[Pyoderma gangrenosum over a saphenectomy scar in a cANCA-positive patient with diverticulitis].

Pyoderma gangrenosum is a chronic, painful, ulcerative skin disease of unknown etiology. In 50 % of the cases, it is associated with systemic disease, primarily with inflammatory bowel disease. It can develop over areas of trauma or surgical wounds, possibly because of local immunoreactant deposits. We describe the case of a cANCA-positive patient with colonic diverticulitis who developed pyoderma gangrenosum over a saphenectomy scar.

Surgical outcomes in elective sigmoid resection for diverticulitis stratified according to indication: a propensity-score matched cohort study with 903 patients.

OBJECTIVE: Weighing the perioperative risk of elective sigmoidectomy is done regardless of the specific diverticulitis classification. The aim of this study is to evaluate surgical outcomes according to the classification grade and the indication. METHODS: All patients who underwent elective colonic resection for diverticulitis during the ten-year study period were included. They were divided into two groups: relative surgery indication (RSI) and absolute surgery indication (ASI). RSI included microabscess and recurrent uncomplicated disease. ASI included macroabscess and recurrent complicate

Conservative Management of Perforated Diverticulitis in Adults: An Updated Systematic Review of Outcomes.

BACKGROUND: The management of perforated diverticulitis (PD) has shifted over the last decade, moving from aggressive surgery toward non-operative strategies for hemodynamically stable patients. However, the safety of non-operative treatment in cases with extraluminal air, particularly Hinchey Ia-II disease, remains debated. This systematic review evaluates the efficacy, safety, and long-term outcomes of non-operative management for PD and identifies radiologic predictors of treatment failure. METHODS: A systematic review was conducted per PRISMA 2020 guidelines. PubMed/MEDLINE, Europe PMC, S

Peanut-Related Perforated Diverticulitis Before the Age of 60.

We present a case in which a 55-year-old male with a past medical history of systemic lupus erythematosus (SLE) and rheumatoid arthritis (RA) presented with sharp, worsening right-sided abdominal pain radiating across the entire abdomen after eating peanuts. Computed tomography (CT) imaging showed evidence of acute sigmoid diverticulitis complicated by a walled-off perforation. The patient's past medical history suggested previous recurrent episodes of diverticulitis. Our patient underwent exploratory laparotomy, sigmoid colon resection with low anterior anastomosis and proctocolectomy, and lo

[Diverticulosis].

Diverticula of the colon are very common in developed countries. In most of those affected diverticulosis is an asymptomatic condition. Only 20% complain of recurrent symptoms. Symptomatic diverticulosis is common only after the age of 50. Dietary treatment combined with antispasmodics is generally sufficient. Diverticular disease is complicated by bleeding or by diverticulitis in 20% of symptomatic patients. Diverticulitis itself can be complicated by intra-abdominal abscess, fistulas, perforation with generalized peritonitis or bowel obstruction. Each clinical appearance is described and tre

[The outcome of conservative treatment of diverticular disease (author's transl)].

Patients with diverticular disease should be categorized for therapeutic purposes into 3 groups: (1) asymptomatic diverticulosis (accidental finding; (2) painful diverticular disease (no inflammation); and (3) acute diverticulitis (pericolitis present). In asymptomatic diverticulosis a high-fiber diet should be given, and an operation is not indicated. In painful diverticular disease resection of the sigmoid colon should be considered after recurrent attacks. In acute diverticulitis resection should be performed after the second inflammatory attack. The younger the patient, the earlier an oper

[Solitary ulcer of the colonic hepatic flexure].

The nonspecific colon ulcer is a not a well-known disorder. A case of ulcer of the colic hepatic flexure is described. It was a case of pseudotumor and the pathological examination confirmed the diagnosis. The precise diagnosis of colon ulcer is useful for conservative treatment with coloscopic surveillance and to prevent a hemorrhagic complication or peritonitis after perforation. Contrary to diverticulitis, this pathology is most dominant on the right colon and particularly on the cecum. This explains the frequency of pseudoappendicular syndromes.

Risk Prediction in Diverticulitis.

Acute diverticulitis is a common sequela of colonic diverticulosis that often poses a significant dilemma for surgeons and patients considering surgical management of the disease. Importantly, the decision to recommend surgery for patients with diverticular disease hinges on identifying those who will benefit most in terms of preventing future disease-related complications. This review focuses on identifying patient risk factors that are predictive of onset of diverticulitis, recurrence of disease, and progression to complicated disease, such as abscess, fistula, or stricture, and aims to assi

Challenges Associated With Diverticular Bleeding in an Elderly Female Patient With Situs Inversus Totalis: A Case Report and Literature Review.

Diverticulitis is a relatively common condition in gastroenterology. It is most often presented by inflammation of one or more diverticula, depending on their presence in the colon, and by abdominal pain and bleeding. Bleeding from the diverticulum has a wide range of clinical manifestations, which in a certain percentage of cases can have a very unfavorable course and prognosis. Situs inversus totalis (SIT) is a congenital condition with multiple, predominantly positional abnormalities of the organs in relation to their anatomical positions. Unlike bleeding from the diverticulum, SIT occurs v

Helicobacter cinaedi bacteremia in four renal transplant patients: clinical features and an important suggestion regarding the route of infection.

Helicobacter cinaedi can cause bacteremia mainly in immunocompromised patients. We present the clinical characteristics of H. cinaedi bacteremia in 4 renal transplant patients. Interestingly, all cases showed triggers of bacterial translocation: 2 cases developed after colonic perforation caused by diverticulitis, 1 case developed post cholecystectomy, and the remaining patient had chronic diarrhea. Accordingly, bacterial translocation caused by severe gastrointestinal complication could be a cause of H. cinaedi bacteremia.

A Rare Case of Diverticulitis With a Colo-Adnexal Fistula.

Diverticulitis is a very common cause of hospitalization in the United States with fistula formation being a common complication. However, a fistula formation between the sigmoid colon and an ovary is an exceptional rarity. We present a case of a 22-year-old female with a colo-ovarian fistula misdiagnosed as a tubo-ovarian abscess after a recent episode of diverticulitis. Initial workup, imaging studies, and treatment ending with Hartmann's procedure with eventual colostomy reversal are described. A review of similar cases within the literature and fistulas is also presented.

Anastomotic Considerations in Diverticulitis.

Diverticulitis is a common indication for colorectal surgery, both in the acute and the elective setting. The anastomosis between the colon and rectum is a critical component of colectomy for diverticular disease and should be approached thoughtfully. This article reviews important surgical considerations when creating a colorectal anastomosis in the setting of diverticular disease, whether following the reversal of an end colostomy, during an acute episode of diverticulitis, or electively for chronic or complicated disease. Timing of surgery and preoperative assessment, minimally invasive app

The left lateral view in the plain-film assessment of abdominal distension.

Since the left lateral position facilitates the entry of air into the rectosigmoid, it has been employed in the plain-film evaluation of patients with abdominal distension. The value of this view is illustrated by examples of patients with (a) colonic ileus due to imipramine hydrochloride (Tofranil) or chlorpromazine, (b) partial mechanical obstruction due to diverticulitis, (c) involvement of the rectum by Hirschsprung's disease, and (d) sequential evaluation of a patient with clindamycin colitis. This simple maneuver is recommended for (a) evaluation of patients with plain-film evidence of l

Italian guidelines for the diagnosis and management of colonic diverticulosis and diverticular disease.

Colonic diverticulosis and diverticular disease are among the most common gastrointestinal disorders encountered in clinical practice. These Italian guidelines focus on the diagnosis and management of diverticulosis and diverticular disease in the adult population, providing practical and evidence-based recommendations for clinicians. Experts from five Italian scientific societies, constituting a multidisciplinary panel, conducted a comprehensive review of meta-analyses, systematic reviews, randomised controlled trials, and observational studies to formulate 14 PICO questions. The assessment o

[Acute solitary diverticulitis of the cecum, a rare cause of acute abdomen. Apropos of 9 cases and review of the literature].

The natural history of colonic diverticulitis rarely includes acute solitary localization in the cecum. Complications are even more rarely seen. We report retrospectively nine cases seen in our unit. There were 5 males and 4 females, mean age 54 years. The patients presented with a more or less painful abdomen with localization in the right iliac region and signs of peritoneal irritation. Physical examination revealed a painful mass in this zone. Fever and hyperleukocytosis were not found in all patients. Barium enema demonstrated signs of pseudocompression of the cecum in four cases suggestin

Sequential pancreatitis and diverticulitis following elective anterior lumbar disc replacement: a case report of a rare postoperative domino effect.

INTRODUCTION: Anterior lumbar disc replacement (LDR) is a motion-preserving alternative to fusion for degenerative disc disease but carries risk of vascular and visceral injury. We report the first documented case of a sequential "domino effect" of acute postoperative pancreatitis followed by perforated diverticulitis after LDR surgery. CASE PRESENTATION: A 66-year-old man with diabetes underwent elective L2-L3 LDR via a left anterior retroperitoneal approach. Although initially stable, he developed acute pancreatitis on postoperative day (POD) 1, confirmed by elevated enzymes and CT findings

When the Ileum Mimics Appendicitis: A Case of Uncomplicated Ileal Diverticulitis.

Although colonic and duodenal diverticulosis is common, ileal diverticular disease is rare. The pathophysiology of ileal diverticula is poorly understood, but the most widely accepted hypothesis currently suggests abnormalities in the myenteric plexus or smooth muscle, high intraluminal pressure, and intestinal dyskinesia. Diagnosis is often delayed and difficult because the clinical presentation is nonspecific, and it is mainly imaging that allows the diagnosis to be established and guided. In the event of complications such as perforation, it can be life-threatening because of delays in diag
