Pylephlebitis With Splenic and Mesenteric Vein Thrombosis in a Patient With Diverticulitis.

Diverticulitis is a common gastrointestinal complaint that refers to inflammation of colonic diverticula. Its incidence has increased partly due to the increase in prevalence of diverticulosis, which results from poor dietary habits and chronic constipation. An acute diverticulitis episode can vary in severity, ranging from outpatient management of mild abdominal discomfort to inpatient admission requiring emergent surgery. Some common complications associated with diverticulitis include bowel wall perforation, microperforation, abscess formation, bowel obstruction, and colonic fistulas. A les…

Acute cholangitis in the context of Lemmel syndrome with signs of diverticulitis.

Here, we present the case of a 51-year-old male with pain in the right hypochondrium, jaundice, fever, chills and septic shock. A computed tomography (CT) scan showed a juxtapapillary diverticulum with inflammatory changes, which caused bile duct ectasia and left biliary radicals. We considered this to be a septic shock of a biliary origin due to the cholangitis associated with Lemmel syndrome, with signs of juxtapapillary diverticulitis.

Laparoscopic Lavage in the Management of Hinchey III/IV Diverticulitis.

Diverticulitis manifestations may cover a spectrum of mild local inflammation to diffuse feculent peritonitis. Up to 35% of patients presenting with diverticulitis will have purulent (Hinchey grade III) or feculent (Hinchey grade IV) contamination of the abdomen, with a high-associated morbidity and mortality. Surgical management may involve segmental resection with or without restoration of bowel continuity. However, emergency resection for diverticulitis can be associated with high mortality rates, as well as low stoma reversal rates at 1 year. Therefore, laparoscopic peritoneal lavage has b…

Colonic diverticulitis in children: A retrospective study of 16 patients.

BACKGROUND: Pediatric colonic diverticulitis (CD) is a rare entity. This study aimed to investigate the clinical features of CD in children. METHODS: We performed a retrospective chart review of children aged ≤15 years who were diagnosed with CD in our institution from May 2006 to November 2016. RESULTS: Sixteen patients were diagnosed with CD. All CD cases were observed to be solitary cecal diverticulitis; 14 cases were detected using ultrasound and the other two cases were diagnosed by computed tomography. Five patients were male (31.3%), and the median age was 12 years (range, 8-15 years)…

The role of quantitative radiological measures of visceral adiposity in diverticulitis.

BACKGROUND: Diverticular disease has been linked to obesity. Recent studies have assessed the role of visceral adiposity with diverticulitis and its complications. The aim of this study was to evaluate the association of quantitative radiological measures of visceral adiposity in patients with diverticulitis with vital signs, biochemistry results, uncomplicated versus complicated diverticulitis and its interventions. METHODS: A retrospective analysis of all patients with diverticulitis admitted from November 2015 to April 2018 at a single institution was performed. Data collected included dem…

Management outcomes of diverticulitis and colitis in patients with active cancer.

BACKGROUND: Little is reported in the literature on management strategies and outcomes in patients with an active cancer diagnosis who undergo emergent general surgery. The purpose of this study is to evaluate preoperative risk factors in both operative and non-operative management, as well as to describe the outcomes of colonic emergencies within a cancer patient population. METHODS: A single institution cancer database was reviewed retrospectively to identify patients with an active cancer diagnosis who had an emergency general surgery consult placed for an acute colonic pathology. RESULTS

Diverticulitis of the appendix as debut of appendicular cystadenoma and carcinoid tumor.

Appendiceal diverticulosis is a rare condition with a frequency of 0.004% to 2.1%, and is sometimes an occasional finding during anatomopathologic study of the surgical specimen. It may be presented acutely as a right lower quadrant pain, similar to acute appendicitis for which differential diagnosis must be carried out, and it is associated with appendicular tumors. We report a case of diverticulitis of the appendix as debut of appendicular cystadenoma and carcinoid tumor with representative iconography, being unusual the association of both diseases and even more its preoperative diagnosis.

Sigmoid resection for diverticulitis is more difficult than for malignancies.

PURPOSE: Sigmoid resection for diverticulitis is usually the first procedure performed when starting the learning process for laparoscopic colorectal surgery. The aim of this study is to evaluate the difficulty of laparoscopic sigmoid resection for diverticulitis in comparison to sigmoid malignancy in order to assess its role in the residents training program. METHODS: A cohort of patients was selected who suffered either from malignancy or recurrent diverticulitis in the sigmoid colon. Laparoscopic sigmoid resection was performed. The degree of difficulty was assessed by intraoperative compl…

Perforated jejunal diverticulitis in a nonagenarian veteran: A case report.

INTRODUCTION: Jejunal diverticular (JD) disease is an uncommon occurrence that frequently present as a diagnostic dilemma. The correct diagnosis from complications of JD is typically made at laparotomy. Most JD are asymptomatic. Of clinically significant small bowel diverticula, only 15% present with surgical problems including obstruction, gastrointestinal bleed, and perforation. PRESENTATION OF CASE: A 90-year-old man presented to the hospital with abdominal pain. He was clinically stable with local tenderness in the left lower quadrant. Computed tomography demonstrated a dot of free air ne

Small bowel diverticulitis: an imaging review of an uncommon entity.

In this review, we discuss the clinical and radiologic findings of small bowel diverticulosis, which is infrequently encountered during practice and far less common than colonic diverticulosis. Small bowel diverticulosis can present with a range of emergent symptomatic complications including diverticulitis, perforation, or hemorrhage. Here, we focus on the clinical features, pathogenesis, radiologic findings, and treatment of small bowel diverticulitis. Although not routinely considered in the differential diagnosis of an acute abdomen, prospective radiologic diagnosis of small bowel divertic…

Anaphylaxis to Polyethylene Glycol (Colyte®) in a Patient with Diverticulitis.

Polyethylene glycols (PEGs) are believed to be chemically inert agents, but larger PEG polymers could have immunogenicity. A 39-year-old man … gastrointestinal tract. However, the loss of mucosal integrity at gastrointestinal membrane such as diverticulitis may be a predisposing factor for anaphylaxis to Colyte®. We report a case of anaphylaxis induced by the ingestion of PEG-3350 in a patient with diverticulitis which might be a risk factor of anaphylaxis.

Application of a modified Neff classification to patients with uncomplicated diverticulitis.

AIM: Severity of acute diverticulitis (AD) has traditionally been assessed using the Hinchey classification; however, this classification is predominantly a surgical one. The Neff classification provides an alternative classification based on CT findings. The aim of this study was to evaluate a modification of the Neff classification to select patients presenting with early-stage AD to receive outpatient management. METHOD: All patients with AD, presenting to a single unit, were prospectively studied. All patients underwent emergency abdominal CT and were assigned a Neff stage, including a mo…

[Caecal diverticulitis in a young woman with suspected acute appendicitis].

Colonic diverticular disease is common amongst the western population in the left hemicolon while right-sided involvement is rare. This case concerns a 20-year-old female, who was hospitalized with suspected acute appendicitis. Perioperative findings included an intramural tumour at the base of the appendix, and an ileocaecal resection was performed. Histopathological examinations revealed a single inflamed diverticulum of the caecum. The case illustrates the pre- and perioperatively differential diagnostic problems of caecal diverticulitis, which can lead to more radical surgery than needed.

A rare but distinctive cause of acute abdomen: appendiceal diverticulitis.

BACKGROUND: Diverticulosis and diverticulitis of appendix vermiformis is a rare diagnosis. Clinical and laboratory examinations do not show a difference between a progressing diverticulitis and simplex appendicitis. But this entity has a higher mortality rate than common appendicitis. OBJECTIVE: This case is presented to illustrate the point that preoperative simple diagnosis of acute appendicitis according to the clinical signs and physical examination may not only be insufficient, but could be fatal in certain circumstances, like appendiceal diverticulitis, if surgical intervention is delay…

Killian-Jamieson diverticulitis with cervical cellulitis: report of a case.

A Killian-Jamieson (K-J) diverticulum is an uncommon hypopharyngeal diverticulum related to the better-recognized Zenker's diverticulum. Cervical cellulitis due to K-J diverticulitis is also highly exceptional. We report the case of a 53-year-old woman with cervical cellulitis caused by K-J diverticulitis. The cellulitis was cured by the administration of an antibiotic agent. The patient underwent a resection of the K-J diverticulum 2 months after the cellulitis was cured. The cervical diverticulum was judged to be a K-J diverticulum because the diverticulum prolapsed laterally just below the …

A case of Meckel's diverticulitis treated on the assumption of Crohn's disease.

Meckel's diverticulum is the most common congenital abnormality of the gastrointestinal system. It usually has an asymptomatic course and is detected incidentally during laparoscopy. Symptomatic cases are mostly observed under two years of age. In adults, it may rarely manifest with life-threatening complications. In this report, we present a 19-year-old case with Meckel's diverticulum operated with ileus while under followup, who was assumed to have Crohn's disease.

Colonic diverticulitis in adolescents: an index case and associated syndromes.

Diverticular disease of the colon, a common problem among adults, is diagnosed rarely in children. We report an adolescent patient with sigmoid diverticulitis who required operative treatment. Pediatric patients with the complications of diverticula typically have conditions that result in genetic alterations affecting the components of the colonic wall. Our patient had Williams-Beuren syndrome, although Ehlers-Danlos syndrome, Marfan syndrome, and cystic fibrosis may also be associated with colonic diverticula in adolescence. Pediatric patients with these disorders who experience abdominal pa…
