Does the use of antipyretics in children who have acute infections prolong febrile illness? A systematic review and meta-analysis.

OBJECTIVE: To review the literature and test the hypothesis that the use of antipyretic drugs in children with acute infections slows recovery. STUDY DESIGN: A systematic review and meta-analysis of the literature was undertaken to investigate the effect of antipyretic drugs upon recovery from infectious diseases in children. A search of Medline (1946 until November 2012) and EMBASE (1980 until November 1, 2012) was undertaken to identify studies in which the authors compared the use of antipyretic medications with nonpharmacologic treatments for fever. RESULTS: Six papers were identified, 5

[An experimental study of possibility of treatment of hemorrhagic fever Marburg by Remicade].

The effect of Remicade, administered according to different schemes, produced on the course of experimental Marburg hemorrhagic fever was studied. When the drug was used from the first infection day, the treated animals died reliably earlier versus the controls (infected animals). A reliably lower concentration of TNF-alpha in blood serum (versus the controls) was registered on day 3. At the same time, when Remicade was used from day 3 after infection, a 50% survival of animals was registered. It is noteworthy, that the TNF-alpha concentration in blood serum did not differ, on day 3, from that

Antimicrobial Sensitivity Pattern of Salmonella Typhi: Emergence of Resistant Strains.

Background Typhoid fever is still an important public health problem in developing countries. Increasing resistance of Salmonella Typhi to antibiotics is alarming. New extensively drug-resistant strains of Salmonella reported first time in Pakistan, resistant not only to first-line drugs and ciprofloxacin but also resistant to ceftriaxone, had spread globally, including the USA. Due to this continuously changing pattern of antimicrobial resistance in typhoid fever due to Salmonella Typhi, there is a substantial need to study the resistance pattern of Salmonella Typhi frequently in different ar

[Comparison of the transcriptional profiles of patients with dengue fever and dengue hemorrhagic fever reveals differences in the immune response and clues in immunopathogenesis].

INTRODUCTION: Dengue infection demonstrates a wide spectrum of clinical manifestations from mild disease (dengue fever) to severe dengue hemorrhagic fever, but the immunopathogenic mechanisms involved in disease severity are not clear. OBJECTIVE: Differentially expressed genes associated to immune response were identified from peripheral blood mononuclear cells of Colombian children with dengue fever and dengue hemorrhagic fever. MATERIALS AND METHODS: Microarray analysis was used as a tool to establish and compare transcriptional profiles of peripheral blood mononuclear cells of six childre

Voriconazole pseudotoxicity: a case report highlighting the importance of sampling timing in TDM interpretation by clinical pharmacists.

This case report describes a 57-year-old male patient who sought medical attention due to severe pneumonia and respiratory failure. The initial empirical antibiotic treatment was ineffective. Bronchoscopy and lung alveolar lavage fluid culture confirmed a case of Aspergillus fumigatus infection. Antifungal treatment with the conventional dose of VRZ was administered, but the patient continued to have a fever, and the trough concentration of VRZ detected exceeded the upper limit of the normal range. The clinical pharmacist discovered that the blood sample collected for TDM of this patient was t

Identification of a Direct-Acting Antiviral Agent Targeting RNA Helicase via a Graphene Oxide Nanobiosensor.

Dengue virus (DENV), an arbovirus transmitted by mosquitoes, causes infectious diseases such as dengue fever, dengue hemorrhagic fever, and dengue shock syndrome. Despite the dangers posed by DENV, there are no approved antiviral drugs for treatment of DENV infection. Considering the potential for a global dengue outbreak, rapid development of antiviral agents against DENV infections is crucial as a preemptive measure; thus, the selection of apparent drug targets, such as the viral enzymes involved in the viral life cycle, is recommended. Helicase, a potential drug target in DENV, is a crucial

Antimicrobial Resistance in Typhoidal Salmonella in Nepal: Surveillance for Enteric Fever, 2016-2019.

BACKGROUND: Enteric fever (caused by Salmonella enterica) has been associated with poor hygiene and is endemic in the South-Asian countries. The increase in resistance to first line antimicrobials has been observed, while the emergence of multi/extremely drug resistance cases have been identified in several countries. The objective of this study is to analyze the current trend of antimicrobial resistance in Salmonella isolates in Nepal, and to identify the status of multi- and extremely- drug resistant isolates. METHODS: We recruited individuals at study hospitals with suspected enteric fever

Treatment failure in typhoid fever with ciprofloxacin susceptible Salmonella enterica serotype Typhi.

Fluoroquinolones are considered the most effective drugs for the treatment of typhoid fever and hence are widely used in the empiric treatment of acute undifferentiated febrile illnesses in India. Recent reports of Salmonella enterica serotype Typhi (S. Typhi) strains with increasing minimum inhibitory concentration (MIC) of ciprofloxacin have raised the fear of potential treatment failures. In this case series of 109 consecutive patients hospitalized with typhoid fever (S. Typhi grown from blood), we documented clinical failure (fever persisting >6 days) in 25 of 46 (54.3%) adults who could b

Cyclooxygenase inhibitors inhibit antibody response through interference with MAPK/ERK pathways and BLIMP-1 inhibition.

Fever is a common symptom of illness in children, and although not harmful in itself, fever and its associated symptoms are often treated with antipyretic drugs. A number of national and other guidelines now recommend against their routine use; a conclusion that was initially supported by a study showing that the prophylactic use of paracetamol might reduce antibody response to some vaccine antigens, although data from booster vaccinations are more equivocal. Although in vivo data on the cause of this inhibition are scarce, in vitro data suggests that the cause may be due to inhibition of the

[Surveillance of drug resistance in pathogenic enterobacteria in Algeria. I. Study of the resistance of major and minor Salmonella species to antibiotics in 1973-1974].

901 Salmonella strains were examined in our laboratory between October 1973 and August 1974 for their sensitivity to antibiotics. The aim of this study was, on the one hand to detect the possible emergence of drug resistance in strains of Salmonella responsible for typhoid and paratyphoid fevers; on the other hand to follow up, as closely as possible, the occurrence of drug resistance in strains of other Salmonella serotypes isolated in infant and child populations; and, in addition, to awaken practitioners to the problems inherent to the widespread use of antibiotics in these diseases. The 75

Drug-induced systemic lupus erythematosus in interferon beta-1b therapy.

Drug-induced systemic lupus erythematosus (SLE) is a rare complication of therapies with some drugs. Breaking out after months or years of therapy with a certain drug, its occurrence is likely to increase with the duration of the medication and the cumulative quantity of the drug. The symptoms of this syndrome include, in particular, arthralgia, myalgia, fever, serositis, skin exanthema and production of antinuclear (ANA) antibodies. In contrast to SLE, its symptoms gradually abate after discontinuation of the inducing agent. The authors describe the case of a 43-year-old patient suffering fro

Antibiotic dispensing by drug store personnel in Bangkok, Thailand.

The current pattern of antibiotic use by drug store personnel in Bangkok was examined. Ten well-trained medical students (simulated patients) presented to 40 randomly selected drug stores with common complaints, namely urethral discharge, acute watery diarrhoea, fever with sore throat, coryza, skin infection and acute dysuria. Analysis of medications obtained revealed that 50-100% of drug stores dispensed antibiotics for each condition. Co-trimoxazole, ampicillin, chloramphenicol, penicillin V and tetracycline were commonly given. Most antibiotics were dispensed inappropriately with respect to

Acute pulmonary toxicity to nitrofurantoin.

Nitrofurantoin is a widely prescribed antibiotic used for the treatment of urinary tract infections. In some patients it can produce an acute pulmonary reaction ranging from mild dyspnea to noncardiogenic pulmonary edema. Symptoms include fever, dyspnea, chills, cough, and chest pain. Physical examination generally reveals an acutely ill, extremely apprehensive patient in varying degrees of respiratory distress. Fever is usually present and there is an increase in heart rate and respiratory rate. Cyanosis, rales, and a maculopapular rash are common findings. Laboratory studies typically demons

Erythema induratum of bazin - Skin lesions with pyrexia of unknown origin undiagnosed for 4 years.

Erythema induratum of Bazin is characterized by chronic, tender, erythematous, indurated subcutaneous nodules on the lower extremities caused by tuberculin hypersensitivity. A 21-year-old woman presented with recurrent episodes of multiple erythematous scaly lesions over both lower limbs below the knee and low-grade fever for 4 years. She was treated from various outside hospitals with multiple courses of antibiotics and nonsteroidal anti-inflammatory drugs without improvement. The histopathology of the skin lesion was suggestive of erythema induratum. She had complete resolution of her skin l

Candida Dubliniensis Tricuspid Valve Endocarditis.

Infective endocarditis is a common complication in patients using intravenous drugs. Fungal endocarditis is a rare clinical entity and diagnosis are often delayed. Only a few cases of candida dubliniensis have been reported in the literature so far. These cases usually present in patients with a history of intravenous drug abuse. If a patient with valvular endocarditis continues to have fever despite antibiotics, then fungal infection should be suspected. Fungal endocarditis is usually treated surgically along with a prolonged course of antifungal treatment. We report an interesting case of a

Agranulocytosis following phenytoin-induced hypersensitivity syndrome.

Phenytoin, one of the most common antiepileptic drugs, is a major cause of antiepileptic drug hypersensitivity syndrome (AHS), which is a rare but potentially fatal complication. We herein report a 5-year-old boy who developed unexpected agranulocytosis with fever approximately one week after recovering from the typical symptoms of AHS, characterized by fever, rash, lymphadenopathy, and hepatitis, but lacking eosinophilia or lymphocytosis. High-dose steroid therapy for the former symptoms of AHS, and immunoglobulin, granulocyte colony-stimulating factor, and cefepime for the latter agranulocyt

Plasmid mediated multidrug resistance in Salmonella typhi.

Between September 1989 and February 1991, strains of S. typhi showing multiple drug resistance were isolated from blood cultures of patients with typhoid fever. A total of 283 isolates were obtained over a period of 18 months. Forty four (11%) of these isolates were resistant to chloramphenicol, ampicillin, co-trimoxazole and tetracycline, the first three being drugs currently used in treating typhoid fever. Forty of the 44 multi-resistant strains tested showed transfer of resistance 'en block' to recipient Escherichia coli K 12 (J62-2). All 44 multi-resistant strains were sensitive to ciprofl
