Diagnosing and treating ventricular tachycardia.

The misdiagnosis of wide complex tachycardias often leads to improper treatment and adverse patient outcomes. This article presents a thorough review of the validated electrocardiographic criteria useful in the diagnosis of ventricular tachycardia versus supraventricular tachycardia with aberrancy. Current information on bedside monitoring, pharmacologic and nonpharmacologic treatment, psychological impact, and discharge planning is discussed.

Risk stratification of children being evaluated for intussusception.

CONTEXT: Intussusception is the most common cause of intestinal obstruction in young children, and delayed diagnosis may lead to bowel perforation. OBJECTIVE: To determine predictive clinical criteria and develop a decision tree to risk-stratify children with possible intussusception. DESIGN/METHODS: This is a prospective observational cohort study of children aged 1 month to 6 years who presented with possible intussusception. A data-collection form was completed before knowledge of any advanced imaging. Univariate analysis was performed, and decision trees were developed using recursive pa…

Enhancing TreeMMoSys with a high-precision strain gauge to measure the wind-induced response of trees down to the ground.

Measuring tree response to wind loads is fundamental for the process-based analysis of wind-tree interactions. Comprehensive knowledge of wind-tree interactions enables the further development of decision support tools available for estimating the probability of wind damage to trees. The assessment of critical wind loads that lead to damage is particularly important. This paper describes the inexpensive Tree Strain Sensor (TSS) suitable for precisely measuring the response of tree parts to external loads such as pulling tests and natural wind loading. It is an addition to the recently develope

Ensembling rules in automatic analysis of pressure on plantar surface in children with pes planovalgus.

PURPOSE: This paper presents a method of ensembling rules obtained through induction of several basic types of decision trees. MATERIAL AND METHODS: The proposed method uses rules generated by means of well-known decision trees: CART, CHAID, exhaustive CHAID and C4.5. The method was tested on data describing pressure distribution under foot during gait in children with pes planovalgus (PV) and typical foot. Children with pes planovalgus underwent surgical intervention and were re-examined. Overall, 316 gait cycles have been used in analysis. RESULTS: The obtained results consist of a set of …

Baseline predictors of treatment outcome in Internet-based alcohol interventions: a recursive partitioning analysis alongside a randomized trial.

BACKGROUND: Internet-based interventions are seen as attractive for harmful users of alcohol and lead to desirable clinical outcomes. Some participants will however not achieve the desired results. In this study, harmful users of alcohol have been partitioned in subgroups with low, intermediate or high probability of positive treatment outcome, using recursive partitioning classification tree analysis. METHODS: Data were obtained from a randomized controlled trial assessing the effectiveness of two Internet-based alcohol interventions. The main outcome variable was treatment response, a dicho

Pericardial diseases in patients with hypothyroidism.

Hypothyroidism is a well-known cause of pericardial effusion (with an incidence of 3%-37%) and can cause cardiac tamponade in severe cases. In this review, we present the current knowledge on the epidemiology of hypothyroid-induced pericardial diseases, the mechanism through which low thyroid hormone levels affect the pericardium, the associated clinical manifestations, diagnostic tests and management options. Hypothyroidism causes pericardial effusion through increased permeability of the epicardial vessels and decreased lymphatic drainage of albumin, resulting in accumulation of fluid in the

Simple and complex renal cysts in adults: Classification system for renal cystic masses.

Ultrasound and slice imaging are currently the key modalities for diagnosing simple renal cysts. Benign simple renal cysts are defined by a set of strict criteria. Any cystic lesions that do not meet these criteria should be classified as "atypical" or "complex" and considered as suspicious. For over three decades, the Bosniak classification has been the central tool for diagnosing renal cystic masses larger than 1 cm. The Bosniak classification is based upon the results of a well-established pre- and post-contrast computed tomography (CT) protocol. Lesions that are difficult to classify or th

Removing Carious Tissue: Why and How?

Caries is no longer seen as an infectious disease, and the aim of treating carious lesions is to control their activity, not to remove the lesion itself. Such control can be implemented by sealing off the lesion from the environment, with sealed bacteria being deprived from carbohydrates and thus inactivated. For cavitated lesions, controlling them usually involves the placement of restorations to rebuild the cleansability of the surface. In this case, dental practitioners have traditionally removed carious tissues prior to the restoration. This has historically been for a number of reasons, w

Acute alcoholic hepatitis: a literature review and proposal of treatment.

Severe acute alcoholic hepatitis (AAH) can lead to a clinical picture with a six-month mortality rate in more than 70% of cases. This clinical picture is characterized by: jaundice with a duration of less than three months, jaundice at the first failure event, serum bilirubin greater than 5 mg/dL, ratio AST/ALT>2/1, AST less than 500 IU/L, ALT<300 IU/L, neutrophil leukocytosis and a GGT increase. In addition, encephalopathy, fever, asthenia and coagulopathy may be present. Its onset may also be characterized by portal-hypertension-related complications, particularly bleeding and hepato-renal s

An update on the management of acute esophageal variceal bleeding.

The mortality rate in acute variceal haemorrhage remains high (around 15%). Treatment is based on the combined use of vasoactive drugs, endoscopic band ligation, and prophylactic antibiotics. Effective resuscitation (haemostasis, volume management) is essential to prevent complications. Treatment failure is best managed by transjugular intrahepatic portosystemic shunt (TIPS). Balloon tamponade or specifically designed covered oesophageal stents can be used as a bridge to definitive therapy in unstable patients. Early, pre-emptive TIPS should be the first choice in patients at high risk of trea

[Diagnosis in Polycystic Kidney Disease].

The diagnosis of ADPKD should be based on a thorough assessment of the family history, radiological study and when indicated, genetic study. The anamnestic definition of 'negative family history' should be accepted only after a full clinical-instrumental assessment of close relatives of the subject . Ultrasound is the diagnostic modality of choice in at-risk patients with a positive family history of ADPKD. In some clinical scenarios (screening kidney donation, family planning and others) misdiagnosis based on morphological criteria may be too high. For this reason, it is not recommended to us

[Acid-base homeostasis: metabolic acidosis and metabolic alkalosis].

Acid-base homeostasis ensured by the kidneys, which maintain the equilibrium between proton generation by cellular metabolism and proton excretion in urine. This requirement is lifesaving because of the protons' ability to bind to anionic proteins in the extracellular space, modifying their structure and functions. The kidneys also regenerate bicarbonates. The kidney is not the sole organ in charge of maintaining blood pH in a very narrow range; lungs are also involved since they allow a large amount of volatile acid generated by cellular respiration to be eliminated.

[Practical conduct for patients with a lonely hypoxemia at rest or during exercise].

INTRODUCTION: Hypoxemia is a decrease in blood oxygen partial pressure. This work aims at presenting a practical conduct for patients with a lonely hypoxemia at rest or during exercise, defined by the absence of dyspnea at rest, obvious clinical or radiographic abnormality. STATE OF THE ART: Diagnostic tools available to the clinician are clinical examination, CT scan, echocardiography, hyperoxia test, trans cranial ultrasound and lung scintigraphy. This work proposes a practical diagnostic approach, with a main role of chest CT. PERSPECTIVES: Work is underway to determine more precisely the

[Portal hypertension in pediatrics: II: Hemorrhagic complications].

Bleeding from esophageal varices is the most severe complication of portal hypertension, and should be managed in specially trained centers. Vasoactive drugs, mainly octreotide, plus endoscopic treatment are able to control bleeding in 90% of the cases. Rescue treatments like TIPS and surgery should be immediately available for those who do not stop bleeding or have varices difficult to manage.

Sporadic primary hyperparathyroidism.

Primary hyperparathyroidism (HPT1) is a common endocrine disorder, which is asymptomatic in 80% of cases. The diagnosis is ordinarily easily made, based on an inappropriately elevated parathormone level (PTH) in the face of hypercalcemia. In 85% of cases, HPT1 is due to hormone secretion from a single parathyroid gland (uniglandular disease) and the remaining patients have multiglandular disease. The best localization study is MIBI scintigraphy (methoxy isobutyl isonitrile) coupled with the results of a neck ultrasound exam (sensitivity >95%). Other investigations are reserved for patients wit

Current management of advanced and metastatic renal cell carcinoma.

INTRODUCTION: Unresectable renal cell carcinoma (RCC) is a technically incurable condition. Historically, RCC is resistant to chemotherapy and radiotherapy. Cytokine therapy was until recently considered the mainstay of treatment. However, responses are modest. Improvement in the understanding of the biology of RCC, particularly the hereditary types, is providing the basis for novel therapeutic targets. Our aim was to review the clinical utility of various systemic agents and surgery in the management of advanced RCC and suggest practice guidelines in the light of current literature. MATERIAL

Recommendations for the diagnosis and treatment of latent and active tuberculosis in inflammatory joint diseases candidates for therapy with tumor necrosis factor alpha inhibitors: March 2008 update.

The Portuguese Society of Rheumatology and the Portuguese Society of Pulmonology have updated the guidelines for the diagnosis and treatment of latent tuberculosis infection (LTBI) and active tuberculosis (ATB) in patients with inflammatory joint diseases (IJD) that are candidates to therapy with tumour necrosis factor alpha (TNFalpha) antagonists. In order to reduce the risk of tuberculosis (TB) reactivation and the incidence of new infections, TB screening is recommended to be done as soon as possible, ideally at the moment of IJD diagnosis, and patient assessment repeated before starting an

[Torticollis in children: diagnostic approach].

Torticollis is an abnormal head posture. Clinical presentation includes ipsilateral tilt and controlateral rotation, and translation. Aetiologies are various including muscular entities, traumatic, inflammatory, infectious, tumour and non-muscular entities ocular, ORL or digestive. Majorities are minor and generally, a complete physical examination is sufficient to eliminate serious entities. Concretely, it is important to separate acute and chronic form. Traumatic torticollis justifies a systematic standard radiograph and then 15 days after dynamic evaluation. Septic torticollis evokes otolar

[Congenital coronary artery anomalies with origin in the contralateral sinus of Valsalva: which approach should we take?].

Recent years have witnessed a change in our perception of congenital coronary artery anomalies. From being regarded as simple coronary angiographic observations of little clinical significance, they have come to be seen as potential causes of sudden death in young people and of myocardial ischemia in adults. Diagnostic difficulties, a lack of knowledge about the mechanisms through which they produce myocardial ischemia, and their highly variable incidence have made congenital coronary artery anomalies of great interest to cardiologists. This article focuses on the group of coronary artery anom

[Acute dysphagia of oncological origin. Therapeutic management].

Dysphagia is one of the most frequent syndromes in patients with tumours of the head and neck, and the oesophagus. This can be the initial symptom or, more frequently, related to the oncological treatment. We review the most important therapeutic and physio-pathological aspects of acute dysphagia of oncological origin. Deglutition is a complex process in which numerous muscular-skeletal structures intervene under the neurological control of different cranial nerves. The complex neuro-muscular coordination needed for a correct deglutition can be affected by numerous situations, both from the ef
