Drowning is a relatively common cause of accidental death in children. Drowning in sea water is an unusual cause of severe hypernatremia. We report the case of a 3.5-year-old boy who died 11h after drowning in sea water, with a serum sodium level of 178 mmoL/L. In this case, hypernatremia was aggravated by diarrhea and hyperglycemia with glycosuria. Usually, correction of acute hypernatremia must be quick and early, aiming at a reduction of serum sodium concentration of up to 1-2 mmoL per liter per hour.

Sudden death during whitewater recreation often occurs through understandable mechanisms such as underwater entrapment or trauma, but poorly defined events are common, particularly in colder water. These uncharacterized tragedies are frequently called flush drownings by whitewater enthusiasts. We believe the condition referred to as cold water immersion syndrome may be responsible for some of these deaths. Given this assumption, the physiologic alterations contributing to cold water immersion syndrome are reviewed with an emphasis on those factors pertinent to flush drowning.

Issues of drowning or near drowning in aquatic sporting have received considerable attention in the literature; however, there is not a lot of attention to nonsubmersion injuries such as water tubing. Water tubing is similar to water skiing but with much less control by the rider, leaving the rider at the mercy of the driver or any obstacle in their path. This report describes unusual events surrounding the injury of a 5-year-old boy enjoying water fun with his father on his boat.
