Percutaneous coronary intervention through a Cabrol composite graft.

There are several different surgical techniques used to repair acute type-A aortic dissections. One such technique is called the Cabrol procedure which uses a Dacron tube graft to re-attach the native coronary arteries. Stenosis of this graft-native coronary anastomosis is a recognized complication of the procedure and one amenable to percutaneous intervention. However, knowledge of the complex anatomy involved with the Cabrol repair is required in order to potentially carry out successful intervention. We report one such case involving a critical stenosis at the anastomosis of the Cabrol graf

Dissecting aneurysm of the right middle cerebral artery in a young patient: A case report.

Dissecting aneurysms are common in early stages of life, and minimally invasive intervention is recommended to reduce the risk of complications due to the challenge of identifying structures with a higher risk of bleeding. An 18-year-old patient presented with a dissecting aneurysm of the right middle cerebral artery, characterized by a poorly defined neck and a high risk of rupture. Endovascular treatment with a flow-diverting stent and coils was performed, successfully correcting the lesion without complications. Recent advances in endovascular therapy allow precise localization of blood ves

Percutaneous stenting of a dissected superior mesenteric artery in a patient with previous surgical repair of Stanford type A aortic dissection.

BACKGROUND/AIM: We report a case of a 54-year-old male patient with background history of hypertension, which suffered a Stanford type A thoraco-abdominal aortic dissection with extension to the visceral arteries. DESCRIPTION OF CASE: The patient initially underwent surgical repair with replacement of the ascending aorta and of the hemiarch in the acute phase of the dissection. Postoperatively, he developed non-specific abdominal pain that was not related to meals but led to weight loss of 20 kg within the first five post-operative months. Follow-up computerized tomography scan revealed a chr

Prosthetic valve sparing aortic root replacement: an improved technique.

We describe a modified surgical technique to treat patients with a previous history of isolated aortic valve replacement who now require aortic root replacement for an aneurysmal or dissected aorta. This technique consists of replacing the aortic root with a Dacron conduit, leaving intact the previously implanted prosthesis, and re-implanting the coronary arteries in the Dacron graft. Our technique differs from other techniques in that we do not leave behind any aortic tissue remnant and also in that we use a felt strip to obliterate any gap between the old sewing ring and the newly implanted

Suitability Analysis of 2 Devices for Endovascular Aortic Arch Repair Following Type A Aortic Dissection Surgery.

OBJECTIVES: Endovascular aortic arch repair (Ar-TEVAR) may be a viable option for patients with residual arch pathology after surgery for type A acute aortic dissection (ATAAD). However, anatomical limitations can hinder its suitability. The primary outcome of this retrospective, dual-centre study aimed to assess the suitability of Ar-TEVAR using 2 branched devices in patients with prior ATAAD repair. METHODS: Patients who underwent surgery for ATAAD at 2 centres between January 2012 and September 2023 were evaluated. The inclusion criteria were prior surgery limited to ascending and/or hemia

Catheter-induced Left Main Dissection: A Minefield in Interventional Cardiology.

Coronary angiography is an invasive diagnostic procedure used to assess the coronary anatomy. Although rare, iatrogenic coronary artery dissection during coronary catheterization is a dreaded complication. Here we report the case of an 89-year-old patient diagnosed with acute coronary syndrome-Non-ST-segment elevation myocardial infarction, who underwent coronary angiography. During the coronary angiogram, he sustained a fatal left main coronary artery dissection. Here we discuss the interventions attempted to tackle the situation and a review of the approach to managing iatrogenic left main d

[A case of surgical repair of aortic dissection associated with tear of the right coronary artery].

A 39-year-old female with dissecting aortic aneurysm of DeBakey type I, underwent replacement of the ascending aorta. We found that the right coronary artery originated just distally from the left sinus of Valsalva, run transversally in the aortic wall. Because the entry of dissection occurred at the right coronary artery, it was torn longitudinally. Aorto (graft)-rt. coronary bypass grafting was performed. She had no ischemic heart symptoms postoperatively. It is supposed that the dissecting aortic aneurysm was caused by the anomalous origin of the right coronary artery.

[Early Outcomes of Total Arch Replacement Using an Integrated Frozen Elephant Trunk for Acute Type A Aortic Dissection].

In Japan, surgical repair for Stanford type A acute aortic dissection(AAAD)has been increasingly performed, accompanied by gradual improvement in early outcomes, While ascending or hemiarch replacement remains the mainstream strategy, late aneurysmal dilatation of the residual dissected aorta is still a concern, The frozen elephant trunk(FET)technique, which facilitates aortic remodeling, has been increasingly adopted in AAAD surgery, Since the introduction of a domestic integrated four-branched FET device in late 2022, expectations have risen for improved procedural safety and simplicity, We

Emergency ascending aortic replacement for acute type a aortic dissection in a 31-week pregnant patient with Marfan syndrome.

BACKGROUND: Acute type A aortic dissection during pregnancy is a rare but life-threatening condition, with an estimated incidence of approximately 4 cases per 1 million women aged 15-45 years. Hormonal and hemodynamic changes during pregnancy are factors that may predispose to aortic dissection. These changes begin in the first and second trimesters but are most pronounced in the third trimester and the early postpartum period. Genetic syndromes, such as Marfan syndrome, significantly increase the risk of aortic dissection. CASE PRESENTATION: A 41-year-old pregnant woman at 31 weeks of gestat

[Bleeding atheroma. A nuclear magnetic resonance tomography differential diagnosis of dissection of the carotid artery].

Improved methods of imaging the cerebral vascular system have increased the detection of traumatic or spontaneous dissection of the vertebral and carotid arteries. Especially MRI allows direct demonstration of intramural hematomas that are a sign of dissection of the vessel wall. We report on a patient who had an acute onset of dysarthria and monocular blurred vision, which recurred several times. Doppler ultrasound showed stenosis in both internal carotid arteries with reduced velocity of the flow; angiography confirmed the stenosis, showing a long stretch of stenosis (70%) of the right and a

Reconstruction of radiation-induced ulcers with free flaps using the perforating vessel as a recipient vessel.

BACKGROUND: Free flaps can be options for coverage of radiation ulcers. However, due to radiation damage, it may be hard to find and dissect a reliable recipient vessel for microsurgical anastomosis. When the radiation fields are targeted for deep tissues, superficial tissues may be less affected by radiation. Therefore, damage to perforator vessels near the skin may be lesser than that to the major vessels in deeper structures. We would like to introduce our experiences of using these less injured perforating vessels within or near the wound as recipient vessels for free flap coverage. PATIE

Artery first approach in robotic pancreatoduodenectomy: right-sided uncinate first technique.

Several advances have been made since the first days of pancreatic surgery and the Whipple-Kausch procedure: the artery-first approach and TRIANGLE resection for pancreatic cancer of the head have been reported in different variations for open surgery to achieve a complete R0 resection. Since this is one of the main factors for oncologic outcome, it is essential to transfer these techniques to the newest iteration of surgical evolution: robotic pancreatoduodenectomy has shown its comparability to the open approach in the last few years. In this paper, we describe our technique of robotic uncin

Acute type A aortic dissection complicated by acute pancreatitis and abdominal hemorrhage: a case report of following exploratory laparotomy.

BACKGROUND: There are few reports of postoperative acute pancreatitis (AP) in patients with acute type A aortic dissection (ATAAD), but we reported a case of ATAAD complicated by AP and later reoperation for intraperitoneal hemorrhage. We discussed the causes of AP in this patient and summarized some of the experiences of postoperative management of patients with ATAAD involving the celiac trunk and/or superior mesenteric artery. CASE PRESENTATIONS: A 44-year-old male patient was diagnosed with ATAAD and underwent partial resection of the ascending aorta with graft replacement and total aorti

Aortic root replacement versus preservation in acute type A aortic dissection repair: meta-analysis of reconstructed time-to-event data.

BACKGROUND: Outcomes of aortic root replacement (ARR) versus conservative root approach (CRA) in patients undergoing acute type A aortic dissection (ATAAD) repair remained controversy. METHODS: The present study was a pooled meta-analysis of Kaplan-Meier-derived individual patient data (IPD) from comparative studies published by September 29, 2024. RESULTS: Forty studies met our eligibility criteria, comprising 11,734 patients (4212 in the ARR group and 7522 in the CRA group). In the overall population, the overall survival was similar between the ARR and CRA groups (hazard ratio [HR], 0.95;

Aortic elastic fiber degeneration during acute type a aortic dissection and reverse aortic remodeling.

BACKGROUND: Progression of proximal or distal aortic dilatation is defined as reverse aortic remodeling after surgery for acute type A aortic dissection (ATAAD) that may be dependent on aortic wall degeneration. METHODS: We investigated whether aortic wall degeneration is associated with reverse aortic remodeling leading to aortic reoperation after surgery for ATAAD. Altogether, 141 consecutive patients undergoing surgery for ATAAD at Tampere were evaluated. The resected ascending aortic wall at surgery was processed for 42 degenerative, atherosclerotic and inflammatory histological variables

Reinforcement of suture lines with adventitial eversion in acute type A aortic dissection.

TECHNIQUE: The reinforcement of the suture lines in acute type A aortic dissection include the treatment of proximal and distal anastomoses. The intima of the proximal site is transected circumferentially, approximately 1.0 cm above the sinotubular junction. The adventitia is folded outwards along the cutting edge of the intima, and the eversion forms an overlap. An autologous pericardial strip is placed inside the aorta as a mattress and secured with 4-0 prolene running sutures to the adventitial eversion overlap to reinforce the proximal cuff without any glue. The distal aortic cuff is trimme

Hemodynamic Implications of STABILISE Technique for Aortic Dissection Repair.

BACKGROUND: The stent-assisted balloon-induced intimal disruption and relamination (STABILISE) technique for treatment of type B dissection has shown promising clinical results at mid-term. Computational modeling is a way of noninvasively obtaining hemodynamic effects, such as pressure and wall shear stress, leading to a better understanding of potential benefits. Particular areas of interest are (1) the effect of intimal disruption and re-lamination and (2) the effect of the bare metal stent in the visceral aortic segment. METHODS: Single-center prospective case series. Data from 5 consecuti
