[Congenital dacryocystocele: presentation and treatment].

OBJECTIVE: To report the presentation, complications, and treatment strategies for infants with congenital dacryocystocele. METHOD: We performed a retrospective chart review of all patients presenting with dacryocystoceles to Beijing Children's Hospital between 2007 and 2011. RESULTS: Thirty-one patients (33 eyes) were identified, presenting at a median age of 10 days of life. Twenty-four (72.72%) patients presented with cellulitis or dacryocystitis and required systemic antibiotics. Two (6.45%) patients presented with respiratory compromise. Resolution occurred with conservative treatment f…

IgG4-related orbital disease: a meta-analysis and review.

IgG4-related orbital disease (IgG4-ROD) is a recently described condition that may account for a significant proportion of idiopathic lymphoplasmacytic or sclerotic orbital lesions. This study is the first meta-analysis of published cases and reveals several differences between IgG4-related disease affecting the orbit and that affecting the pancreas. IgG4-ROD affects a slightly younger group of patients, affects men and women approximately equally, is commonly associated with salivary gland lesions, is associated with a relatively higher serum IgG4 and may confer an increased risk of non-Hodgk

[Morphological changes in autoimmune dacryoadenitis associated with thyroid eye disease].

Increase of lacrimal gland (LG) volume was revealed in 1/3 of patients with thyroid eye disease (TED) using visualization methods. Authors show morphological data confirming development of autoimmune process in LG in TED, biopsy samples were studied within 6 hours after exitus. Specific histomorphological characteristics were analysed considering dystrophic and degenerative changes in LG tissue. Quantitative assessment of fibrosis and vascularization extent is determined using fibrosis and vascularization index.

Dacryocystorhinostomy with silicone tubes: evaluation of 253 cases.

Two hundred fifty-three patients with chronic epiphora underwent dacryocystorhinostomy combined with silicone intubation of the lacrimal drainage system. An overall success rate of 91.3% was achieved. Since the tubes were well-tolerated postoperatively, complications were rare and benign. Although our success rate is similar to those reported for similar cases using other techniques, we find that silicone intubation facilitates surgery in cases with excessive intraoperative bleeding or nasal mucosal tears, and simplifies postoperative follow-up.

Nasal oncocytoma causing unilateral epiphora in an elderly patient: the vital role of nasendoscopy.

Oncocytomas are rare glandular tumours, typically found in salivary glands and kidneys. A 76-year-old man presented to an ophthalmology department after 3 months of persistent right-sided epiphora. Lacrimal syringing and a fluorescein dye test demonstrated an obstruction at the level of the nasolacrimal duct (NLD). Owing to the patient's history of dacryocystitis, endoscopic dacryocystorhinostomy was organised. When in theatre however, nasendoscopy identified a polypoid mass at the lateral aspect of the inferior turbinate, obstructing the right NLD opening. Histopathology confirmed an oncocyto…

Endoscopic DCR without stents: clinical guidelines and procedure.

It is a retrospective analysis of cases of endoscopic Dacryocystorhinostomy (DCR) in whom stents were not inserted after completion of surge … 6 to 12 months (average of 8 months). Success was determined by symptomatic relief of epiphora and dacryocystitis, together with the endoscopic findings of free passage of the fluorescein dye that is applied to the conjunctiva to the nasal cavity. In this study, all the patients had both symptomatic and clinical improvements making the success rate 100%. As a c …

Vasculitis of the lacrimal sac wall in Wegener granulomatosis.

A 35-year-old woman with a 4-year history of generalized Wegener granulomatosis (WG) had clinically controlled disease. She was evaluated for a 6-month history of right lacrimal sac mass. On examination, a right chronic dacryocystitis and mucocele were observed. A right external dacryocystorhinostomy was performed. The surgical biopsy specimen from the lacrimal sac showed leukocytoclastic vasculitis with more aggressive damage to the small vessels in the deeper mucosa and focal microhemorrhages. The patient was free of symptoms 1 year after surgery. We believe this is the first report of gener…

Endoscopic laser-assisted dacryocystorhinostomy.

PURPOSE: This study evaluates the efficacy of endoscopic laser-assisted dacryocystorhinostomy (LADCR) in the treatment of epiphora and chronic dacryocystitis. PATIENTS AND METHODS: Treatment results were prospectively reviewed for 17 patients undergoing 19 LADCR procedures of which 12 were primary and 7 were revision cases. Silastic intubation tubing was removed after surgery at a mean of 76.5 days in the primary cases and 184 days in the revision cases. Patients were followed-up for a mean of 15.6 months (range, 5 to 25 months). RESULTS: LADCR was successful in 100% of primary cases and 60%…

[Cysts of the lacrimal tracts in the newborn infant].

At birth, a thin membrane often remains at the distal end of the nasolacrimal duct in the inferior meatus. The rupture is spontaneous during the first month of life. Sometimes this impatency can lead to a symptomatology usually limited to an uni or bilateral dacryocystitis. Exceptionally a cyst can develop in the nasolacrimal passageways. The authors describe three cases of intranasal occurrences, two of which brought about an obstructive dyspnea. Twelve cases have been described in the literature. This affliction predominantly occurs in young girls. Whenever the nasal obstruction is isolated,…

Heterochronic Parabiosis Causes Dacryoadenitis in Young Lacrimal Glands.

Aging is associated with inflammation and oxidative stress in the lacrimal gland (LG). We investigated if heterochronic parabiosis of mice could modulate age-related LG alterations. In both males and females, there were significant increases in total immune infiltration in isochronic aged LGs compared to that in isochronic young LGs. Male heterochronic young LGs were significantly more infiltrated compared to male isochronic young LGs. While both females and males had significant increases in inflammatory and B-cell-related transcripts in isochronic and heterochronic aged LGs compared to level

Herpes Zoster Ophthalmicus With Dacryoadenitis Complicated by Recurrent Orbital Inflammation.

Herpes zoster ophthalmicus represents a zoster infection in the first division of the trigeminal nerve and has potentially serious complications involving the ocular and orbital structures. Herpes zoster ophthalmicus occurs in approximately 10% to 20% of individuals with herpes zoster and can lead to significant morbidity, particularly in patients with multiple comorbidities. The authors present a case of herpes zoster ophthalmicus along with dacryoadenitis in a patient with uncontrolled diabetes and rheumatoid arthritis who was misdiagnosed, had delayed treatment, and experienced recurrence w

Unresolved epiphora in childhood is one of the most frequent issues in paediatric ophthalmology. In this context, connatal nasolacrimal duct … enever CNLDO is suspected. Invasive treatment is recommended in case of complications such as acute dacryocystitis or recurrent ascending infections. Furthermore, CNLDO seems to be amblyogenic. In addition to probing and syringing, temporary intubation of the lacrimal duct is possible. Further transcanalicular approaches include dacryoendoscopy or balloon dacryo …
