Measles control in developing and developed countries: the case for a two-dose policy.

Despite major reductions in the incidence of measles and its complications, measles control with a single dose of the currently used. Schwarz strain vaccine has failed to eradicate the disease in the developed countries. In developing countries an enormous toll of measles deaths and disability continues, despite considerable efforts and increasing immunization coverage. Empirical evidence from a number of countries suggests that a two-dose measles vaccination programme, by improving individual protection and heard immunity can make a major contribution to measles control and elimination of loc…

Cost and efficiency considerations in community-based trials of vitamin A in developing countries.

Recent investigations conducted in Indonesia have identified an important role for adequate vitamin A nutriture in preventing mortality in young children. A randomized, community-based trial of vitamin A supplementation in northern Sumatra demonstrated a 34 per cent reduction in mortality among pre-school aged children who received twice yearly vitamin A supplementation. These results have stimulated renewed interest in vitamin A as a potentially important mediator of improved child survival. Cost and efficiency considerations are important in these types of trials but must be balanced with lo

The role of dietary aflatoxin in the genesis of hepatocellular cancer in developing countries.

Impaired activity of the liver microsomal mixed-function-oxidase (MFO) system is characteristic of protein malnutrition. It explains the accumulation of aflatoxin (AFB1) in livers of kwashiorkor victims, whose staple foods are usually heavily contaminated with this fungal toxin. Dietary rehabilitation of such children with high-protein foods not only increases the activity of the liver MFO system but also stimulates DNA replication and rapid regeneration of liver cells. Under such circumstances highly reactive metabolites of AFB1, such as the AFB1-epoxide, can produce malignant transformation

Clinical survey techniques to estimate prevalence and annual incidence of poliomyelitis in developing countries.

Recent epidemiological data suggest that poliomyelitis is a more important public health problem in developing countries than was originally suspected. Because of the availability of a cheap, effective, but labile oral vaccine, it is important that countries determine the incidence of poliomyelitis in order to determine the necessity and speed with which a vaccination programme should be introduced.Sequelae of poliomyelitis are distinctive and can be used to study the incidence of residual paralysis in children above the age of 5 years. Prevalence data for lameness due to poliomyelitis can the…

Developing standards of care for HIV prevention research in developing countries -- a case study of 10 research centers in Eastern and Southern Africa.

Standards of care provided to volunteers in HIV prevention research in developing countries are evolving. Inconsistency in standards, particularly within a research network highlights the need to balance volunteers' health and wellness with the efficient conduct of research. Ten research centers (RC's) in East and Southern Africa affiliated with the International AIDS Vaccine Initiative (IAVI) were studied using a mixed methods approach to understand variations, similarities and gaps in services provided, recipients of services, referral systems, and barriers to referral uptake. These data wer…

Developing National Priority Assistive Products Lists: experiences and lessons from six Sub-Saharan African countries.

PURPOSE: This study examines the experiences and lessons learned from six Sub-Saharan African countries in developing National Priority Assistive Products Lists (APLs) between 2020 and 2024, aiming to provide insights for improving assistive technology (AT) access in low- and middle-income countries (LMICs). MATERIALS AND METHODS: The study employed qualitative descriptive methodology with a multi-faceted approach, including a comprehensive desktop review of documentation and publications, followed by semi-structured interviews and information review with key stakeholders (n = 9) from all six

Heavy metal contamination in wastewater-irrigated vegetables: assessing food safety challenges in developing Asian countries.

Vegetables are crucial for human nutrition, providing essential micronutrients and beneficial compounds. Heavy metal contamination of vegetables irrigated with wastewater poses a significant public health risk in developing Asian countries. This review analyses recent research on heavy metal accumulation in vegetables across India, Bangladesh, Pakistan, and China. Studies consistently report concerning levels of cadmium, lead, chromium, arsenic, nickel, and mercury in vegetables, often exceeding international safety standards. Leafy vegetables consistently show higher heavy metal accumulation

Urinary detection of high-risk HPV DNA to enhance cervical cancer screening in developing countries.

To increase cervical cancer screening capacity and participation, we evaluated the performance of the newly developed high-risk human papillomavirus (hrHPV) ReadyMix qPCR Kit for detecting hrHPV in urine samples while genotyping for HPV-16, HPV-18, and HPV-52. A total of 876 samples were used to assess the performance of the hrHPV ReadyMix qPCR Kit in detecting hrHPV in standard cervical swab samples compared with that of the Roche Cobas 6800 HPV system. hrHPV detection in urine was compared with that in corresponding paired cervical swab samples. The sensitivity of the hrHPV ReadyMix qPCR Kit

Infectious causes of fever of unknown origin in developing countries: An international ID-IRI study.

BACKGROUND: Fever of unknown origin (FUO) in developing countries is an important dilemma and further research is needed to elucidate the infectious causes of FUO. METHODS: A multi-center study for infectious causes of FUO in lower middle-income countries (LMIC) and low-income countries (LIC) was conducted between January 1, 2018 and January 1, 2023. In total, 15 participating centers from seven different countries provided the data, which were collected through the Infectious Diseases-International Research Initiative platform. Only adult patients with confirmed infection as the cause of FUO…

Paroxysmal nocturnal haemoglobinuria, diagnosis and haematological findings, first report from Iran, model for developing countries.

Since paroxysmal nocturnal haemoglobinuria (PNH) was first described in 1881, the diagnosis and follow-up patients diagnosed with the illness has remained an area of concern, with several different techniques of varying sensitivity having been described in the literature for both the diagnosis and monitoring treatment of the disease. PNH is a rare and life-threatening disease that manifests symptoms of haemolytic anaemia. Hence, a quick and reliable technique for precise diagnosis would be crucial. PNH patients who have previously been diagnosed with aplastic anaemia or myelodysplastic syndrom

Factors Affecting Neonatal Hearing Screening Follow-up in Developing Countries: One Insitution Prospective Pilot Study.

OBJECTIVE: To detect factors related with loss to follow-up (LTF) in neonatal hearing screening (NHS) program of one institution in a developing country. METHODS: A prospective study was planned based on the data collected in a pilot study conducted a year before in the same institution. In this pilot study, hearing screening was performed before hospital discharge for every infant (1217 newborns) in six months period. Total referral rate was 19.1% (223/1217). Loss to follow-up (LTF) was 38.1% (85/223). Telephonic interviews were done with 50 parents who had not come with their child to the s

Precision medicine and its implementation in patients with NTRK fusion genes: perspective from developing countries.

Precision oncology is the field that places emphasis on the diagnosis and treatment of tumors that harbor specific genomic alterations susceptible to inhibition or modulation. Although most alterations are only present in a minority of patients, a substantial effect on survival can be observed in this subgroup. Mass genome sequencing has led to the identification of a specific driver in the translocations of the tropomyosin receptor kinase family (NTRK) in a subset of rare tumors both in children and in adults, and to the development and investigation of Larotrectinib. This medication was gran
