A multi-scale fusion model based on multi-phase contrast-enhanced CT for predicting pancreatic cancer resectability.

Purpose.Develop a multi-scale fusion model (MSFM) based on multi-phase contrast-enhanced computed tomography (CECT) to predict pancreatic cancer (PC) resectability, thereby assisting expert decision-making.Methods.This retrospective study enrolled 280 patients with PC from four institutions, which were randomly divided into a training cohort (202 patients) and an independent test cohort (78 patients). Three-phase CECT images (arterial, venous, and delayed phases) were used for modeling. The MSFM comprises two sub-networks: (1) a multi-phase fusion network for extracting cross-phase shared fusi

A Dirichlet Distribution-Based Complex Ensemble Approach for Breast Cancer Classification from Ultrasound Images with Transfer Learning and Multiphase Spaced Repetition Method.

Breast ultrasound is a useful and rapid diagnostic tool for the early detection of breast cancer. Artificial intelligence-supported computer-aided decision systems, which assist expert radiologists and clinicians, provide reliable and rapid results. Deep learning methods and techniques are widely used in the field of health for early diagnosis, abnormality detection, and disease diagnosis. Therefore, in this study, a deep ensemble learning model based on Dirichlet distribution using pre-trained transfer learning models for breast cancer classification from ultrasound images is proposed. In the

Simplified sperm testing devices: a possible tool to overcome lack of accessibility and inconsistency in male factor infertility diagnosis. An opportunity for low- and middle- income countries.

BACKGROUND: Manual semen assessment (MSA) is a key component in a male's fertility assessment. Clinicians rely on it to make diagnostic and treatment decisions. When performed manually, this routine laboratory test is prone to variability due to human intervention which can lead to misdiagnosis and consequently over- or under- treatment. For standardisation, continuous training, quality control (QC) programs and pricy Computer-Assisted Sperm Analysis (CASA) systems have been proposed, yet, without resolving intra- and inter-laboratory variability. In response, promising simplified sperm testin

Increased corticolimbic connectivity in cocaine dependence versus pathological gambling is associated with drug severity and emotion-related impulsivity.

Neural biomarkers for the active detrimental effects of cocaine dependence (CD) are lacking. Direct comparisons of brain connectivity in cocaine-targeted networks between CD and behavioural addictions (i.e. pathological gambling, PG) may be informative. This study therefore contrasted the resting-state functional connectivity networks of 20 individuals with CD, 19 individuals with PG and 21 healthy individuals (controls). Study groups were assessed to rule out psychiatric co-morbidities (except alcohol abuse and nicotine dependence) and current substance use or gambling (except PG). We first e

Risk factors and molecular epidemiology of community-onset extended-spectrum β-lactamase-producing Escherichia coli bacteremia.

PURPOSE: Inadequate empirical therapy for severe infections caused by extended-spectrum β-lactamase-producing Escherichia coli (ESBLEC) is associated with poor outcomes. This study was designed to investigate risk factors for community-onset ESBLEC bacteremia at admission to a tertiary care hospital. MATERIALS AND METHODS: A case-control study was performed that included all episodes of ESBLEC bacteremia in the outpatient department or within 48 hours of admission from January 2005 to March 2009. Data on predisposing factors were collected. The molecular epidemiology of ESBLEC clinical isolat

Functional connectivity in a frontoparietal network involving the dorsal anterior cingulate cortex underlies decisions to accept a hypothesis.

Optimally interpreting our situations and experiences frequently requires comparing the evidence supporting conflicting hypotheses and deciding which to accept. This decision is comparable to an "Aha!" moment reached during insightful problem solving. We used a probabilistic reasoning task to investigate the neural activity underlying these processes. Participants rated the probability that a given focal hypothesis, rather than its alternative, was true. Decisions to accept the focal hypothesis elicited a stronger signal than decisions to reject it in a network involving the dorsal anterior ci

Serotonin selectively modulates reward value in human decision-making.

Establishing a function for the neuromodulator serotonin in human decision-making has proved remarkably difficult because if its complex role in reward and punishment processing. In a novel choice task where actions led concurrently and independently to the stochastic delivery of both money and pain, we studied the impact of decreased brain serotonin induced by acute dietary tryptophan depletion. Depletion selectively impaired both behavioral and neural representations of reward outcome value, and hence the effective exchange rate by which rewards and punishments were compared. This effect was

Is radiological shortening of the ramus a reliable guide to operative management of unilateral fractures of the mandibular condyle?

Several studies have published measurements of the height of the ramus on orthopantomographic (OPT) images of patients with unilateral fractures of the mandibular condyle as a possible quantitative measure for making decisions about treatment. However, we know of no studies that have described the accuracy and validity of such measurements. The aim of the present study was to assess the shortening of the ramus in patients with such fractures, and compare them with differences found in a control group. Seventy-four patients and 74 controls were studied. The height of the ramus on the fractured

Longitudinal task-negative network analyses in preclinical Huntington's disease.

Functional neuroimaging studies have reported task-related brain activation changes in preclinical individuals carrying the Huntington's disease (HD) gene mutation (preHD). Little is known about "task-negative" activity, i.e., patterns of task-related deactivation in preHD, and about the stability of any deactivation changes over the course of the disease. Here, we explored task-related deactivation and functional connectivity of "task-negative" networks (TNNs) in preHD followed over a time period of 2 years. Thirteen far-from-onset preHD (mean time to estimated motor onset = 19.5 years) and t

Task-related BOLD responses and resting-state functional connectivity during physiological clamping of end-tidal CO(2).

Carbon dioxide (CO(2)), a potent vasodilator, is known to have a significant impact on the blood-oxygen level dependent (BOLD) signal. With the growing interest in studying synchronized BOLD fluctuations during the resting state, the extent to which the apparent synchrony is due to variations in the end-tidal pressure of CO(2) (PETCO(2)) is an important consideration. CO(2)-related fluctuations in BOLD signal may also represent a potential confound when studying task-related responses, especially if breathing depth and rate are affected by the task. While previous studies of the above issues h

Deconstructing risk: separable encoding of variance and skewness in the brain.

Risky choice entails a need to appraise all possible outcomes and integrate this information with individual risk preference. Risk is frequently quantified solely by statistical variance of outcomes, but here we provide evidence that individuals' choice behaviour is sensitive to both dispersion (variance) and asymmetry (skewness) of outcomes. Using a novel behavioural paradigm in humans, we independently manipulated these 'summary statistics' while scanning subjects with fMRI. We show that a behavioural sensitivity to variance and skewness is mirrored in neuroanatomically dissociable represent

The temptation of suicide: striatal gray matter, discounting of delayed rewards, and suicide attempts in late-life depression.

BACKGROUND: Converging evidence implicates basal ganglia alterations in impulsivity and suicidal behavior. For example, D2/D3 agonists and subthalamic nucleus stimulation in Parkinson's disease (PD) trigger impulse control disorders and possibly suicidal behavior. Furthermore, suicidal behavior has been associated with structural basal ganglia abnormalities. Finally, low-lethality, unplanned suicide attempts are associated with increased discounting of delayed rewards, a behavior dependent upon the striatum. Thus, we tested whether, in late-life depression, changes in the basal ganglia were as

Neural correlates of real-world route learning.

Classical theories of spatial microgenesis (Siegel and White, 1975) posit that information about landmarks and the paths between them is acquired prior to the establishment of more holistic survey-level representations. To test this idea, we examined the neural and behavioral correlates of landmark and path encoding during a real-world route learning episode. Subjects were taught a novel 3 km route around the University of Pennsylvania campus and then brought to the laboratory where they performed a recognition task that required them to discriminate between on-route and off-route buildings. Greater activation in left hemisphere language-related regions during simple judgment tasks among substance-dependent patients in treatment for alcoholism.

BACKGROUND: Alcoholism is often associated with impaired emotional control. Alcoholics have also been found to have deficits in frontal lobe executive functions. Recent functional imaging studies have suggested that alcoholics show greater activation than nonalcoholics in circuits involving frontal lobes, as well as more posterior brain regions, when engaged in executive-type tasks. In this study, we compared brain activations of alcohol-dependent patients and healthy nonalcoholics while they performed 2 simple judgment tasks designed to activate frontal circuits involved in a basic form of de

AI-integrated human-relevant alternatives to animal experimentation in drug development: Toward a predictive translational ecosystem, from in vitro and in silico models to clinical and real-world evidence.

Animal experimentation has long been associated with biomedical research and drug development. However, ethical concerns, limited translatability to humans and high clinical attrition rates (> 90%) necessitate a transition toward human-relevant approaches. Although diverse in vitro, in silico and artificial intelligence (AI)-enabled methodologies have emerged, their integration into a unified, regulatory-acceptable framework remains limited. This review proposes an AI-centred, multi-tier decision-support framework, to enable a phased shift toward animal-reduced and, ultimately, animal-free dru
