An urgent open surgical approach for left ventricle venting during peripheral veno-arterial extracorporeal membrane oxygenation for refractory cardiac arrest: case report.

The objective of the study is to describe an emergency procedure for left ventricle venting during veno-arterial extracorporeal life support for refractory cardiac arrest. Veno-arterial extracorporeal membrane oxygenation is widely used in refractory cardiac arrest but is characterized by an increase in left ventricle afterload, which may impair cardiac contractility improvement. Different left ventricle venting techniques are available. We report the use of a surgical approach with sternotomy for left ventricle venting in a 21-year-old patient who was placed under veno-arterial extracorporeal

Validation of transpulmonary thermodilution variables in hemodynamically stable patients with heart diseases.

BACKGROUND: Transpulmonary thermodilution is recommended in the treatment of critically ill patients presenting with complex shock. However, so far it has not been validated in hemodynamically stable patients with heart disease. METHODS: We assessed the validity of cardiac output, global end-diastolic volume index (GEDVI), an established marker of preload thought to reflect the volume of all four heart chambers, global ejection fraction (GEF) and cardiac function index (CFI) as variables of cardiac function, and extravascular lung water index (EVLWI) as indicator of pulmonary edema in 29 pati

Heart Morphogenesis Requires Smyd1b for Proper Incorporation of the Second Heart Field in Zebrafish.

Background/Objectives: Abnormal development of the second heart field significantly contributes to congenital heart defects, often caused by disruptions in tightly regulated molecular pathways. Smyd1, a gene encoding a protein with SET and MYND domains, is essential for heart and skeletal muscle development. Mutations in SMYD1 result in severe cardiac malformations and misregulation of Hand2 expression in mammals. This study examines the role of Smyd1b in zebrafish cardiac morphogenesis to elucidate its function and the mechanisms underlying congenital heart defects. Methods: Smyd1b (still hea

Cardiac Magnetic Resonance Characterizes Myocarditis in a 16-Year-Old Female With Lyme Disease.

Myocarditis may occur during early disseminated Lyme disease. A 16-year-old girl with serologic evidence of Borrelia burgdorferi infection and transient first-degree atrioventricular block underwent cardiac magnetic resonance imaging, which demonstrated myocardial hyperemia, edema, and delayed gadolinium enhancement. We discuss the use of T1- and T2-weighted dark blood sequences in addition to inversion recovery delayed enhancement imaging to support the diagnosis of Lyme myocarditis.

Reduced coronary blood flow in cardiac tamponade: mystery solved.

A 67-year-old male presented with several week history of progressive shortness of breath, lower extremity edema, and distended jugular veins. Transthoracic echocardiography showed moderate pericardial effusion with thickened visceral pericardium and septal bounce but no chamber collapse. Right and left cardiac catheterization showed discordance of the right and left ventricular systolic pressures during respiration and severely reduced cardiac output. There was near equalization of diastolic pressures in all four chambers suggestive of effusive-constrictive pericarditis with cardiac tamponade

The Use of Bedside Echocardiography to Diagnose Post-COVID Cardiomyopathy and Left Ventricular Thrombus in the Emergency Department.

This case report chronicles a 47-year-old male with no known past medical history, who presented to the emergency department with a chief complaint of progressive dyspnea and lower extremity edema. The patient was previously healthy until he contracted COVID-19 approximately six months prior to the date of presentation. He made a full recovery two weeks later. However, in the ensuing months, he progressively declined with worsening shortness of breath and lower extremity edema. On outpatient cardiology evaluation, he was found to have cardiomegaly on chest radiograph and sinus tachycardia on e

Fluid therapy in the acute brain injured patient.

Adequate fluid therapy in the acute brain injured (ABI) patient is essential for maintaining an adequate brain and systemic physiology and preventing intra- and extracranial complications. The target of euvolemia, implying avoidance of both hypovolemia and fluid overloading (or "hypervolemia," by definition associated with fluid extravasation leading to tissue edema) is of key importance. Primary brain injury can be aggravated by secondary brain injury and systemic deterioration through diverse pathways which can challenge appropriate fluid management, e.g. neuroendocrine and electrolyte disor

The Heart of the Matter: Secondary Hypogammaglobulinemia and Constrictive Pericarditis.

Constrictive pericarditis is the final common result of a number of processes that affect the pericardium. Establishing the diagnosis and determining the underlying etiology of constrictive pericarditis are often a diagnostic rendezvous. Here, we describe a patient who presented to the general practitioner with edema, ascites, and weight gain and was found to have constrictive pericarditis secondary to an inflammatory myofibroblastic tumor of the mediastinum. Interestingly, she had a relative lack of cardiorespiratory complaints, and, aside from the edema and mildly elevated jugular venous pre

Mechanism of death in massive fluid infusion.

To determine the effects of infusion of large volumes of crystalloid, a series of dogs was subjected to intravenous injection of oleic acid and moderate hypovolemic shock for 1 hour. They were then resuscitated with 50 cc/kg of Ringer's lactate over 1 hour in one group and 2 hours in the other. Eight of nine receiving the fast infusion died during the first 2 days, and seven of eight receiving the slow infusion lived for the full 3 days of the study. The dogs that died all had large intrapulmonary shunts and gross pulmonary edema at autopsy. Both groups received the same total amount of fluid.

[Mendelson's syndrome in coronary care units (author's transl)].

By means of the case of a patient resuscitated after cardiac arrest attention is brought to the occurring of Mendelson's syndrome in coronary care units. Aspiration pneumonitis following inhalation of acid gastric contents is little known outside of anesthesia, but is likely to occur more frequent than its diagnosis is made. A considerable black number is to be expected, mostly because of the danger that the aspiration itself has not been registered and the pulmonary edema setting in after a latency of several hours is regarded as sequela of the underlying heart disease. But this misunderstand

Acute clinical presentation of nonischemic cardiomyopathies: early detection by cardiovascular magnetic resonance.

Nonischemic cardiomyopathies include a wide range of dilated, hypertrophic and arrhythmogenic heart muscle disorders, not explained by coronary artery disease, hypertension, valvular or congenital heart disease. Advances in medical treatments and the availability of implantable cardioverter defibrillators to prevent sudden cardiac death have allowed a substantial increase in the survival of affected individuals, thus making early diagnosis and tailored treatment mandatory. The characterization of cardiomyopathies has received a great boost from the recent advances in cardiovascular magnetic re

Hamartoma of mature cardiomyocytes in right atrium: A case report and literature review.

RATIONALE: Tumors in the heart are rare. Myxomas, rhabdomyomas, and fibromas are the most common benign cardiac tumors. Hamartoma of mature cardiomyocytes (HMCM) is another benign cardiac tumor, are very rare and have only been reported in a few literatures. PATIENT CONCERNS: We report a case of 41-year-old male who suffered short of breath for 3 years, and lower limbs edema for 2 years. DIAGNOSES: Transthoracic echocardiogram (TTE) and cardiac magnetic resonance (CMR) showed a large amount of pericardial effusion and confirmed a mass of 18 × 14 mm on the superior vena cava near the outer ed

Attenuation of ECG voltage in cirrhotic patients.

Two patients with cirrhosis (CIR), ascites (ASC), and peripheral oedema (PEROD) are presented. They were followed for many years, had multiple hospital admissions and clinic evaluations, had repeated laboratory testing, and many ECGs recorded. As their condition worsened, they developed attenuation of the ECG voltage (ATTECGV), which was more pronounced during intervals of increased fluid overload. Attenuation of the ECG voltage was decreased at times of successful diuresis and amelioration of PEROD, while abdominal paracenteses for ASC did not have any influence on the ATTECGV, suggesting tha

Subclinical pulmonary oedema and intermittent haemodialysis.

It has been postulated that patients with chronic renal failure, even in the absence of cardiopulmonary symptoms, accumulate interstitial pulmonary fluid, which is removed by haemodialysis. To test this hypothesis we used the indocyanine green (ICG)-heavy water double indicator dilution method to measure lung water, cardiac output, and central blood volume in relation to haemodialysis. Ten uraemic patients, without cardiopulmonary symptoms, were investigated at the beginning and end, and 2 h after, a regular dialysis session. A group of 18 surgical patients about to undergo elective abdominal

[Restrictive ostium secundum: a new fetal malformation syndrome].

Echocardiographic examination of a 23 weeks' hydropic fetus disclosed abnormal kinetics of the valve of the foramen ovale. This valve was constantly bulging out, dome-like, into the left atrium, and on TM-mode tracings in transatrial projection the alpha and beta peaks which occur respectively during the opening and closure of the atrioventricular valves throughout fetal life were missing. This anomaly suggested that the ostium secundum was restrictive; the foramen ovale itself was not restrictive. Such abnormal kinetics have not been encountered among 16 other cases of hydrops fetalis of card

Sudden death in a dog after doxorubicin chemotherapy.

A case is reported of fatal cardiomyopathy in an 8-year-old female German Shepherd after standard chemotherapy with doxorubicin for splenic hemangiosarcoma. The main gross lesion was a moderate bilateral cardiac ventricular dilation with diffusely pale myocardium. Histological analysis revealed severe multifocal vacuolar degeneration of cardiomyocytes, myocytolysis, myofibril loss, myocardial fibrosis, and edema. Myocardial fiber vacuolization and myocytolysis were highly suggestive of doxorubicin cardiotoxicity.

Metachronous severe multisystem immune-related adverse events during tremelimumab plus durvalumab therapy for hepatocellular carcinoma: a clinicopathological autopsy case report.

Immune checkpoint inhibitors (ICIs) have improved outcomes in advanced hepatocellular carcinoma (HCC), but severe multisystem immune-related adverse events (irAEs) remain a major clinical challenge. We report an autopsy-confirmed case of advanced HCC treated with tremelimumab plus durvalumab that developed severe metachronous multisystem irAEs characterized by sequential pancreatitis, laryngeal edema with sialadenitis, and myocarditis. Twelve days after treatment initiation, the patient presented with fever and abdominal distension, and computed tomography showed CTCAE grade 3 pancreatitis. On

The Dyspnea Dilemma: Immune Checkpoint Inhibitor-Associated Myocarditis Mimicking Overlapping Cardiac Pathologies-A Case Report.

BACKGROUND: Immune checkpoint inhibitors (ICIs) such as durvalumab improve survival in solid tumors but can rarely cause fulminant myocarditis (reported in 1% of cases, with 25%-40% mortality). Clinical overlap with coronary artery disease (CAD), chemotherapy-related cardiomyopathy, or malignancy-associated complications makes diagnosis challenging. CASE PRESENTATION: A 62-year-old woman with gallbladder adenocarcinoma on gemcitabine, cisplatin, and durvalumab presented with 1 week of progressive dyspnea, leg edema, and chest pain. She had a history of portal vein thrombosis on apixaban. On e
