OBJECTIVE: To compare a solution of 3% dextran-60 (D60) in Ringer's lactate (RL) with RL alone as maintenance fluids for abdominal aortic surgery.
DESIGN: Randomized control trial of 20 consecutive patients undergoing elective aortic reconstructive surgery.
SETTING: A surgical ICU in a university hospital.
PATIENTS: Consecutive patients, mean age 64 yr. Five patients had abdominal aneurysm, 12 had aortic obstruction disease, and three had aortic renal bypass surgery. These patients were followed for 1 month.

Iloprost, a stable carbaprostacyclin analog, was infused (up to 2-4 ng/kg/min) for 12 h daily on 5 consecutive days into the forearm vein of 13 patients with peripheral arterial occlusive disease (PAOD) of legs (stages IIb-III). All vasodilatory and antiplatelet drugs were stopped three weeks earlier. For comparison, dextran was infused in a randomized, crossover, double-blind manner with an average interval of 3 months. Glucose has several disadvantages such as low pH, high osmolality and hyperglycemia. Rapid glucose absorption contributes to hyperlipidemia, obesity and ultrafiltration failure in peritoneal dialysis patients. During a 3-yr period, 31 adult victims of severe traumatic shock were enrolled in a prospective randomized investigation of the relative effectiveness of dextran 70 vs. Ringer's acetate to treat shock and protect against trauma-induced adult respiratory distress syndrome (ARDS). Twenty female pigs underwent laparotomy during which transection of both uterine horns and wedge resection of both ovaries were performed with a standard technique. After rapid gelatin infusion in many cases a release of relatively small amounts of histamine was observed. Clinical symptoms occured in 5 cases in correspondence to the increased histamine levels, but were relatively harmless. The application of dextran 60 induced anaphylactoid reactions in 2 persons, with death in one case. Plasma histamine level were normal in both cases. Histamine release probably is not the cause of dextrn induced anaphylactoid reactions.
