External skin excision in the sebaceous nose and supratip deformity.

Some sebaceous noses cannot be properly reduced in size because of redundant skin. After standard rhinoplasty some noses develop a supratip deformity that recurs even after subcutaneous removal of the scar tissue. These noses can be corrected only by wedge-shaped skin excision. Most patients much prefer a pleasantly shaped nose, even at the cost of a midline scar on the nose. Most surgeons are hesitant to add scars to the face. However, the vast experience with wounds following accidents, tumor excisions, or corrections of malformations has shown that generally scar formation on the nose is in

Assessment of percutaneous fasciotomy in the management of Dupuytren's contracture.

This prospective study of the treatment of Dupuytren's Contracture in 78 hands has been designed to investigate the role of subcutaneous fasciotomy. The results suggest that in hands where the contracture is predominantly at the metacarpophalangeal joint, then percutaneous fasciotomy is of value.

Aplasia cutis congenita. A follow-up evaluation after 25 years.

Aplasia cutis congenita is a rare disease with less than 300 cases reported in the literature. A patient with extensive involvement of the skin was observed for 25 years. During adolescence, a reactive depression related to the residual cicatricial alopecia on the left side of the scalp developed. The psychiatric distress disappeared after corrective surgery was performed. It is recommended that patients with this disorder be regularly observed to select cases appropriate for early plastic surgery.

Evidence-based management of vitiligo: summary of a Cochrane systematic review.

Vitiligo affects around 1% of the world's population. Despite it being relatively common, there is still no effective treatment. The objective of this study was to update the Cochrane systematic review of randomized clinical trials (RCTs) to evaluate the efficacy of treatments for vitiligo. We carried out searches of a range of databases to October 2013 for RCTs of interventions for vitiligo regardless of language or publication status. At least two reviewers independently assessed study eligibility and methodological quality and extracted data using data extraction forms approved by the Cochr

Tumescent anesthesia for dermatologic surgery. Cosmetic and noncosmetic procedures.

BACKGROUND: Tumescent anesthesia was originally described by Klein and Lillis for liposuction surgery. Its extension to many other interventions in dermatologic surgery (cosmetic and noncosmetic) has provided a significant contribution in the performance of safe, ambulatory, and simplified procedures. OBJECTIVE: To demonstrate the validity of the technique in various procedures. METHODS: Over the course of 1 year all large reconstruction or cosmetic surgery was performed using tumescent anesthesia. In a total of 242 cases performed, the advantages and disadvantages of the procedure were anal

The dermatology out-patient service: a study of out-patient referrals in a Scottish population.

A comprehensive survey of the dermatology out-patient service, including private practice, was undertaken by the medical staff of the Department of Dermatology, in Edinburgh. The unit serves a population of 1,171,000 in the south-east region of Scotland. One thousand two hundred and thirty-two new patients and 1587 follow-up patients were seen in 1 month. Ninety-five per cent of new referrals were from general practitioners and the remaining 5% were from hospital doctors. Benign tumours (19.3%), viral warts (16.9%) and dermatitis/eczema (16.7%) accounted for over half of all new consultations.

Robust surgical approach for cutaneous neurofibroma in neurofibromatosis type 1.

BACKGROUND: Cutaneous neurofibromas (cNF) are physically disfiguring, painful, and cause extensive psychologic harm in patients with neurofibromatosis type 1 (NF1). There is currently no effective medical treatment and surgical procedures are inaccessible to most NF1 patients globally. OBJECTIVE: While research is underway to find an effective medical treatment for cNF, there is an urgent need to develop surgical approach that is accessible to all NF1 patients in the world with the skill set and equipment found in most general medical office settings. Here, we present a robust surgical approa

Treatment of locally metastatic melanoma: a novel approach.

We report a case of an 83-year-old female with locally metastatic melanoma treated with imiquimod and tazarotene. The patient originally presented to our dermatology clinic with local metastases of malignant melanoma after having undergone multiple surgical procedures and adjuvant radiation therapy for disease recurrence. At this juncture, she refused further surgical management but was interested in topical therapy. A 4-week course of topical imiquimod therapy was initiated. As no clinical response was noted at the end of the treatment period, tazarotene cream was introduced. The patient expe

Systematic review of non-surgical etiologies of nasal necrosis: evidence from case reports and case series.

PURPOSE: Nasal necrosis is a rare but potentially devastating manifestation of diverse conditions, including fulminant infection, immune-mediated vasculopathy, drug toxicity, neoplasia and device-related pressure injury. Surgical intervention is often prioritized; however, a subset of patients is managed non-surgically and evidence-based guidance remains limited. The objective is to systematically synthesize non-surgical etiologies, clinical patterns, diagnostic workup, and management strategies reported in cases of nasal necrosis. METHODS: A systematic search of PubMed, Scopus and ScienceDir

Nonsurgical Endodontic Management of an Odontogenic Cutaneous Sinus Tract in a Child: A Case Report.

Odontogenic cutaneous sinus tracts represent an uncommon clinical manifestation of chronic dental infection and are frequently misdiagnosed due to the absence of dental symptoms and their resemblance to dermatologic lesions. This case report describes the nonsurgical endodontic management of a cutaneous sinus tract of dental origin in a 13-year-old patient. The patient presented with a persistent extraoral lesion in the mandibular region, initially evaluated by non-dental specialists. Clinical and radiographic examination revealed a necrotic mandibular first molar associated with a periapical

Cutaneous laser surgery for secondary burn reconstruction: Cost benefit analysis.

Thermal disease presents a major burden to individual patient morbidity, healthcare cost as well as to over all economy. Burns also also represent a significant per-patient utlilisation of finite healthcare resources. Secondary complications in these patients, such as multiple drug resistant organisms, may have a devastating effect. Laser surgery has recently come of age as an optimal tool in the secondary reconstruction of burn injury, that is able to simultaneously address significant sheet scar tightness, hypertrophic, atrophic, and keloid complications, pruritus, microstomia, ectropion, sk

Single free anterolateral thigh flap for simultaneous reconstruction of composite hypopharyngeal and external neck skin defect after head and neck cancer ablation.

Single flap for complex hypopharyngoesophageal and anterior neck skin defect reconstruction is still a challenge for reconstructive surgeons. Herein, we present five patients, with advanced hypopharyngeal cancer and anterior neck skin invasion, which received a single anterolateral thigh (ALT) fasciocutaneous flap for composite inner pharyngeal and outer skin defect reconstruction after wide composite resection. Two ALT flaps were divided into two distinct paddles supplied by two or more separate perforators, one part for reconstructing the inner pharyngeal defect and another for neck skin cov

Wide topical negative pressure wound dressing treatment for patients undergoing abdominal dermolipectomy following massive weight loss.

Postbariatric plastic surgery is considered to be a high-risk procedure, which entails such frequent minor complications as postoperative seroma, bleeding and wound dehiscence. These occur with a high incidence, especially, following postbariatric abdominal dermolipectomy. In order to reduce these complication rates, a new type of dressing with wide abdominal topical negative pressure (TNP) application was applied. We performed abdominal dermolipectomy in 23 obese patients. The average body mass index was 32.8 kg/m(2), and the median age of the patients was 42.9 years. Ten patients received co

Meningococcal septicaemia and plastic surgery--a strategy for management.

Meningococcal septicaemia is a severe life threatening illness that is part of the spectrum of meningococcal disease. It is of particular concern to the plastic and reconstructive surgeon because as the medical and intensive care of these patients improves, more are surviving the acute phase of the septicaemia and suffering from the associated complications such as skin necrosis and extremity loss. Recently there have been several reports in the literature concerning the management of skin infarction with both skin grafting and free flap surgery. This paper will provide an update on the propos

Rapid Response to Certolizumab Pegol in Hidradenitis Suppurativa: A Case Report.

INTRODUCTION: Hidradenitis suppurativa (HS) is a chronic inflammatory skin disease of hair follicles which usually affects patients' quality of life negatively. Although there are various therapeutic approaches including topical and systemic antibiotics, retinoids, biological agents, and also surgical modalities, there is no curative treatment option and clinical management of HS is still challenging. CASE PRESENTATION: Herein, we present a 43 year-old man with HS who had used different treatments in 15 years. The patient showed improvement initially with adalimumab, then exhibited secondary

Reconstruction of Cutaneous Nasal Defects.

Mohs micrographic surgery has become the standard of care for the treatment of cutaneous malignancies. Reconstructing cutaneous defects of the nose can be challenging, as form and function must be respected to the greatest extent possible. A wide range of reconstructive techniques are used. Secondary intent, primary closure, skin grafts, local flaps, and the interpolated workhorse flaps represent the spectrum of options, each with specific advantages and disadvantages. Vigilant postoperative care, including judicious use of adjunctive procedures, can improve outcomes. A subunit approach to rec
