Pulmonary cryptococcosis presenting as an endobronchial tumour-like growth has rarely been described. We report the case of a male patient with normal immune function who presented with a right upper lobe mass lesion. Bronchoscopy revealed a tumour-like growth that completely occluded the anterior segment of the right upper lobe bronchus. Bronchial biopsy and computed tomography-guided lung biopsy revealed Cryptococcus, and culture of lung biopsy specimen grew Cryptococcus neoformans and Mycobacterium tuberculosis. The patient responded clinically to amphotericin B, fluconazole and anti-tuberc…

We encountered a 46-year-old immunocompetent male patient who presented with a right upper lobe collapse on a chest x-ray. A flexible bronchoscopic examination revealed an endobronchial mass emanating from the right upper lobe and obstructing the right mainstem bronchus. Bronchial washings and biopsy of the lesion were consistent with cryptococcal infection. The response to oral diflucan therapy was suboptimal. Subsequently, the patient underwent a successful bronchoscopic resection of this mass with the placement of an endobronchial stent. We report this rare case of endobronchial cryptococca

Pulmonary cryptococcosis is a fungal infection caused by inhalation of Cryptococcus neoformans. Pulmonary cryptococcal infections tend to occur in immunocompromised individuals, although they can occasionally develop even in immunocompetent hosts. This report presents a retrospective clinical study of 8 patients who underwent a surgical resection for pulmonary cryptococcosis between 1999 and 2008. The age of the patients ranged from 49 to 85 years old (mean 62.6). There were 4 male and 4 female patients. All patients except for 1 had no symptoms. Two patients were immuno-compromised hosts unde…

Osseous involvement occurs in 5-10% of patients with disseminated cryptococcosis. We are reporting an unusual case of disseminated cryptococcosis involving the sternum and lumbar vertebra with the formation of psoas abscess with pulmonary tuberculosis. The patient presented with fever for 3 months. A diagnosis of pulmonary tuberculosis was made on thoracic contrast-enhanced computerized tomography and she was put on antituberculosis treatment. She was immunocompetent with negative human immunodeficiency virus. She conceived subsequently and had complaints of backache and swelling over the ster…

Neuromeningeal cryptococcosis is a serious infection witch occurs essentially in immunodepressed patients and especially AIDS patients. We report 22 cases of cryptococcosis meningitis confirmed by the parasitology laboratory, in the Tunis Rabta hospital, over a 16-year period. Sixteen patients were HIV infected and six were not HIV infected. The clinical examination documented fever and headache as well as focal neurological signs especially in HIV infected patients. The mycological examination of CSF proved the diagnosis of neuromeningeal cryptococcosis in all cases. The first line treatment …

Four cases of concomitant tuberculosis and cryptococcosis infection in HIV-positive patients are described. As the HIV pandemic progresses and the proportion of patients with end-stage disease increases, a high suspicion of incidence and unusual forms of infections must always be kept in mind.

A 46-year-old Hispanic male from Ecuador presented with fever, malaise, weight loss, and dyspnea. He was HIV-positive with a CD4+ cell count of 4/microL. He was hospitalized, and therapy for community-acquired pneumonia with broad-spectrum antibiotics was started. Both Histoplasma capsulatum and Cryptococcus neoformans were cultured from bronchial lavage, blood, and bone marrow specimens. Despite aggressive therapy with amphotericin B, the patient died 8 days after admission.

Cryptococcosis is a systemic mycosis with a worldwide distribution. It frequently occurs in patients who are immunologically compromised or chronically ill. Clinical manifestations are usually confined to the central nervous system, lungs and skin. Involvement of the hepatobiliary system is very rare. We describe the MR imaging appearance of a rare case of disseminated cryptococcosis in an immunocompetent child in whom the clinical presentation was dominated by biliary and lymph nodal involvement.

A wide spectrum of pathomorphological changes in the brain was revealed. They reflect various periods of cryptococcal infection under various conditions of immunodeficiency. In cases of brain cryptococcosis in HIV-infection, a significant spread of necrotic changes was noted. The dependence of structural changes severity in the brain on the size of fungal burden is found. It is shown that the disturbance of structural integrity of microvessel wall, its destruction under the influence of cryptococci is a major factor causing penetration of this infection directly into the substance of the brain…

Disseminated cryptococcosis presenting as biliary obstruction is rare. We report a 35-year-old HIV-negative man who presented with clinical features suggestive of obstructive jaundice, and radiological features suggestive of Klatskin's tumor, but who ultimately was found to have cryptococcal involvement of the liver and biliary tract as part of disseminated cryptococcosis. The patient responded to antifungal therapy.

Cryptococcal skin lesions are found in 10 to 15% of patients with disseminated cryptococcosis. Primary skin inoculation by Cryptococcus neoformans is rare but has been reported. We report the first known case of primary cutaneous cryptococcosis in a lung transplant recipient. Our patient, a 57-year-old man, underwent left single-lung transplantation and presented with a nonhealing ulcer 50 months later. Skin histopathology and culture confirmed C. neoformans. Serum and cerebrospinal fluid cryptococcal antigen tests were negative. The lesion healed after treatment with fluconazole. To date, dis…
