[Anesthesia and postoperative recovery for parathyroid gland surgery].

Anesthesia for surgery of primary hyperparathyroidism (HPT) usually concerns asymptomatic elderly women with moderate hypercalcemia. Cardiovascular repercussions of the endocrine disorder are possible, but they are not frequent except for hypertension. Hyperparathyroid crisis is a life-threatening condition with severe hypercalcemia. Intravenous diphosphonates are very effective drugs to control hypercalcemia. The improvement is transient but allows curative parathyroidectomy to be performed with a minimal risk of cardiac arrhythmias. Anesthesia for surgery of secondary HPT concerns patients w…

Osteonecrosis of the Jaw in Two Rheumatoid Arthritis Patients Not Treated with a Bisphosphonate.

Medication-related osteonecrosis of the jaw (MRONJ) is a side effect in patients taking bone-modifying agents (BMAs), which are highly beneficial for treating osteoporosis and cancer. Bisphosphonates are prescribed to treat secondary osteoporosis in patients with rheumatoid arthritis (RA). We recently encountered two unusual cases of intraoral ONJ in RA patients who had not been treated with a BMA and did not have features of methotrexate- associated lymphoproliferative disorder. Their ONJ stage II bone exposures were treated by conservative therapy, providing good prognoses. These cases indic

Interest of Bone Histomorphometry in Bone Pathophysiology Investigation: Foundation, Present, and Future.

Despite the development of non-invasive methods, bone histomorphometry remains the only method to analyze bone at the tissue and cell levels. Quantitative analysis of transiliac bone sections requires strict methodologic conditions but since its foundation more 60 years ago, this methodology has progressed. Our purpose was to review the evolution of bone histomorphometry over the years and its contribution to the knowledge of bone tissue metabolism under normal and pathological conditions and the understanding of the action mechanisms of therapeutic drugs in humans. The two main applications o

Refractory serositis in Gorham-Stout syndrome.

BACKGROUND: Gorham-Stout syndrome (GSS) is a rare disorder with various presentations and unpredictable prognoses. Previous understandings of GSS mainly focused on progressive bone destruction, while we identified a group of GSS patients with serous effusion as the first symptom. This study aimed to investigate the clinical characteristics of patients with GSS having serous effusion as the first symptom. METHODS: Patients diagnosed with GSS were identified through the Peking Union Medical College Hospital Medical Record System. The demographic, clinical, laboratory, and imaging data were coll

[A Rare Cause of Hypercalcemia].

A Rare Cause of Hypercalcemia Abstract. Sarcoidosis is a rare cause of hypercalcemia, which is in most cases caused by primary hyperparathyroidism or a malignant tumor. Other rare causes include vitamin D intoxication, other granulomatous diseases (such as tuberculosis, or fungal infection) or mutiple myeloma. Because of the broad differential diagnosis, a systematic diagnostic approach is crucial to establish the correct diagnosis. Therapeutic measures include volume replacement and subsequent administration of bisphosphonates. If hypercalcemia is caused by sarcoidosis, therapy with systemic

Upregulation of mmu_circ_0001066 attenuates the inhibitory effect of bisphosphonates on osteoclastogenesis.

OBJECTIVE: Medication-related osteonecrosis of the jaw (MRONJ) is the main adverse side effect of bisphosphonates (BPs), mainly owing to the inhibitory effect of BPs on osteoclastogenesis. CircRNAs were identified to be an important factor in regulating cellular processes. The aim of this study was to explore the effect of mmu_circ_0001066 on BP-inhibited osteoclastogenesis. MATERIALS AND METHODS: The expression of MRONJ-related miRNA in RANKL-induced RAW264.7 cells treated with BP was analyzed using qRT-PCR analysis. Bioinformatics techniques were applied to screen potential circRNAs. Tartra

Effect of Bisphosphonates on the Osteogenic Activity of Osteoprogenitor Cells Cultured on Titanium Surfaces.

PURPOSE: This study investigated the effects of bisphosphonates, namely, alendronate and zoledronate, on the osteogenic activity of osteoprogenitor cells cultured on titanium surfaces at therapeutic doses in order to assess if altered osteoblastogenesis could compromise osseointegration and contribute to etiopathogenesis of painful disorders such as bisphosphonates-related osteonecrosis of the jaw (BRONJ) following implant placement. MATERIALS AND METHODS: MC3T3-E1 Subclone 4 cells were utilized in this study. Therapeutic doses of alendronate and zoledronate were calculated based on reported

Bisphosphonate related osteonecrosis of the jaws.

Bisphosphonates are used widely for a variety of bone conditions, most notably intravenous bisphosphonate in the treatment of metastatic bone lesions and multiple myeloma and oral bisphosphonates for osteoporosis. Recently, a peculiar form of osteonecrosis limited to the jaws has been discovered especially with the use of intravenous bisphosphonates. We review briefly the mechanism of action of these drugs, the clinical features and staging of the disease, prevention strategies and management options.

Osteonecrosis of the jaw and the role of bisphosphonates: a critical review.

Osteonecrosis of the jaw (ONJ), a condition characterized by necrotic exposed bone in the maxillofacial region, has been reported in patients with cancer receiving bisphosphonate therapy, and rarely in patients with postmenopausal osteoporosis or Paget disease of bone receiving such therapy. In the absence of a uniform definition, the American Academy of Oral and Maxillofacial Surgeons (AAOMS), the American Society for Bone and Mineral Research (ASBMR), and other groups have established similar diagnostic criteria for bisphosphonate-related ONJ, which is more commonly reported in patients with

[Muscle and bone health as a risk factor of fall among the elderly. Prevention of fracture and bisphosphonate].

Fracture risk reduction in patients with osteoporosis by nitrogen containing bisphosphonate (N-BP) has been well confirmed in the daily practices. N-BPs suppress bone resorption by inhibiting farnesyl diphosphate synthase in mevalonate pathway, reducing protein-prenylation, consequently disrupting the membrance structure in osteoclasts. A once-yearly infusion of zoledronic acid during 3-year period significantly reduced the risk of vertebral fracture by 70% and that of hip fracture by 41% in patients with postmenopausal osteoporosis at the mean age of 73. The annual infusion within 90 days aft

Anabolic effects of bisphosphonates on peri-implant bone stock.

The long-term durability of total joint replacements is critically dependent on adequate peri-implant bone stock, which can be compromised by wear debris-mediated osteolysis. This study investigated the effects of bisphosphonates on enhancing peri-implant bone in the presence of clinically relevant ultra-high molecular weight polyethylene (UHMWPE) wear debris. Fiber-mesh coated titanium-alloy plugs were implanted bilaterally in the femoral condyles of 36 New Zealand white rabbits. Implants in the left femora were covered with submicron UHMWPE particles during surgery. Rabbits were administered

Treatment of osteoporosis with bisphosphonates.

Several bisphosphonates appear to be effective for preventing bone loss associated with estrogen deficiency, glucocorticoid treatment, and immobilization and for partially reversing bone loss in patients with postmenopausal osteoporosis and steroid-induced osteoporosis. The most promising of these agents are etidronate, alendronate, tiludronate, and risedronate. These agents should have an important role in prevention and treatment of osteoporosis; however, more research is needed regarding optimal doses and regimens (continuous versus intermittent), comparisons with other agents, and use in c
