation Council for Graduate Medical Education orthopaedic trainees participated in the 2017 to 2018 Top Gun (TG) skills competition at the International Pediatric Orthopaedic Symposium. Trainees were judged by expert pediatric orthopaedic surgeons on how they manipulated a clubfoot model, applied a cast, and performed a simula…

Clinical outcomes of iatrogenic complex clubfoot treated with modified Ponseti method.

INTRODUCTION: There are not enough studies demonstrating the results of the modified Ponseti method on iatrogenic complex clubfoot that occurs due to errors during the application of the method for treating idiopathic clubfoot. The present study aimed to present the treatment results of the modified method reported by Ponseti for treating feet that became complex solely due to errors during casting. METHODS: Patients with the confirmed diagnosis of iatrogenic complex clubfoot were according to initial physicians' report and photographs were included in this retrospective case series study. Pa…

Dimeglio Score Predicts Treatment Difficulty During Ponseti Casting for Isolated Clubfoot.

BACKGROUND: The Dimeglio score (DS) is widely used to assess clubfoot severity, but its ability to predict long-term outcomes following Ponseti treated isolated clubfoot (IC) is controversial. This study tested the association between the initial DS and its individual parameters with the number of Ponseti clubfoot casts required to achieve correction and the rate of early recurrence following treatment. METHODS: Data were retrospectively collected from patients who underwent treatment of IC between March 2012 and March 2015 and were followed for ≥2 years. DSs were collected at the initial cas…

Aesthetic Improvement of Undeveloped Calves After Treatment of Congenital Clubfoot Deformity.

BACKGROUND: Even when clubfoot deformity is treated in a timely manner, the consequences observed in adulthood include hypoplasia of the calf muscles, gait impairment, decreases in foot size, and it can also affect the tibial length. These consequences may have negative impacts on the patient's subjective appraisal of long-term outcomes, and can influence the patient's self-esteem in both male and female patients. OBJECTIVES: We present our experience in the treatment of undeveloped calves after surgical treatment of congenital clubfoot. METHODS: In total, 72 patients underwent corrective su…

Clubfoot Does Not Impair Gross Motor Development in 5-Year-Olds.

PURPOSE: To evaluate the gross motor development of 5-year-olds using the Peabody Developmental Motor Scales, 2nd Edition (PDMS-2), test after initial nonoperative management of clubfoot as infants. METHODS: The PDMS-2 Stationary, Locomotion, and Object Manipulation subtests were assessed on 128 children with idiopathic clubfeet at the age of 5 years. Children were categorized by their initial clubfoot severity as greater than 13, unilateral or bilateral involvement, and required surgery. RESULTS: Children with treated clubfeet had average gross motor scores (99 Gross Motor Quotient) compare…
