Sirolimus delays recovery of rat kidney transplants after ischemia-reperfusion injury.

BACKGROUND: Sirolimus (SRL) seems to impair renal recovery from ischemic injury in animal models and delayed graft function after clinical renal transplantation. This study determined the impact of SRL on renal recovery after ischemia-reperfusion injury in a rat kidney transplant model. METHODS: Syngeneic kidneys were preserved in University of Wisconsin solution before transplantation into bilaterally nephrectomized rats. Recipients received vehicle or SRL targeting for whole-blood trough levels of 10 to 20 ng/mL as measured by high-performance liquid chromatography. Renal function was asses

Transient receptor potential melastatin 8 is required for nitroglycerin- and calcitonin gene-related peptide-induced migraine-like pain behaviors in mice.

Migraine is a complex neurovascular disorder that is one of the leading causes of disability and a reduced quality of life. Even with such a high societal impact, our understanding of the cellular and molecular mechanisms that contribute to migraine headaches is limited. To address this complex disorder, several groups have performed genome-wide association studies to elucidate migraine susceptibility genes, with many identifying transient receptor potential melastatin 8 (TRPM8), a cold-sensitive cation channel expressed in peripheral afferents innervating the trigeminovascular system, and the

Nonlinear bioheat model for dynamics of hypothermia and frostbite. Ⅰ. Modeling aspects.

We propose a new bioheat model for thermoregulation in the human body in response to cold environments, with emphasis on hypothermia and frostbite in exposed extremities. The model couples the bioheat transfer equation in the extremity with a differential equation that describes the core temperature. We used simulations to illustrate the connection between microscale vascular exchange and the effective perfusion term in the bioheat transfer equation. The nonlinear coupling proposed here incorporates physiologically motivated feedback laws for local and reflex vasoconstriction, as well as heat

Yellow Granular Material in Hair: A Bedside Clue to Overdose from Cold Medication Containing Acetaminophen.

CASE PRESENTATION: A 28-year-old woman was brought to the emergency department after being found collapsed at home. Because she was unable to provide a history, contextual bedside clues were important during the initial assessment. Bright yellow granular material with a medicinal odor was noted adherent to her hair. This unusual finding raised suspicion of overdose with an acetaminophen-containing over-the-counter (OTC) cold medication. Her serum acetaminophen concentration was elevated at 135 micrograms per milliliter, and N-acetylcysteine was promptly administered. She did not develop hepati

THC and gabapentin interactions in a mouse neuropathic pain model.

Clinical studies have shown that the major psychoactive ingredient of Cannabis sativa Δ9-tetrahydrocannabinol (THC) has some analgesic efficacy in neuropathic pain states. However, THC has a significant side effect profile. We examined whether the profile of THC could be improved by co-administering it with the first-line neuropathic pain medication gabapentin. This was done using the chronic constriction injury (CCI) model of neuropathic pain in C57BL6 mice. At 8 days post-CCI nerve injury, acute systemic administration of gabapentin produced a dose-dependent decrease in CCI-induced mechanica

Impairment of VGLUT2 but not VGLUT1 signaling reduces neuropathy-induced hypersensitivity.

Glutamate is the major excitatory neurotransmitter in the central nervous system with an important role in nociceptive processing. Storage of glutamate into vesicles is controlled by vesicular glutamate transporters (VGLUT). Null mutants for VGLUT1 and VGLUT2 were poorly viable, thus, pain-related behavior was presently compared between heterozygote VGLUT1 and VGLUT2 mice and their respective wild-type littermates using a test battery that included a variety of assays for thermal and mechanical acute nociception, and inflammatory and neuropathic pain syndromes. Behavioral analysis of VGLUT1 mu

Extraterritorial temperature pain threshold abnormalities in subjects with healed thermal injury.

Approximately 1.25 million individuals sustain burn injuries annually in the United States. Pain is frequent in patients with burn injuries and is often refractory to pharmacotherapy. We report quantitative sensory data from five subjects who sustained external thermal injuries to their limb(s) 8 weeks to 11 years previously, demonstrating reduced thermal pain thresholds in regions outside the burn injury zone, including contralateral limbs. Warm and cold detection thresholds were not significantly different from controls. These results complement data from animal models that demonstrate that

Activation of peripheral cannabinoid receptors by CB13 attenuates burn injury-induced chronic pain through TRP channel modulation.

BACKGROUND AND PURPOSE: Burn injury-induced pain is a devastating disorder affecting more than 20% of the population worldwide. Cannabinoid receptors (CB1 and CB2) modulate pain signalling, but central CB receptor agonists result in side effects (sedation, respiratory depression, cognitive dysfunction, addiction and abuse). To address this challenge, we have investigated the potential effect of CB13, a peripherally restricted CB1/CB2 receptor dual agonist, for the first time on burn injury induced pain rats and elucidated other associated mechanisms. EXPERIMENTAL APPROACH: Burn injury was ind

Physical neurosensory testing versus current perception threshold assessment in trigeminal nerve injuries related to dental treatment: a retrospective study.

OBJECTIVE: To provide a preliminary comparison of traditional clinical neurosensory examination (CNE) and current perception threshold (CPT). METHOD AND MATERIALS: This is a retrospective chart audit of patients with trigeminal nerve injuries related to dental treatment who were evaluated with both CNE and CPT assessments (electrical stimulus tests) after presenting with altered sensation involving either the inferior alveolar nerve (IAN) or lingual nerve (LN) distributions. The tests were performed on the anterior two-thirds of the tongue for LN injuries and the lower lip and chin for IAN in

Development of Transplant Surgeon-Innovated Ex Vivo Machine Perfusion: Validation in a Porcine Donation after Circulatory Death-Simulated Liver Transplant Model.

PURPOSE: Ex vivo machine perfusion (EVMP) is increasingly recognized as a promising technique for enhancing the preservation and viability of donor organs, particularly in donation after circulatory death (DCD) liver transplantation (LT). This study validates a transplant surgeon-innovated EVMP protocol by assessing its efficacy in preserving liver function and reducing ischemia-reperfusion injury (IRI) in a porcine DCD-simulated liver transplant (DCD sLT) model. MATERIALS AND METHODS: Twenty Yorkshire pigs were used to compare static cold storage (SCS) and EVMP. In Model 1, the SCS group (n=

Understanding donor heart preservation: Cellular mechanisms and clinical outcomes.

Static cold storage (SCS) on ice has remained the gold standard preservation method for heart transplantation, and prolonged cold ischemia outside the typical 4-6 hour window is associated with an increased risk of primary graft dysfunction - a consequence attributed to ischemic damage and reperfusion injury. This, unfortunately limits the travel radius for donor heart procurement, a key factor that contributes to the overall shortage of donor organs. Recent research and clinical data have illustrated the validity of other preservation systems in preserving cardiac allografts, and many of thes

The CmABF1-CmBt/CmBr Module Regulates Cucurbitacin B Biosynthesis to Promote Cold Tolerance of Oriental Melon Seedlings.

Low temperature is a common abiotic stress in the early spring and winter cultivation of oriental melons, which seriously affects their normal growth and development, and plants can activate the defence mechanism related to reactive oxygen species through secondary metabolic pathways. Cucurbitacins are a defensive secondary metabolite that produces a bitter taste in Cucurbitaceae plants. However, the molecular regulatory mechanism between low temperature and biosynthesis of cucurbitacins, as well as whether cucurbitacins will enhance the plant's resistance to cold stress, has not been studied

A century of exercise physiology: concepts that ignited the study of human thermoregulation. Part 3: Heat and cold tolerance during exercise.

In this third installment of our four-part historical series, we evaluate contributions that shaped our understanding of heat and cold stress during occupational and athletic pursuits. Our first topic concerns how we tolerate, and sometimes fail to tolerate, exercise-heat stress. By 1900, physical activity with clothing- and climate-induced evaporative impediments led to an extraordinarily high incidence of heat stroke within the military. Fortunately, deep-body temperatures > 40 °C were not always fatal. Thirty years later, water immersion and patient treatments mimicking sweat evaporation we

Axillary Artery Injury In Blunt Shoulder Trauma - An Uncommon Association With Proximal Humerus Fracture.

Axillary artery injury along with proximal humerus fracture as a result of blunt trauma is a rare incidence. We present a 65-year-old male with a pulseless cold hand, who had blunt trauma 3 days ago, leading to fracture of proximal end of humerus along with axillary artery injury. Surgical intervention revealed contusion of axillary artery with intravascular thrombosis causing complete occlusion. Internal fixation of fracture along with excision of contused axillary artery was done followed by interposition venous graft. The diagnostic modalities and treatment options for such a case are discu

Adjuncts to reduce the incidence of embolic brain injury during operations on the aortic arch.

Two adjunctive techniques for operations involving the aortic arch, retrograde perfusion of cold oxygenated blood through the superior vena cava and establishment of antegrade flow after hypothermic circulatory arrest, are described. These techniques should reduce or possibly eliminate the embolization of air and particulate matter into the cerebral circulation and may reduce the incidence of brain injury.

Minocycline Prevents Muscular Pain Hypersensitivity and Cutaneous Allodynia Produced by Repeated Intramuscular Injections of Hypertonic Saline in Healthy Human Participants.

UNLABELLED: Minocycline, a glial suppressor, prevents behavioral hypersensitivities in animal models of peripheral nerve injury. However, clinical trials of minocycline in human studies have produced mixed results. This study addressed 2 questions: can repeated injections of hypertonic saline (HS) in humans induce persistent hypersensitivity? Can pretreatment with minocycline, a tetracycline antibiotic with microglial inhibitory effects, prevent the onset of hypersensitivity? Twenty-seven healthy participants took part in this double-blind, placebo-controlled study, consisting of 6 test sessio

A fibroblast-centric network drives cold fibrosis in the tumor microenvironment of lung squamous cell carcinoma.

The tumor microenvironment (TME) of chronic inflammation-associated cancers (CIACs) is shaped by cycles of injury and maladaptive repair, yet the principles organizing fibrotic stroma in these tumors remain unclear. Here, we applied the concept of hot versus cold fibrosis, originally credentialed in non-cancerous fibrosis of heart and kidney, to lung squamous cell carcinoma (LUSC), a prototypical CIAC. Single-cell transcriptomics of matched tumor and adjacent-normal tissue from 16 treatment-naive LUSC patients identified a cold fibrotic architecture in the LUSC TME: cancer-associated fibroblas

Sensory alteration patterns in burned patients.

INTRODUCTION: Burned patients may present with different type and severity of sensory dysfunction. Regenerative mechanisms in the peripheral nervous system are diminished after burn injury and thus unable to accurately regenerate somatosensitive skin receptors. The pattern by which neuronal regeneration occurs to regain this sensitivity in burn patients is still unclear. PATIENTS AND METHOD: This observational retrospective study focuses on determining the patterns of heat, heat-pain, cold, cold-pain, sympathetic skin response and touch following severe burns. Twenty-six burn patients with di
