Estimating the annual cost burden of diabetic peripheral neuropathy in the United States.

PURPOSE: Diabetic peripheral neuropathy (DPN) is one of the leading complications of diabetes. Given the significant growth in prevalence of diabetes and therefore DPN in the United States in recent years, a quantitative analysis is needed to estimate the annual cost burden of DPN. METHODS: A cost-of-illness burden model was created to estimate the cost burden on DPN in the United States. A review of the literature was undertaken to identify the most appropriate epidemiologic and economic inputs. One-way sensitivity analyses were conducted for every model input parameter to show the potential

The Association Between Detailed Obesity Measurements and Peripheral Neuropathy in Persons With Diabetes.

BACKGROUND: Obesity increases the risk of diabetic peripheral neuropathy (DPN). However, past studies have typically assessed obesity using anthropometric measurements. Our primary aim determined associations between detailed obesity measurements, DPN, and painful DPN (pDPN). Our secondary aim compared the discriminatory capabilities of these measurements. METHODS: We performed a cross-sectional study of persons with diabetes. Obesity was assessed using anthropometrics, bioelectrical impedance (BIA), abdominal MRIs (aMRI), and/or dual x-ray absorptiometry (DEXA). Obesity measurements were cat

Non-Inferiority Study of Two Capsaicin Formulations for Painful Diabetic Neuropathy.

While capsaicin topical formulations are established treatments, conventional creams using this substance have limited application due to handling-related adverse events. This study aimed to demonstrate that the efficacy of a novel 0.75 mg/g capsaicin roll-on solution is non-inferior to the approved 0.75 mg/g cream in patients with painful diabetic neuropathy (PDN). In total, 160 patients were randomized to receive either the roll-on or the cream, applied four times daily for 8 weeks, followed by a 4-week washout period and crossover to the alternate treatment. The primary endpoint was pain in

Molecular mechanisms of enteric neuropathies in high-fat diet feeding and diabetes.

BACKGROUND: Obesity and diabetes are associated with altered gastrointestinal function and with the development of abdominal pain, nausea, diarrhea, and constipation among other symptoms. The enteric nervous system (ENS) regulates gastrointestinal motility. Enteric neuropathies defined as damage or loss of enteric neurons can lead to motility disorders. PURPOSE: Here, we review the molecular mechanisms that drive enteric neurodegeneration in diabetes and obesity, including signaling pathways leading to neuronal cell death, oxidative stress, and microbiota alteration. We also highlight potenti

Diabetic neuropathy results in vasomotor dysfunction of medium sized peripheral arteries.

BACKGROUND: The effect of the sympathetic nervous system on peripheral arteries causes vasoconstriction when smooth muscle cells in the walls of blood vessels contract, which leads to narrowing of arteries and reduction of the blood flow. AIM: To compare sympathetic vasomotor activation of the brachial arteries in healthy subjects and patients with painful diabetic neuropathy; and therefore, to assess whether there is significant vasomotor dysfunction of medium sized arteries in diabetic neuropathy. METHODS: The study included 41 diabetic neuropathy patients and 41 healthy controls. Baseline

[Chinese expert consensus on diagnosis and treatment of diabetic optic neuropathy (2022)].

Diabetic neuropathy is one of the common chronic complications of type 1 and 2 diabetes, involving the motor, sensory, and autonomic nerves of the central nervous system as well as the peripheral nervous system. When the optic nerve is involved, it is called diabetic optic neuropathy (DON). DON may or may not be associated with diabetic retinopathy. At present, ophthalmologists at home and abroad have insufficient understanding of DON, and early intervention will bring more benefits to the recovery of patients' visual function. Therefore, the Neuro-ophthalmology Group of Ophthalmology Branch o

The impact of sensory neuropathy and inflammation on epithelial wound healing in diabetic corneas.

Diabetic peripheral neuropathy (DPN) is the most common complication of diabetes, with several underlying pathophysiological mechanisms, some of which are still uncertain. The cornea is an avascular tissue and sensitive to hyperglycemia, resulting in several diabetic corneal complications including delayed epithelial wound healing, recurrent erosions, neuropathy, loss of sensitivity, and tear film changes. The manifestation of DPN in the cornea is referred to as diabetic neurotrophic keratopathy (DNK). Recent studies have revealed that disturbed epithelial-neural-immune cell interactions are a

Diagnostic Tools, Biomarkers, and Treatments in Diabetic polyneuropathy and Cardiovascular Autonomic Neuropathy.

The various manifestations of diabetic neuropathy, including distal symmetric sensorimotor polyneuropathy (DSPN) and cardiovascular autonomic neuropathy (CAN), are among the most prevalent chronic complications of diabetes. Major clinical complications of diabetic neuropathies, such as neuropathic pain, chronic foot ulcers, and orthostatic hypotension, are associated with considerable morbidity, increased mortality, and diminished quality of life. Despite the substantial individual and socioeconomic burden, the strategies to diagnose and treat diabetic neuropathies remain insufficient. This re…

Diabetes-Induced Cardiac Autonomic Neuropathy: Impact on Heart Function and Prognosis.

Cardiovascular autonomic neuropathy (CAN) is a severe complication of the advance stage of diabetes. More than 50% of diabetic patients diagnosed with peripheral neuropathy will have CAN, with clinical manifestations including tachycardia, severe orthostatic hypotension, syncope, and physical exercise intolerance. Since the prevalence of diabetes is increasing, a concomitant increase in CAN is expected and will reduce quality of life and increase mortality. Autonomic dysfunction is associated with reduced baroreflex sensitivity and impairment of sympathetic and parasympathetic modulation. Vari

Inflammatory Mechanisms in the Pathophysiology of Diabetic Peripheral Neuropathy (DN)-New Aspects.

The pathogenesis of diabetic neuropathy is complex, and various pathogenic pathways have been proposed. A better understanding of the pathophysiology is warranted for developing novel therapeutic strategies. Here, we summarize recent evidence from experiments using animal models of type 1 and type 2 diabetes showing that low-grade intraneural inflammation is a facet of diabetic neuropathy. Our experimental data suggest that these mild inflammatory processes are a likely common terminal pathway in diabetic neuropathy associated with the degeneration of intraepidermal nerve fibers. In contrast t
