INTRODUCTION: The aim of the study was to identify the factors that influence grief among donor families regarding organ donation.
METHODS: This cross-sectional study, utilizing an analytical approach, employed a convenience sampling method to gather data from family members. Over the three years, all families who consented to organ donation were invited to participate, from all, 222 questionnaires were completed by donor family members who had given consent and successfully proceeded with the donation. We collected data using the Grief Experience Questionnaire (GEQ-34). Descriptive and analy

PURPOSE: No standard exists for reconstruction after extralevator abdominoperineal excision (ELAPE) and pelvic exenteration. We propose a tailored concept with the use of bilateral gluteal V-Y advancement flaps in non-extended ELAPE and with vertical myocutaneous rectus abdominis muscle (VRAM) flaps in extended procedures. This retrospective study analyzes the feasibility of this concept.
PATIENTS AND METHODS: We retrieved all consecutive patients after ELAPE or pelvic exenteration for rectal, anal, or vulva cancer with flap repair from a prospective database. Perineal wound complications wer

BACKGROUND: The concept of "ownership" in global health policymaking broadly assumes that external donors and advisors expect recipient countries to actively steward their national programmes when assistance is provided. This study specifically explored country ownership of national policy decision-making for Plasmodium vivax malaria using three case studies: Laos, Pakistan and the Solomon Islands.
METHODS: Yin's comparative case study model, Kingdon's Multiple Streams policy theory guided this analysis. From 2021 to 2024, interviews were undertaken with 29 national stakeholders, external par

Advances in microsurgery together with improvements in reconstructive surgical techniques over recent decades have enlarged the scope of available techniques for mutilated hand reconstruction, shifting the reconstructive paradigm from restoring hand function to providing the best functional and aesthetic results with minimal donor-site morbidity. Successful reconstruction of a mutilated hand should no longer be measured only by the degree of improvement of hand function but also by a more aesthetic hand appearance as well as by improved psychological well-being. In this article, the authors pr

BACKGROUND: Reactivation of latent viruses such as human cytomegalovirus (HCMV) after allogeneic hematopoietic stem cell transplantation (HSCT) results in high morbidity and mortality. Effective immunization against HCMV shortly after allo-HSCT is an unmet clinical need due to delayed adaptive T cell development. Donor-derived dendritic cells (DCs) have a critical participation in stimulation of naïve T cells and immune reconstitution, and therefore adoptive DC therapy could be used to protect patients after HSCT. However, previous methods for ex vivo generation of adoptive donor-derived DCs w

Sri Lanka's kidney transplantation programme has matured within a resource-constrained public health system and has achieved short-term outcomes that appear comparable to published ranges from similar middle-income programmes, despite major limitations in real-time transplant immunobiology support. However, increasing recipient sensitisation, prolonged dialysis vintage, and a growing deceased-donor programme demand more reliable immunological risk assessment to improve equity and graft outcomes. This pragmatic narrative review summarises the minimum immunobiology package that can support safer

Prolonged cold storage (CS) of donor kidneys results in poor outcomes after transplantation. We reported earlier that cold storage (CS) of rat kidneys for 18 h followed by transplantation (CS + Tx) reduces proteasome function, disrupts protein homeostasis, and compromises graft function. The goal of the present study was to define the contribution of specific heat shock proteins (Hsp) to CS-induced disruption of renal graft function and determine the benefit conferred by their pharmacological inhibition. We subjected kidneys isolated from donor Lewis rats to 18-h CS with or without pharmacolog

Natural killer (NK) cells are increasingly recognized as central effectors in kidney allograft injury, extending beyond their traditional role in innate immunity. This narrative review summarizes current evidence linking NK cells to microvascular inflammation, with a focus on both antibody-dependent and antibody-independent rejection. In antibody-mediated rejection, NK cells may be activated through Fc gamma receptor IIIa (CD16a) engagement with endothelium-bound donor-specific antibodies, leading to antibody-dependent cellular cytotoxicity and proinflammatory cytokine release. In parallel, em

Fecal microbiota transplantation (FMT) has become a powerful experimental tool for dissecting microbiota-driven mechanisms in murine models of gastrointestinal and systemic disease. This review provides a comprehensive methodological and translational overview of FMT in mice, focusing on lessons learned from inflammatory bowel disease (IBD) research and emerging perspectives in short bowel syndrome (SBS). We first outline the fundamental role of the gut microbiota in immune regulation, metabolic homeostasis, and maintenance of epithelial barrier integrity, establishing the rationale for modula

Over the past 20 years, endothelial keratoplasty procedures have revolutionized the treatment of corneal endothelial disorders. These conditions have now become the leading indication for corneal transplantation in Western countries and account for half of all donor cornea usage. Despite their undeniable success, the global shortage of donor tissues and major disparities between nations justify the development of alternatives to donor grafts. Cell therapy using injections of suspended endothelial cells has proven effective, and tissue-engineered endothelial keratoplasty (TEEK), comprising a me

UNLABELLED: The discovery of the air-impermeable pre-Descemet's layer prompted discussions on revising the microanatomy of the cornea, enabled classification of the DALK (deep anterior lamellar keratoplasty) technique based on the location of the bubble relative to the pre-Descemet's layer (PDL), and led to the introduction of a new type of Descemet membrane transplantation - pre-Descemet's endothelial keratoplasty (PDEK). We believe that if it is possible to isolate the PDL, then it can be used it as an analogue (or imitation) of the corneal epithelial basement membrane to separate epithelial

PURPOSE: To limit the burden of long-term immunosuppression (IS) after uterus transplantation (UTx), removal of the uterine allograft is indicated after maximum two pregnancies. Hitherto this has required graft hysterectomy by laparotomy. Our objective was to demonstrate, as a proof of concept, the feasibility of less traumatic transplantectomy by total laparoscopic hysterectomy (TLH).

BACKGROUND: Conventional cell-based bone regeneration suffers from the major disadvantage of limited cell supply, time-consuming in vitro expansion cultures, and limited patient-friendliness related to cell isolation and multiple visits to the clinic. Here, we utilized an alternative concept using "easy access cells" that can be obtained in an intraoperative manner to prepare cell-based constructs.
METHODS: We used stromal vascular fraction (SVF) from human adipose tissue and human monocytes for intraoperative preparation of bone constructs. Conventional constructs grafted with expanded human

The chronic shortage of human transplants to treat tissue and organ failure has led to the development of xenotransplantation, the transplantation of cells, tissues and organs from another species to human recipients. For a number of reasons, pigs are best suited as donor animals. Successful, routine xenotransplantation would have an enormous impact on the health of the human population, including the young, who sometimes require a replacement organ or islet cells, but especially the elderly, who more often suffer the consequences of organ failure. The first form of xenotransplantation applied

INTRODUCTION: Lymphedema remains a challenging clinical problem. A new field of lymphatic surgery using micro and super microsurgery techniques is a rapidly advancing field aimed to treat recalcitrant cases. The objective of this study was to evaluate outcomes and complications of vascularized lymph node transfer (VLNT). Several early preliminary studies have reported promising outcomes, but they are limited by small numbers, short follow-up, and are inconsistent in addressing the origin and recipient site of the transferred lymph nodes as well as the donor site morbidity.
METHODS: A review o

Organ transplantation is an effective treatment for chronic organ dysfunctioning conditions. However, a dearth of available donor organs for transplantation leads to the death of numerous patients waiting for a suitable organ donor. The potential of decellularized scaffolds, derived from native tissues or organs in the form of scaffolds has been evolved as a promising approach in tissue-regenerative medicine for translating functional organ replacements. In recent years, donor organs, such as heart, liver, lung and kidneys, have been reported to provide acellular extracellular matrix (ECM)-bas

Since 1995, keratinocytes are grown into cultures and used as allografts for the coverage of deep dermal defects in our burn unit. Donor skin samples are mostly acquired from other burn patients. In addition, special methods of skin preservation allow us the use of skin, which has been taken in redundancy for split thickness skin grafting from nonburned patients.Thirty five patients with deep partial thickness burns in the face were treated since 1996 according to the following concept: Dermabrasion or tangential excision was performed before the 5(th) day following trauma. Hematopoietic stem and progenitor cell (HSPC) transplantation is a cornerstone for studying hematopoiesis. However, classical conditioning regimens such as irradiation or chemotherapy induce strong inflammation, alter the bone marrow (BM) microenvironment, and severely limit the interpretation of differentiation processes. Moreover, donor HSPC engraftment efficiency in immunocompetent recipients without conditioning is usually very low. In this work, we produced and purified the monoclonal anti-c-Kit antibody ACK2 and tested its capacity to transiently deplete HSPCs in immunocompetent C57BL/6

INTRODUCTION: Pancreaticoduodenectomy (PD) is a complex operation with high postoperative morbidity, largely driven by pancreaticojejunostomy (PJ) leak. In 2021, US general surgery residency graduates performed only 9.5 ± 7 pancreatic operations, suggesting a need for additional experience with complex pancreatic anastomosis. We hypothesized that human deceased donor pancreas and jejunum could be utilized to create a high-fidelity PJ training experience.
METHODS: Faculty pancreas surgeons were surveyed to determine factors influencing resident participation in PJ and expected impact of a proc
